    Observation. An incident was noted in the minor incident log book where a child was injured and then transported by the parent to see a dentist from the facility due to the injury. The incident was not reported to Licensing as required. Any injury to a child that requires emergency care by a medical practitioner is required to be reported to Licensing within 24 hours of occurrence as per Schedule H of the Child Care Licensing Regulation. Submit the incident report to Licensing by October 23, 2025. Care plans were observed to be on file for multiple children receiving care. The plans did not contain information on what specifically will be implemented by the staff or at the facility for each child requiring extra support. The plans did not contain evidence that they were developed in consultation with the parent. Care plans are intended to guide employees with specific information regarding a child's care who require extra support. The plan is required to be developed in consultation with the parent and any person requested by the parent. A response is required by October 30, 2025 to confirm care plans are complete.

    Observation. Staff files are incomplete. One file is missing character references, two files are missing evidence that the person has complied with the Province's immunization control program. This is a reoccurring contravention originally found out of compliance on June 8, 2022. A person must not be employed until the manager has obtained the required records to ensure the person is suitable to work with children. Confirm all staff files are complete and outline the system developed to ensure the required records have been obtained. One employee does not have a cleared criminal record check on file. This is a reoccurring contravention originally found out of compliance on September 19, 2023. Any person over the age of 12 must have a cleared criminal record check if they are ordinarily present in a facility. Confirm the cleared criminal record check has been received and outline the system to ensure criminal record checks are on file and kept up to date. It was observed that the emergency plan was not practiced by employees within the past year. Every employee must practice implementing the emergency plan at least once per year. Submit the system developed to ensure emergency plans are practiced by employees as required.

    Observation. Through a review of a sampling of children's files, one file is missing the name and telephone number of an emergency contact other than the parents. This is a reoccurring contravention originally found out of compliance on June 8, 2022 and September 19, 2023. All required information needs to be on file for children who are in attendance at the facility. Outline the system developed to ensure children's files are complete. It was disclosed that two children who require extra support do not have care plans developed by the facility. Care plans are required to be developed in consultation with the parents for any child who requires any type of extra support. Outline the system created to ensure care plans are developed for any child who requires extra support. It was disclosed that play space inspections are occurring however are not documented. A log of playspace inspections, maintenance and repairs is required to be kept on file. Outline the system developed to document playspace inspections as required.
