Observation. Care plans for children requiring extra support have not been reviewed in the past year. This is a reoccurring contravention originally found out of compliance on July 15, 2024. Each care plan for a child requiring extra support is to be reviewed at least once per year with the parent. This will ensure information is relevant to the health and safety of the child. A response is required by October 14, 2025 to confirm all care plans have been reviewed and contain evidence of the review.

Observation. Children in the Group Child Care (School Age) programs are mixing together and sharing indoor program spaces while in operation at the same time. It was disclosed that the cubby area and sink are shared between the two programs. Care programs are not to combine in any capacity while utilizing indoor licensed spaces. Outline the system developed to ensure the School Age programs operate independently as per Schedule E of the Child Care Regulation.
- C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical
Observation. It was observed that the emergency drill has not been practiced in the past year. Every employee must practice implementing the emergency plan at least once a year and provide evidence of the implementation. Outline the system developed to ensure the emergency plan is reviewed and documented at least once per year.

Observation. One child's care plan has not been updated since 2020. The manager disclosed the plan has been verbally reviewed on a regular basis however has not been documented to reflect discussions. Care plans need to be reviewed and evidence of review documented at least once a year. Outline the system developed to ensure care plans are reviewed in consultation and documented once per year. It was disclosed that children in the Group Child Care (School Age) program move between the two licensed programs and the attendance is not updated to reflect the change. Attendance needs to be updated each time a child enters or leaves any care program. Outline the system developed to ensure that attendance is accurate for each care program, documenting how many children are present and who is in attendance.

Observation. In reviewing staff files one staff ECE certification has expired.

Observation. It was observed during inspection on two separate instances children from the 30 months to School Age program going inside the daycare without supervision from staff while the rest of the group was playing outside.

Observation. Outdoor play space inspections are not occurring or documented on a daily basis.

Observation. A log of minor accidents, illness and unexpected events was not available in one Group Child Care (30 months to school age) program and the School Age program. All of the children's files were missing the date on which the child stops attending the facility, one child's file was missing the medical insurance plan number.

Observation. Staffing files were reviewed for new staff since last routine inspection. Missing from both files are an immunization document.

Observation. Observations: System in place to ensure children's records are complete. FLO tested system and found child files to be complete, log of minor injuries/incidents unexpected events updated regularly, play space inspections recorded appropriately, children's arrival and departure times consistently recorded. Contravention: A review of the minor incident/injury/unexpected event log indicated an incident occurred that needed to be reported to Licensing.
- C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical
Observation. In the Group Child Care (30 months to School Age) outdoor play area there is a hole that has been dug under the fence. The resilient surfacing under the tire swing needs redistributing and/or replenishing. In the Group Child Care (<36 months) there were several gaps that exceeded 3.5 " - underneath the gate in the chain link fence and at the bottom of a shorter plank of the wooden fence
