  A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

  Observation. The fire extinguishers required servicing by 28/Feb/2026.

  Corrective action. Please obtain servicing for the fire extinguishers.

• RECORDS AND REPORTING

  The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.

  Observation. Child A's care plan had no parental signature or date. Child B's care plan was last reviewed by the parent on 05/Sep/2024.

  Corrective action. Child A's care plan was reviewed by, & signed/dated by their parent during this inspection. The manager explained that Child B's care plan will be signed upon their return to the facility next week.

• STAFFING

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

  Observation. An employee's CRC is addressed to another organization.

  Corrective action. CRCs must be licensee & site specific. In the meantime, this employee cannot work alone w/ children or assist w/ diapering/toileting.

• EQUIPMENT AND FURNISHINGS

  A licensee must have one toilet and wash basin for every 10 children or fewer.

  Observation. LO observed and discussed the following with the Manager: 1 toilet stall in one of the GCC (301) classroom is not used and accessible to the children. This stall is used as a storage.

  Corrective action. Ensure that both toilets are available and accessible to children in GCC (301) classroom.

  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. LO observed and discussed the following with the Manager: - Nap space: floor length curtains used in all nap spaces accessible to the children (posing potential entanglement and suffocation hazard), with limited continuos supervision during nap time. - Outdoor play area: geo dome climbers more than 3 ft in height are placed in 2 outdoor play areas used by children from GCC (301) under 36 months old. - Bathrooms: mops are hanged in each bathroom stalls, wet/dirty mop heads are accessible to the children.

  Corrective action. Ensure that a healthy and safe environment is provided at all times while children are in care.

• STAFFING

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.

  Observation. 10 staff files were reviewed; 1 staff's immunization status was missing.

  Corrective action. Ensure that all staff files are complete.

  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. During the inspection the following was observed: -Two blind cords were not secured. This could cause a potential hazard. This is a repeat contravention from inspection #CLAN-CTPSPW -A storage cupboard located in a nap room had the key in the lock. This could cause a potential hazard as this was at the children's level -Several stereo cords were not secured

  Corrective action. Please ensure to monitor the facility regularly to ensure a healthy and safe environment is provided

  A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

  Observation. Licensing observed the following during the inspection: -The kitchen door was easy to open and a chemical was located under the sink. This is a repeat contravention from inspection #CLAN-CTPSPW -Plastic bags holding new diapers were located on the ground in the washroom. This could present a potential hazard.

  Corrective action. Please ensure children do not have access to any objects or substances that may be hazardous to their health and safety.

• EQUIPMENT AND FURNISHINGS

  A licensee must supply equipment, furniture and supplies that are (b) located so as not to block or hamper an exit in the case of fire or other emergency.

  Observation. Licensing observed a stool and easel and a table partially blocking an exit in the classroom. In addition Licensing observed the emergency exit in the kitchen was blocked by food warmers/coolers.

  Corrective action. Please ensure all exits and entrances remain clear to be prepared in case of an emergency evacuation.

  A licensee must have one toilet and wash basin for every 10 children or fewer. Observation (CORRECTED DURING INSPECTION): Licensing observed the following: -One toilet was not accessible to the children in care. The stall door was closed. There were 16 children in care. This is a repeat contravention from inspection #CLAN-CTPSPW

  Corrective action. In discussion the staff stated the toilet was blocked and she wanted to prevent the children from using the toilet. Licensing recommended staff contact the Manager. The toilet was fixed.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. The carpets in the rooms were observed. The carpets were stained and debris was observed on the carpets. This is a repeat contravention from inspections #CLAN-CF5QXZ and #CLAN-CTPSPW

  Corrective action. In discussion with the staff they stated the carpets are vacuumed each night and steam cleaned each month. Licensing recommends a more frequent cleaning schedule be put into place.

• MEDICATION

  If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (a) administered to the child in the amount and at the times specified by the child's parent or in the child's record or care plan.

  Observation. Licensing observed the following: -Medication for a child that was expired. -A care plan in place was not signed by the parents and a medical authorization form was not completed.

  Corrective action. Please ensure to regularly check expiry dates of medication and ensure authorizations for medication are completed and signed.

  If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (b) readily accessible to employees.

  Observation. A child requires medication in an emergency. Licensing observed the child was present in the facility while the medication was left in the outdoor play area.

  Corrective action. Please ensure that employees have access to emergency medication at all times to ensure the health and safety of the children in care.

  A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

  Observation. During the inspection the children's records were reviewed. One child was missing an emergency contact other than the parent. This is a repeat contravention from inspection #CLAN-CF5QXZ

  Corrective action. Please ensure regular checks of children's records are completed to ensure there is no missing information

  A licensee must keep, for each child, a record showing the following information: (h) any special instruction respecting the child's diet, medication, participation in a program of activities, or other matter relevant to the child's care, (i) given by the child's parent to the licensee in writing.

  Observation. Licensing observed the children's records. One child's record was missing written documentation from parents regarding drop off and pick up.

  Corrective action. Please ensure children's records contain current information and documentation from parents

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. During the inspection Licensing observed the following inside the facility: -Blind cords were not secured in two rooms. Please correct this by July 13, 2023 -Several sleep cots were stacked on top of a cabinet. On top of the cots were several items. This could cause a potential hazard. During the inspection Licensing observed the following outside the facility: -Sharp edges were observed in the metal trim and fencing in the outdoor play area -A climber was observed close to a fixed climbing structure. This was corrected during the inspection.

  Corrective action. Please ensure a healthy and safe environment is provided to the children at all times.

  A licensee must have one toilet and wash basin for every 10 children or fewer.

  Observation. During the inspection the washrooms were observed. In one washroom stall a door to one of the toilets was locked and items were stored on and beside the toilet. In another washroom stall buckets and mops were stored and the stall door was closed.

  Corrective action. Please ensure that all of the toilets that the facility is licensed for are accessible to the children in care.

  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. Licensing observed several carpets in the rooms with stains and debris. In discussion with the Manager she stated the carpets are vacuumed regularly and steam cleaned every 3 months. This is a repeat contravention from inspection #CLAN-CF5QXZ

  Corrective action. Please ensure the facility and equipment and furnishings are clean.

  A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

  Observation. Licensing observed the following: -An open fish tank with a chair propped next to it. This could cause a potential hazard. -A chair was propped up to the counter and bottles with bleach and water were within reach -The door to the kitchen was easy to open. Chemicals were observed under the sink -A staff bag was accessible. This was corrected during the inspection -Several cupboards had keys placed in them. One cupboard was accessible and contained staff bags. This was corrected during the inspection. -A bag with strings was observed under a sleep cot in the nap room. -Hand sanitizer and cream was within reach in a child's cubby.

  Corrective action. Please ensure the children do not have access to any object or substance that may be hazardous to their health and safety.

• MEDICATION

  If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (b) readily accessible to employees.

  Observation. A care plan was observed in the facility. Licensing requested to see where the medication was stored. The medication was not present in the facility.

  Corrective action. Please ensure medication required for a child in an emergency is located in the facility and accessible to employees.

  The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.

  Observation. Several care plans were not reviewed annually with the parent or any person requested to by the parent.

  Corrective action. Please ensure that children's care plans are reviewed annually and that this is documented.

• A licensee must ensure that a child, while under the care or supervision of the licensee, is not subjected to any of the following: (c) harsh, belittling or degrading treatment by an employee or another child, whether verbal, emotional or physical, that could humiliate the child or undermine the child's self respect.

  Observation. Through an interview with the Manager and the staff and a review of evidence, Licensing determined a staff member subjected a child to harsh, belittling and degrading treatment in numerous instances.

  Corrective action. Corrective action plan was discussed

• A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee.

  Observation. Through an interview with the Manager and the staff and a review of evidence, Licensing determined the Manager did not report a reportable incident to Child Care Licensing within a 24 hour period.

• A licensee must not employ a person in a community care facility as (b) a responsible adult unless the person has the qualifications required under Division 2 [Employee Qualifications].

  Observation. There was no evidence of diplomas or certificates documenting the required 20 hours of education and training qualifications for a responsible adult required under Division 2 [Employee Qualifications] in the staff record.

  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. Licensing observed that items in the washroom were stored on top of the diaper mat. As this mat is used for changing diapers there is some concern that potential bacteria on the diaper mat could contaminate other items stored on the mat.

  Corrective action. Please ensure items are not stored on top of the diaper mat to ensure a healthy and safe environment is provided.

  Observation. The following was observed during the inspection: -Two open garbage containers with multiple used diapers and gloves at the children's level. These were located next to a diaper disposal unit. -Multiple jars of diaper cream were located on a shelf at the children's level.

  Corrective action. Please ensure children do not have access to any objects or substances that are hazardous to their health and safety.

  A licensee must provide to children a comprehensive and coordinated program of indoor and outdoor activities that (c) complies with the program standards set out in Schedule G.

  Observation. Minimal toys and materials for the number of children in care to use were observed in the outdoor play area.

  Corrective action. Please ensure to have sufficient outdoor activities and materials to comply with the program standards set out in Schedule G of the CCLR.
