A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. - One child did not have a doctor / clinic name and phone number on file - One child did not have an emergency contact on file / one child's emergency contact on file were the parents This item is a repat contravention on routine inspection report #CGEE-CRGRCP on 03/May2023.

Corrective action. Ensure that children's files are complete, and that the name of the child's doctor or clinic name along with the phone number is on file. Emergency contacts should also be different from the parent information.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

Observation. Absent children are not indicated and recorded on the daily attendance sheet. This item is a repeat contravention on routine inspection report #CGEE-CERR8P on 25/May/2022.

Corrective action. Ensure that absent children are indicated and recorded daily.

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. The facility manager informed Licensing that the items in the emergency supplies kit have expired, and that the items are in the process of being replaced and replenished.

Corrective action. Ensure that items in the emergency supplies kit are updated.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. The 1st aid kit's benzalkonium chloride wipes (antiseptic topical skin cleanser) expired in 2018/12.

Corrective action. Please replace w/ fresh benzalkonium chloride wipes.

A licensee must do all of the following: (b) operate the community care facility in a manner that will promote (i) the health, safety and dignity of persons in care,

Observation. Repeat violation from FIR #CGEE: - C49UZ5 (23/Jun/2021) - CERR8P (25/May/2022) Of the 6 children in care today, 1 child was > 48 months & 5 children were 24–48 months.

Corrective action. FCC permits ≤ 4 children < 48 months @ any 1 time. Ways to come into compliance: • limit care to 4 children < 48 months @ any 1 time • apply for a temporary placement (approval subject to CCFL's approval)

Observation. Repeat violation from FIR# CGEE: - B8VRM7 (29/Jan/2019) - BMSTZY (17/Mar/2020) 2 children's medical practitioner contact information was left blank on their file.

Corrective action. All requirements of CCLR s.57 are required by children's 1st day of care.

In respect of a record referred to in this Division, a licensee must (a) keep in a single place at the community care facility the records referred to in sections 56 to 58.

Observation. Only partial evidence of the employee's 20 hours of RA training is onsite.

Corrective action. Evidence of the employee's full ≥ 20 hours of RA training must be onsite.

The licensee must not permit a person over the age of 12 to be ordinarily present on the premises of a community care facility while children are present, unless the person is of good character and the licensee has obtained a criminal record check for that person.

Observation. Repeat violation from FIR #CGEE-BMSTZY (17/Mar/2020). An ordinarily present individual lacks her CRC.

Corrective action. All ordinarily present individuals ≥ age 12 require a valid CRC. In the meantime, this individual cannot be onsite during operational hours.

Observation. *REPEAT VIOLATION FROM FIR #CGEE-C49UZ5 (23/JUN/2021)* 7 children were in care today, which included 5 children 26-46 months. The licensee explained that: • these 5 children < 48 months attend on Mondays, Wednesdays, & Fridays • 4 children < 48 months attend on Tuesdays & Thursdays • she prefers to care for children ≥ 24 months

Corrective action. Family Child Care permits ≤ 4 children < 48 months. FHA & the licensee discussed ways to come into compliance: • limiting care to 4 children < 48 months @ any 1 time • applying for a temporary placement (approval subject to FHA's approval)

A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].

Observation. *REPEAT VIOLATION FROM FIR #CGEE-AMVPVG (31/MAY/2017) & FIR #CGEE-B8VRM7 (29/JAN/2019)* The licensee explained that fire drills are practiced monthly & emergency drills are practiced every 3-4 months. The last documented practices were: • fire drill (Oct/2021) • emergency drill (Mar/2021)

Corrective action. Fire drill & emergency drill practices must be documented upon completion.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure. Observation (CORRECTED DURING INSPECTION): *REPEAT VIOLATION FROM FIR #CGEE-AMVPVG (31/MAY/2017), FIR #CGEE-B8VRM7 (29/JAN/2019), & FIR #CGEE-BMSTZY (17/MAR/2020)* For the past 3 days, a school-age child has been signed in @ 07:30 but not signed out upon departure.

A licensee must keep, for each child, a record showing the following information: (k) the date on which the child stops attending the community care facility. Observation (CORRECTED DURING INSPECTION): A former child's last day was not documented.

Immediately after a person who was the subject of a character reference is no longer employed by or ordinarily present at the community care facility, a licensee must return all character references to the person, or destroy the character references. Observation (CORRECTED DURING INSPECTION): A former employee's references remained in her file.

Observation. There were 6 children in care today of the following ages: 45 mos, 36 mos, 35 mos, 34 mos, 29 mos, & 23.5 mos. There's another child enrolled who's 46 mos, who was absent today. Schedule E (s. 34 [group sizes & employee to children ratios]) stipulates the following for Family Child Care where there's no child < 12 mos present: A maximum group size of 7 children, having ≤ 4 children < 48 mos, & of these 4 children, ≤ 2 children < 24 mos.

Corrective action. CCFL & the licensee discussed ways to come into compliance (giving notice to families, amending the attendance schedule to care for ≤ 4 children @ any 1 time, applying for a temporary placement).

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): 2 electrical outlets were uncovered.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. A child lacked her medical practitioner's phone #.

Corrective action. All requirements of CCLR s. 57 must be obtained by children's 1st day of care.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. A child lacked her MSP #.

Corrective action. All requirements of CCLR s. 57 must be obtained by children's 1st day of care.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure. Observation (CORRECTED DURING INSPECTION): A child wasn't signed out on yesterday's attendance.

Corrective action. Accurate attendance records are required to aid 1st responders in the event of an emergency, FHA in the event of a complaint investigation, & MCFD for funding purposes.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (d) copies of any diplomas, certificates or other evidence of the person's training and skills.

Observation. A substitute employee's file only showed 8 hrs of RA training. The licensee stated that the employee has the full 20 hrs & will obtain a copy of the remaining 12 hrs.

Corrective action. All requirements of CCLR s. 19 must be obtained by employees' 1st day of employment.

The licensee must not permit a person over the age of 12 to be ordinarily present on the premises of a community care facility while children are present, unless the person is of good character and the licensee has obtained a criminal record check for that person.

Observation. An ordinarily present individual lacks a Ministry of Justice Criminal Records Review Program CRC.

Corrective action. This individual can't be present in the facility during operational hours w/o a valid CRC.
