A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Licensing Observed the following: - [CORRECTED DURING INSPECTION] A full set of knifes was stored in the lower cupboard with no lock in the kitchen area. Staff has moved the knifes set to another area that is out of reach from the children. - Other cleaning chemicals, such as commercial dish waser detergents were stored in the lower cupboard with no lock in the kitchen area.
Corrective action. Staff will talk to the Director to install locks on the cupboard doors where the chemicals are stored.
11200 - CCLR s.17 - LICENSING
A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager.
Observation. Licensing observed the following repeated contravention to inspection #CLAN-D9B5PF: - [CORRECTED DURING INSPECTION] Manager's name was not posted for Preschool Program - Manager's name was not posted for School Age Program.
10050 - CCLR s.11(1) - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].
Observation. The 2024/2025 record of fire and earthquake drill for School Age Program was not avaiable for review during inspection. Staff expalined that they practice both drills monthly, but was not able to find the record
Corrective action. Staff will ask the Coordinator to send the records to Licensing
13050 - CCLR s.56(1)(c) - RECORDS AND REPORTING
The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.
Observation. There is no record in the child's file that indicates the care plan was last reviewed with the parent in 2022.
Corrective action. Staff will communicate with the Coordinator to obtain the most updated information.
19260 - CCLR s.58(2)(b) - STAFFING
If for any reason an employee described in Column 1 of the table in section 37 [absent employees] ceases employment, (a) section 37 applies, for up to 30 days from the date the employee ceases employment, as if the employee were absent.
Observation. During inspection, Licensing obtained the following information: - One of the staff from the School Age Program had informed Licensing that their manager had resigned from the position two months ago. The Coordinator had been supporting the program and staff are aware to contact the Coordinator if they have questions or needed support. - Manager named for the Preschool Program has not been employed since 2024. Change of Manager form will be submitted to the Licensing.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following: -The toilet lid was broken and held together with duct tape -The couch has a large tear that is held together with duct tape -The industrial dishwasher was broken.
Corrective action. Please ensure items in the facility are repaired in a timely way. In discussion the Manager has discussed her concern with the Supervisor but it has not been fixed as of yet
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must supply equipment, furniture and supplies that are (b) located so as not to block or hamper an exit in the case of fire or other emergency.
Observation. During the inspection Licensing observed several large couches located in the room. A large couch was partially blocking the emergency exit
Corrective action. In discussion the Manager stated that she will move the couch and the facility will be removing the couch from the facility soon.
11090 - CCLR s.15(1)(b) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): Two bottles of hydrogen peroxide were observed on the floor. These were removed during the inspection
11200 - CCLR s.17 - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. During the inspection the following was observed in the facility: -Staff bags were located within easy reach of the children -A diaper mat is not disinfected after each use -A TV was located on a cart. This was not anchored to the cart. This could present a potential hazard. Please provide active supervision until corrected In discussion the Manager disclosed that a playground check is not completed prior to the children playing in the play area. This could be a potential hazard as the facility play area is open to the public.
11010 - CCLR s.13(1) - LICENSING
A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager.
Observation. Licensing observed the Manager's name was not displayed in the facility. This is a repeat contravention from inspection #MZIN-C5BUHR
10050 - CCLR s.11(1) - MEDICATION
If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (a) administered to the child in the amount and at the times specified by the child's parent or in the child's record or care plan.
Observation. Emergency medication was observed for a child in the facility. A permission to administer form was not signed by a parent or guardian and no information on when to administer this was observed. In addition a second vitamin was observed with the medicine making this challenging to know what medication was required to be administered in an emergency
Corrective action. Please ensure to have a detailed plan on when to administer medication and information provided and signed by parents/guardians.
16010 - CCLR s.53(1)(a) - POLICIES AND PROCEDURES
A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Licensing was informed by the Manager that the emergency supplies were expired.
Corrective action. The Manager stated the supplies were on order. Please ensure supplies are checked several months prior to the expiry date to ensure supplies arrive before other supplies expire.
13020 - CCLR s.22(1)(b) - RECORDS AND REPORTING
In respect of a record referred to in this Division, a licensee must (a) keep in a single place at the community care facility the records referred to in sections 56 to 58.
Observation. During the inspection Licensing observed that one of the children's records was not located at the facility. In addition the Manager disclosed that children who were attending for the summer months from another facility only had emergency cards and not the entire child's records. Upon review many items were missing from the children's emergency cards for two schools.
Corrective action. In discussion Licensing and the Manager discussed creating a system to ensure all of the children's records are complete and maintaining that system prior to children attending the facility.
19320 - CCLR s.59(a) - RECORDS AND REPORTING
The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.
Observation. Two care plans were not reviewed annually with the parent or any person requested to by the parent.
19260 - CCLR s.58(2)(b) - RECORDS AND REPORTING
A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (a) the diagnoses relevant to the child's requirement for extra support, as made by health care professionals.
Observation. Licensing observed there was no care plan available for a child requiring extra support.
Corrective action. In discussion the Manager stated this is available on her computer. She will print a copy for the staff to review.

A licensee must ensure that each employee (b) practices implementing the fire drill system at least once each month.
Observation. Licensing observed the record of fire drills. A fire drill was recorded as completed every second month

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Licensing observed that there was a cabinet just outside the classroom door beside the fridge with substances such as cleaning products stored in it. This cabinet was not locked and can easily be accessed by children.
Corrective action. Ensure that children do not have access to any substances that may be hazardous to their health or safety.
11200 - CCLR s.17 - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (d) written policies and procedures that are intended to guide employees in the care and supervision of children.
Observation. Licensing talked to a facility member who told Licensing that children typically have screen time for 2 hours every Friday, which is not consistent with the screen use policy of the facility.
Corrective action. Ensure policies and procedures are followed by all facility members.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): Licensing observed the following inside the facility: - A large, heavy, black rectangular bag with wheels was stored upright beside the emergency exit door of one of the classrooms. This could potentially fall on children, or block the exit if it falls. When this was pointed out during the inspection, the bag was moved and stored horizontally by a staff to avoid the bag from potentially falling. - During inspection, the door to the staff washroom was closed. However, it was observed that a child opened the door and went into the washroom to wash their hands. A staff stood just outside the door. There was ice melting chemicals stored in the bathroom on the floor just below the sink, and was in an open plastic bag. This could easily be reached by children using the bathroom. A staff moved and stored the ice melting chemicals elsewhere during the inspection. This is a repeat contravention. This item was identified and contravened on March 16, 2020 on routine inspection report #CLAN-BMRVER, and on July 27, 2021 on routine inspection report #MZIN-C5BUHR.
Corrective action. Ensure children do not have access to any objects or substances that may be hazardous to their health and safety.
11200 - CCLR s.17 - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].
Observation. During inspection, Licensing reviewed the fire and emergency drill record and observed that for January and February 2022, it was not clearly specified which drill (fire or emergency) was completed, as only the word "drill" was written down. Furthermore, it was noted that for the months of March, May and June 2022, the fire drill was not recorded, as "earthquake" was written down for these months. This is a repeat contravention. This item was identified and contravened on July 27, 2021 on routine inspection report #MZIN-C5BUHR.
Corrective action. Ensure that monthly fire drills and yearly emergency drills are completed and recorded. The record should clearly specify which drill was completed (fire drill or emergency drill).

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. During the inspection the staff washroom was unlocked. In the washroom there was ice melting chemicals on the ground and under the sink there was dishwashing detergent. These chemicals were within easy reach of the children.

Observation. The following was observed in the facility: -4 electrical outlet covers were missing -a shelf over 4 feet tall was not secured to the wall

11010 - CCLR s.13(1) - POLICIES AND PROCEDURES
A licensee must have all of the following: (b) an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Three fire extinguishes out of three were last inspected in August , 2019. These fire extinguishers were observed as being within the area of fully charged.
Corrective action. Please ensure fire extinguishers are checked on an annual basis and that the inspection tag is attached to prepare for, mitigate and respond to an emergency.
13020 - CCLR s.22(1)(b) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. The attendance records were reviewed for the month of March. Eight arrival or departure times were missing. This is a repeat contravention from inspection # RDHI-B9ZRUX.
Corrective action. Please ensure the attendance records are complete.
