  A licensee must ensure that each piece of furniture or equipment for sleeping (b) is clean and comfortable. Observation (CORRECTED DURING INSPECTION): Mats were stored with sheets touching, which can give opportunity for and are washed once per week.

  Corrective action. Licensee said moving forward they will remove sheets and wash bedding daily, then store them separately.

  A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

  Observation. Licensing reviewed 5 children's file. The emergency and Alternative contact person other than the parent, was not recorded for one child.

  Corrective action. Licensee will obtain the emergency contact for the child.

Observation (CORRECTED DURING INSPECTION): Thumb tacks were used on walls accessible to children in care. Licensee removed them during inspection. Blind cords were hanging accessible to children in care, posing choking risk. Licensee moved them during inspection,

  Observation. The licensee immunization records, and references were stored in another area of the facility and were unavailable for review during the inspection.

  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. The change pad in the bathroom was covered in a fabric cover, and was visibly soiled at the time of the inspection.

  Corrective action. Please ensure the change pad is easy to sanitize, and sanitized after each use.

  Observation. During the inspection licensing reviewed 6 children's files, of those, 1 file did not contain the name and telephone number for the medical practitioner.

  Corrective action. Please ensure to keep for each child a record showing the name and telephone number for the medical practitioner.

  Observation. One of two care plans reviewed was not reviewed annually with the parent or any person requested to by the parent.

  Corrective action. Please ensure to review the care plan annually with the parent or any person requested to by the parent.

• STAFFING

  A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation) Observation (CORRECTED DURING INSPECTION): Licensing reviewed two staff files, of those, one file had no record that the ECE certification was verified by the system maintained by the Early Childhood Educator Registry.

  Corrective action. Please ensure to obtain record of that the certificate was verified by the system maintained by the early childhood registry.

  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. DLSOP Safe Play Space –debris was observed on the playground surface,including; broken tile and discarded paint tray.

  Corrective action. Please ensure the parameter of the play area is reviewed and all debris are removed prior use.
