  Observation. Upon review of all child registration records it was noted that one did not have a Medical Practitioner's name and number listed, and one other file did not have the phone number of the Medical Practitioner. The name and number of the child's Medical Practitioner is required in order to have that information available in the event Emergency Services requires it. One record did not have Emergency Contact phone numbers available. Emergency Contact phone numbers are required so that the Licensee can contact a designated person in the event the parent or guardian is unable to attend the facility when a child is ill or injured, or needs to be picked up due to an unexpected event. One care plan reviewed did not have instructions on actions to be taken by the Licensee if symptoms of a condition were observed. The Licensee must have on hand direction from parents of actions to take in the event symptoms are observed so that there is clarity, understanding and agreement between parents and care provider. Notify Licensing of records being updated as noted on or by Oct. 30, 2025. Fire Drill practice record, Emergency Preparedness practice record, Minor Incident Log and Outdoor Inspection log were all reviewed and appeared complete and up-to-date.

  Observation. Upon review of a sampling of child records it was noted that one was missing a statement of immunization status, one was missing the medical practitioners name (phone number was present), and a care plan is required. Care plans has been previously noted as a contravention.

  Observation. Upon review of a sampling of child records it was noted that one did not have a Medical Practitioner's name and contact number. As per discussion, if families do not have a family doctor, they may list the nearest ER or medical clinic and the phone number. Upon discussion it was noted that the Outdoor Inspection log has not been maintained.
