  Observation. Upon review of a sampling of child records it was noted that one requires a photo or digital image, and three require care plans in order to be complete. Facility attendance records, fire drill records, outdoor inspection logs appeared up-to-date during review. Minor incident recording method is in place and easily accessible if necessary.

  Observation. Upon review of records it was noted that the Emergency Preparedness Plan practice has not been documented in the past 12 months. The Emergency Preparedness plan (evacuation or shelter in place) must be practiced annually, and documented when completed. Fire drill records reviewed and appeared complete.

  Observation. Upon review of a sampling of child records it was noted that Doctor's name and contact information is required on one or more forms. Reviewed the Minor Incident Log, Fire Drill Record, Emergency Preparedness Plan practice record, all of which appears to be in order and complete.

  Observation. Child registration records were reviewed for all children in attendance. Some information that could not be located includes care card number (1), Immunization status (1) and emergency consent (3). Please ensure all records are up to date and complete for all children enrolled. Attendance was found to be incomplete on more than one occasion. Both time of arrival and departure must be recorded as per section 57 of the CCLR.
