    Observation. Upon review of all child records it was noted that two did not have indication of a Medical Practitioner name and phone number. This is required in the event the facility or emergency services needs to contact the child's medical practitioner in an emergency. It was also noted that some photos of children were not up-to-date. Photos are required to be up-to-date so that in the event a child is missing, emergency services has a recent likeness of the child to work with to identify the child. Minor Incident Log, Fire Drill practice log, Emergency Preparedness log and attendance records all appeared up-to-date and to be used regularly, as needed.

    Observation. Upon discussion with the Licensee it was determined that currently there are 4 children under 36 months attending on more than one day per week. An exemption application is required, Licensing Officer to send information.

    Observation. Upon review of an employee record it was noted that validation of the certificate of qualification was not on file. Licensing Officer to send Licensee the link to validate all certificates of qualification for employees.

    Observation. Upon review of the Minor Incident Log, and discussion with the Licensee, it was determined that the log book has not been used to it's full capacity to document all minor accidents, illnesses and unexpected events.

    Observation. Upon documentation review it was noted that the facility was closed unexpectedly in the recent past, and this was not reported to Licensing in an Incident Report. Upon review of a sampling of child records it was observed that one required a care plan. Child records require space to document "Other Identifying Information" as per Section 57 (2) (i) of the Child Care Licensing Regulation.
