    Observation. A support worker is present with the children and does not have evidence of a criminal record check or character references. Unknown criminal record clearance status and character references increase risk to the health and safety of children. Submit to licensing by March 5, 2026, evidence the missing items have been collected and are at the the facility. Staff files are incomplete. One staff criminal record clearance has expired, and several certificate validations are not current. Incomplete staff files may increase risk children if all the information in the file is not kept current. Submit to licensing by March 5, 2026, evidence that the missing information has been collected and is in the staff files.

    Observation. One child file is missing a photo or digital image. Unavailable child photo's may increase risk to children in an emergency. Submit to licensing by March 5, 2026, evidence the missing photo has been collected and is in the child's file. Some medications forms are incomplete. Missing information included: What the medication was for, start and end dates, and unspecific administration times. Incomplete medication forms increase risk of the medication being improperly administrated to the child. Submit to licensing by March 5, 2026, evidence of a plan that will ensure staff complete the medication forms completely.

    Observation. A bottle labeled window cleaner was observed to be accessible to children in the sleep room located on the main floor. This was resolved during the inspection. It was observed that two electrical outlets located in the preschool were uncovered causing a potential risk of electric shock. Ensure that all electrical outlets are covered or have tamper-resistant shutters.

    Observation. A review of the facilities medication administration forms was conducted during the inspection. It was observed that some non-prescription medications are being administered on a as needed or on going basis. The medication form did not include special instructions from the parent respecting the child's medication in writing and agreed to by the licensee. Provide evidence of having collected the parents written instructions regarding the administration of non-prescription medications.

    Observation. A review of the children's care plans was conducted during the inspection. It was observed that some of the active care plans did not contain evidence of having been reviewed yearly. Ensure evidence is available that each child's care plan is reviewed on a yearly basis.

    Observation. One employee record from August 2020 has an RCMP criminal record check rather than a clearance letter from the Criminal Record Review Agency and another file has one reference check on record.

    Observation. - Some of the newest child registration forms in use have missing information such as medical insurance plan number, name and phone number for a medical practitioner, any illness, medication administered, any special instructions regarding diet, and persons not permitted access to the child. - The date on which a child stops attending could not be located in active or closed files upon request. - The attendance records for children entering and leaving the Preschool and the Group Child Care (30 Month to School Age) room to attend both programs are recorded to be the exact same arrival and departure times for every single day that children attend. Ensure that attendance records are accurate and complete at all times.

    Observation. The facility completed a radon test in 2014 during warmer months of the year. Long term testing between November to April had been requested. The manager has deployed two radon detectors to ensure that correct test results are received no later than April 1, 2021.

    Observation. Sharp knives are accessible to children on the upstairs kitchen counter in the 30 month to school age room. When this was brought to the managers attention, they were removed from children's reach.
