Observation. The complaint alleged that a child's privacy was violated when a photograph was taken of them with their pants down while in the classroom. Staff were interviewed and described the circumstances under which the photograph was taken. Based on the information received, the allegation is substantiated. Observation. The staff's character references were not verified, and one of the staff was missing the immunization status. Incomplete staff records may pose a risk to children's safety. Submit to Licensing by August 19, 2025, on how you have resolved the issue and ensure that required staff records are obtained prior staff's start of employment.

Observation. The children's records were missing written consent from a parent authorizing to call a medical practitioner or ambulance in case of accident or illness, if the parent cannot be reached immediately. Some children's records were also missing required information such as doctor's name and phone number. Incomplete children records may cause delays in responding to emergencies. Submit to Licensing by August 19, 2025, on how you have resolved the issue and ensure that all children records are up-to-date.

Observation. A training potty was placed in one of the classrooms. This may compromise hygiene and pose the risk of cross-contamination. Submit to Licensing by August 15, 2025, detailing on how you have resolved the issue and ensure that a healthy and safe environment is maintained at all times.
