The walls and the doors of the Group Child Care (School Age) program's bathrooms are unclean. Regular cleaning and sanitization of surfaces help prevent the spread of illnesses in child care facilities. Observation. There was no evidence of a fire drill practice in June and September 2025. It is a requirement that monthly fire drills are conducted to ensure staff and children are prepared for emergency's. Submit to licensing by October 17, 2025, a plan of how you will ensure that fire drills are practiced and recorded every month. One staff ECEA certificate was not verified by the licensee using the system established and maintained by the early childhood educator registry. Incomplete staff files increase risk to children. Submit to licensing by October 17, 2025, evidence that the ECEA certificate has been validated.

Observation. Evidence of informing parents of illness or injury was not available in the facilities minor incident book. Parents must be notified of all illness or injuries that occur at the facility. Submit to licensing by October 17, 2025, a plan of how you will ensure parents are notified of illness and injury and how this information will be documented. Several child files are missing immunization status, one child file is missing medical insurance number and start date. Incomplete child files increase risk to children in the event of a disease outbreak or in an emergency. Submit to licensing by October 17, 2025, evidence of having collected the missing information.

Observation. As a result of a complaint received to licensing on July 26, 2021 it was discovered the licensee did not submit plans or receive written approval for the addition of a wooden sun shade structure located in the outdoor play space. As a result of the investigation the structure has been removed from the outdoor play space.

Observation. The addition of a wooden sun shade structure was installed without submitting plans or approval from the medical health officer. The photographs submitted upon request raised concerns regarding the structures integrity. A written statement from a qualified building professional confirming the structural integrity was requested from the licensee. On August 6, 2021 licensing received photographic evidence that the structure has been dismantled and removed from the outdoor play space.

Observation. The paper towel dispensers in both bathrooms located in the out of school care program were empty. There was a small basket with folded hand towels located outside the bathrooms on a high shelf however this may not be easily accessible for all children.

Observation. - The facility has recently starting keeping the log of minor incidents on a computer program, however the reports were not able to be accessed today due to a computer error. - Two child files are missing medical service plan numbers - Some children were not signed out of the out of school care program today. - A care plan for a child has not been reviewed since January 30, 2019.
