  Observation. One staff file is missing character references. Incomplete staff files pose potential risk to children. Submit to licensing by October 17, 2025, evidence that the missing character references have been obtained and are in the staff file.

• C4.1 - Are facility records current and complete?Non-critical

  Observation. Some child files are incomplete, items missing included photo or digital images, and medical insurance numbers. Incomplete child files increase risk to children in an emergency. Submit to licensing by October 17, 2025, evidence the missing information has been collected and is in the child files. Additionally, submit a plan of how you will ensure the children's files are complete prior to the child attending the facility.

  Observation. A shed that contained sharp objects and tools was accessible to children. A variety of garden tools were accessible to children in walkway to outdoor play space.
