  Observation. The diaper change mat is no longer in good condition. The landscaping fabric in the outdoor play space is torn and coming up through the pea gravel, this is a potential trip hazard.

  Observation. A reportable incident was not reported to Licensing. Some photos were missing from "child emergency cards".

  Observation. Staff have not practiced the emergency plans in the last 12 months.

  Observation. Licensing conducted an inspection of staff files and found them to be incomplete. Confirmation of immunization status and two criminal record checks complete by the criminal record review program were missing.
