The screw that held the handle was exposed. 3) Dirty step stools in the Gan Aviv bathroom.

Observation. On January 29, 2025, Licensing observed the following: 1) In the Gan Yeladim and the Yeladim rooms the bottom vinyl art counter drawers had broken, chipped and loose vinyl. The particle board underneath was exposed. 2) The Gan Aviv room had a vinyl couch with worn and ripped covers, exposing the foam underneath. On March 10, 2025, the manager informed licensing of the following: 1) Maintenance will replace the art counters in both the Yeladim and the Gan Yeladim rooms. Work will be completed this week. 2) The couch will be replaced by the end of the week.

On January 29, 2025, Licensing observed the following: 1) In the Yeladim outdoor play space the upright logs bedside the water feature were rotten and no longer stable and secure. 2) In the Gan Aviv outdoor play space the metal wire underneath the concrete walkway was exposed with sharp edging as the wood chips had eroded. 3) In the Gan Aviv outdoor play space the railing at the top of the concrete ramp was wobbly as one post was rotten at the ground. 4) In the Gan Aviv outdoor play space one fence post besides the blue tunnel was rotten and wobbly. On March 10, 2025, the manager informed licensing that the above items will be addressed by maintenance during the Easter break.

Licensing observed the following in the Gan Yeladim room: 1) A loose electrical cord located directly above a nap mat. 2) A wood doll bed and doll stroller stacked on top of a wood bench that posed a risk of falling onto a child. 3) Worn away vinyl floor tiles located by the door to the outdoor play space. It is recommended to keep the door to the large storage room secured at all times.

Observation. Licensing observed one anaphylactic care plan in place with specific instructions on whether the Epi Pen should be administered or not when the child exhibits signs they ingested nuts. This would be depend on certain symptoms present or not present. A discussion was held with the manager regarding the following: The course of an anaphylactic episode cannot be predicted with certainty at the onset and may differ from one person to another and from one episode to another in the same person. Epinephrine should be given at the start of a known or suspected anaphylactic reaction. Administration of Epi Pen should not be delayed.

Observation. CCFL observed the following: In the Shalom Yeladim room - One child has a cream that is administered as needed. The child has a signed Request for Administration of Medication form in their file, however when medication was being administered this was not recorded on the form but in the log book.

Observation. CCFL observed the following: In the Shalom Gan Yeladim room, - the couch has a large rip on the seat. Staff shared with CCFL that tape was used to repair it however during the inspection, the tape was no longer sticking. - broken vinyl on cupboard door located under the art sink. CCFL mentioned these observations to the Manager. Manager stated a new couch is in the building, it will be placed in the room within the coming days by the maintenance staff. Regarding the broken vinyl cupboard, Manager will inform the maintenance staff.

Observation. Due to COVID, drop off and pick up now occurs from the outdoor playground. The infant/toddler room also picks up and drops off from the 3-5 yard. The Licensee has installed a locking mechanism out of children's reach on both sides of the gate that secures the outdoor playground. Supervising staff does not have a direct line of supervision to this gate unless they are positioned by the sand pit or climber. Licensing observed a parent exit and leave the gate wide open. Two children were digging nearby the gate in the sand pit, 21 children were outside in total with 4 staff. None if the staff noticed the wide open gate. The gate stayed open for about 2 minutes. Then the parent noticed and realized they left the gate open and closed it. Licensing discussed the incident with the program manager. It was discussed to develop a supervision plan that incorporates staff positioning themselves to ensure the gate is always closed. (A self closing hinge is recommended).
