Observation. Licensing observed the following: 1) A worn vinyl couch with duct taped corners. The duct tape is loose, exposing the foam underneath. This item is no longer in good repair. 2) Carpets with frayed edges in the nap room and main activity room that posed a tripping hazard. Although the activity room carpet is steam cleaned monthly, due to its age, the carpet is permanently stained and in need of replacement.

The Manager informed a contractor is schedule to come in the next week to work on the walkway paver so no edges are protruding above ground level.

Observation. Licensing observed the following: 1) A carpet with frayed edges in the nap room that posed a tripping hazard. 2) Worn/ripped corners on vinyl couch cushions. 3) Holes in the bathroom drywall beside the toilet paper holders. 4) Stained dirty bathroom baseboards and floor.

Some of the wood stumps are showing signs of rotting due to aging and wear and tear. It is recommended to monitor these and target for future replacement.

Observation. Licensing observed the following: 1) In the nap room, the vinyl baseboard moulding had come loose in a corner. This was repaired by maintenance during the inspection. 2) In the nap room, the drywall was chipped off around the bottom of the door frame. 3) The paint on the diaper changing steps had chipped in several places exposing the wood underneath. This poses a sanitizing challenge.
