Observation. There is one child currently in care requiring a care plan due to sesame allergy. Licensee has a care plan posted and in child's file. Noted the expiry date on the care plan said November 2025. We checked the epipen and it had expired as of October 31, 2025. Licensee stated she would let the family know at pick up, that a new epipen was required. Licensee mentioned that the father was a doctor. I said the parents would understand then, the need for an updated epipen. Licensee will amend the care plan to indicate the new expiry date once the new epipen is received. In the meantime, Licensee will be vigilant to ensure there is no exposure to allergens. Licensee will inform CCFL when the new epipen is in place.

Observation. LO observed the Licensee ask and then help two children to be seated and buckled into the stroller. Another child was crying while sat on the patio door ledge. In her attempt to distract the crying child, the Licensee said that she was going to get a special item from inside her home. The two children in the stroller then demonstrated that they wanted out of the stroller and the Licensee responded by giving them each a tissue, saying 'you can play with this'. The two children then began to shred the tissue. When the Licensee returned with the item, she showed it to the crying child and the other two children. The children in the stroller began to make efforts to get out of the stroller with the Licensee trying to distract them further. LO commented that the children appeared to want out. The Licensee disagreed. The children continued to not be distracted by the item. LO commented that it was important that the children be permitted to get out of the stroller. The Licensee disagreed. In response, LO commented that she was going to get the children's footwear. The Licensee said 'no no' and then began pushing the children around the garden in the stroller, saying 'this is what we do, we go around the garden to look at different things.' Upon circling back to the patio door, one of the children was holding a doll, the other a ball. It was then that the child holding a ball communicated they wanted out of the stroller and the Licensee helped them. LO observed the other child, who was holding a doll, was squirming - it appeared that they too wanted out of the stroller. The Licensee placed her hands on the doll and simultaneously asked the child if they wanted out. The child answered 'no' however, it appeared to the LO that the child perceived that the Licensee was taking the doll away. The Licensee then said to the child, 'you want to stay in the stroller' at the same time removing her hands from the doll. The child said 'yes'. As the LO was informing that she was about to leave, the Licensee asked, 'did you hear her say that she wanted to stay in the stroller?' Because the children needed the Licensee's attention, the LO did not respond to the question and instead, informed that the LO will follow-up with the Licensee about today's inspection. LO observed a young child emotional after saying good bye to their parent at morning drop-off. While at the beginning, the Licensee made efforts to comfort the child, as time passed and the child continued to cry, the Licensee was observed having the child sat on their lap, telling the child to stop crying and raised their finger to a pointing position to quiet the child. When the LO commented that this interaction was not appropriate, the Licensee responded by saying that the child was 'just making sound - no tears'. Later in the inspection, the child remained emotional, crying earnestly for their parent. At one point, the child was sat on the patio door ledge, the Licensee was sat across from the child and two other children were seated in the stroller. The crying child looked at the LO at which point the LO acknowledged their emotions and provided a box of tissue to the Licensee so that she may help the crying child. The Licensee took the box of tissue, offered a tissue to the child and said to them, 'you can wipe your tears' and continued singing the song, "If you're happy and you know it..."

Observation. The Licensee, an Early Childhood Educator Assistant, was often observed vocalizing a sound repeatedly and accompanied with wagging her finger at children when their behaviour was deemed undesirable. The vocalization sounded like 'nah nah nah'. Licensing Officer reminded the Licensee that this vocalized sound and wagging of their finger is not appropriate, that instead, the Licensee is to speak with the children, guiding their behaviour in a respectful manner.

Observation. Schedule H of the Child Care Licensing Regulation provides the following definition of "neglect": "neglect", which means the failure of a care provider to meet the needs of a child, including... care or supervision; The Licensee left a child unsupervised in a wagon, on a stairwell landing. As a result of this, the child fell down 4-5 stairs. Upon reading the Licensee's documentation, Licensing Officer spoke with the Licensee, asking of any injuries that the child sustained. The Licensee informed with exception to a mark on the child's forehead, they were aware of no other injuries. The Licensee commented that they will never again place a child in such a situation. Licensing Officer read in the documentation that the Licensee will "eliminate the stair climbing exercises". This was also discussed with the Licensee. Due to the serious lack of judgement on the part of the Licensee which could have resulted in a significant injury to a child, Licensing has determined that the Licensee was neglectful in their care and supervision of a child.

Observation. Licensing observed colouring markers with lids in a zippered pouch on the floor. Licensing Officer brought them to the Licensee during the inspection who then gave it to a preschool-aged child. Some other markers with lids were observed available for the children as well. Due to the age of a couple of the children in care (toddlers), the lids present as a choking hazard. It is important that they are removed from access and presented to children when the Licensee is able to provide close supervision.

Licensing is notified within 24 hours after a child is involved in, or may have been involved in, a reportable incident described in Schedule H or has a reportable communicable disease. [CCLR Sec.55(2)(a)(b)]
Observation. When reviewing the Licensee's documentation, Licensing Officer observed an account where a young toddler child fell down the stairs on February 7 2025 at approximately 12:15 pm. The definition of "fall" ss provided in Schedule H of the Child Care Licensing Regulation is as follows: "fall", which means a fall of such seriousness, experienced by a child, as to to require emergency care by a medical practitioner or nurse practitioner, or transfer to hospital. As provided in the Licensee's documentation, a young toddler child was placed in a "plastic red wagon which is around 15 cm or 20 cm (above the ground / high)." The Licensee left the child in the wagon on the 5th floor landing so as to support other children go up the stairs. It was at this time when the young toddler child was heard crying. The Licensee informed that they caught the young toddler child in their arms after the child had rolled down 4-5 steps.

Observation. When reviewing the Licensee's documentation, Licensing Officer observed an account where a young toddler child fell down the stairs on February 7 2025 at approximately 12:15 pm. When asked, Licensing Officer was informed that the child's parent was informed of the fall at pick-up time. A review of the daily attendance record occurred. It demonstrated that the child was picked up at around 5 pm.

Observation. The Licensee's criminal record check clearance expires today, March 25 2025. The Licensee informed that she believed she had submitted a consent when signing up for a class at a college. Licensing has not received the new clearance. Observation. On two occasions, Licensing observed the Licensee dry children's hands at the bathroom sink without asking them if they needed help or if they wanted to do it on their own. Similarly, Licensing observed the Licensee lifting children down from the step stool and placing them outside of the bathroom after they had washed their hands. Again, the Licensee did not ask the children if they needed help getting down. The Licensee simply moved the children.

Observation. During today's inspection, the Licensee informed that she brought the children to the library for a Hallowe'en story and song time. When asked which entrance / exit the Licensee used when arriving and leaving the library, the Licensee informed that it was the Kingsway main entrance. Licensing Officer informed that due to safety reasons, the Licensee is not permitted to use the Kingsway main entrance, rather to instead use the library's 8th Avenue entrance by way of the laneway.

Observation. During today's inspection, Licensing observed the child-safe lock not engaged on the primary bathroom (Licensee's bathroom). The door was ajar and the space was accessible to children. During a previous inspection, the Licensee informed that children would not have access to this bathroom due to the availability of various products and personal hygiene items. (Addressed during the inspection.) Licensing observed the closet located in the primary bedroom as being open and with all items stored there being accessible to children. Licensing observed the closet located in the entryway as being unlocked and with various items stored there. Typically this closet is locked and not accessible to children. The Licensee informed that she had been decorating and it was for this reason that the closet was unlocked. A discussion occurred with the Licensee about having a plan in place to self-inspect the home prior to children's arrival each morning.

Licensing Officer observed the Licensee physically redirect a young child away from the front door by picking them up from behind and moving them without telling the child what they were doing. This practice on the part of the Licensee is a reoccurring contravention. Licensing Officer informed the Licensee that this type of practice is inappropriate, that instead the Licensee is to offer their hand to the child, invite the child to come with the Licensee.

Observation. When taking a child to the library, the Licensee did not bring the child's epi-pen. The epi-pen remained in a cupboard in the Licensee's kitchen. Upon inspection of the epi-pen, Licensing Officer observed that the medication had expired in July 2024.

Licensing observed the child safe-lock not functioning for a closet with sliding doors located in the second bedroom. Licensing observed at the door threshold of the second bedroom, the wood which offers a transition on the two types of flooring, is broken. Licensing observed that the table and high chair trays were only sprayed with a bleach / water solution - they were not cleaned beforehand

This unhygienic practice is a reoccurring contravention and continues to be marked in non-compliance. Licensing observed that the surfaces, inside and outside of the fridge, are in need of cleaning.

Observation. Upon arrival to the Licensee's home, Licensing Officer was directed by the Licensee via the front door intercom to go to the public library as this was where they were with the children. Licensing Officer arrived to the library and greeted the Licensee who had three children in care. The children were participating in a story - sing-a-long time. Licensing Officer asked to review the Licensee's first aid kit and emergency medical consents. The Licensee informed that they had removed them the previous day from the bag that is carried when taking children off-site. After the library story - sing-a-long time had concluded, Licensing Officer joined the Licensee and children to return to the Licensee's home. Upon arrival to the Licensee's home, Licensing Officer requested the first aid kit for inspection. The Licensee was not able to locate the first aid kit.

During a previous inspection, the Licensee informed that children would not have access to this bathroom due to the availability of various products and personal hygiene items. Observation. Licensing observed two children's bibs - used only for messy art / painting projects - hanging on a towel rack in the bathroom. During the inspection, the Licensee relocated them to hang on a clothes hanger outside of the bathroom. Licensing observed the bathroom as being in need of cleaning, specifically the floor, counter behind sink (a black substance observed), mirror and bath tub. Licensing observed several tears to the surface of the diaper change mat preventing it from being thoroughly cleaned and disinfected. For a short while, the Licensee's son was present in the bedroom where children would rest / sleep each day. While acknowledging that the Licensee had not yet had an opportunity to prepare the bedroom for the children, a discussion did occur with the Licensee about removing an extension cord, addressing a bedside table's drawer that contained cords and to close the bedroom's closet doors to prevent children's access. The Licensee removed the extension cord and committed to preparing the bedroom for the children.

Observation. During the inspection, Licensing observed a child take another child's puzzle piece. The child who had been using the puzzle was upset and went to the Licensee who was in the bathroom helping another child. When the child explained to the Licensee what had happened and that the other child does not get to have a sticker, the Licensee said of the other child, 'he does not deserve a sticker for this behaviour.' The child then repeated this statement which resulted in the other child being upset. The other child then said, 'crying children don't deserve stickers either'. The Licensee's comment is not developmentally or age appropriate for children this age.

Licensing observed the following as instances of physical restraint: * a child wanted to play at the same table as another child; the Licensee then held the child by the wrist and walked the child to sit at a different table on their own to do a different activity; this occurred multiple times throughout the inspection with the same child * a young toddler child went into the kitchen unobserved by the Licensee; when Licensing brought this to the attention of the Licensee, the Licensee went into the kitchen, picked up the child from behind and carried them out of the kitchen * a toddler child walked up to a child who was playing at a table, the toddler grabbed one of the puzzle pieces that the child was using; the Licensee took the puzzle piece away and then held on to the child's wrist and walked the child to the other side of the room so that the child could play on the slide * a toddler child was upset about wanting stickers; this child was standing by the main door to the home and looking for stickers; the Licensee followed this child to the area, took hold of the child by the upper arm; the child showed physical resistance to being moved and was crying; the Licensee then picked up the child and began walking away from the area - the child continued to show physical resistance; when the Licensee set the child down in the living room, the child then ran back to the area of the main door

Observation. Licensing observed a young toddler child, who was walking around the home, eating food that had fallen to the floor from another toddler child who was eating in a highchair. Due to a disagreement between this child and another, this child began crying. Licensing expressed concern that this child may choke on the food in their mouth due to the child crying. From behind, the Licensee stopped the child from walking away and without advance notice, scooped the food from the child's mouth. Upon Licensing's arrival to the home, a child was observed seated in a highchair for approximately 25 minutes. The first 15 minutes, the child was observed eating their morning snack. After no longer eating for approximately 10 minutes, Licensing communicated to the Licensee that the child was finished eating. It was then that the Licensee removed the child from the highchair.

Observation. When reading the log book that is used by the Licensee to account for minor injuries, illnesses and unexpected events, documentation was observed about a situation involving two young children who were having a difficulty over a piece of equipment and when the Licensee was preparing morning snack. This difficulty resulted in one child scratching another child. The Licensee documented that one child was placed in a portable crib and the other in a highchair. As discussed with the Licensee, it is inappropriate to place children in a portable crib or a high chair for anything other than the equipment's intended purpose ie eating, sleeping. Licensing Officer explained that to do so would be using a piece of equipment as a restraint.

Observation. When asked, the Licensee informed that there is no log kept of all inspections, maintenance and repairs specific to the on-site outside play space nor of areas of play in the community where children are taken to.

A child(ren), while under the care or supervision of the licensee, has been subjected to confinement or physical restraint by an employee which was not authorized in a child care's plan and was not an "emergency restraint". [CCLR Sec. 52(1)(b) and Schedule H]
Observation. Licensing Officer observed the Licensee hold a child on her lap when the child was perceived as having "bad behaviour". Licensee informed that "it is a consequence", that the child was to "sit for awhile", that even though the child was upset and said clearly that they did not want to sit down for a while, the Licensee insisted that they remain in her lap at which time the child was held in a cradled position. Licensing Officer informed the Licensee that this practice is not permitted yet the Licensee responded by saying that she and the child have a strong bond, that the child wants to be a good (child), that the child might continue playing when the Licensee had asked them to clean up. The child was then able to move away from the Licensee. Licensing Officer informed the Licensee that it would be important to talk more about this practice. Later in the inspection, Licensing Officer observed a child trying to crawl away from the Licensee, with the Licensee pulling at the child's arms and legs to stop the child from moving away. The child was crying, saying "no" repeatedly and then calling for their father. The Licensee tried to gather the child in her lap. Licensing Officer spoke again to the Licensee stating that the Licensee must not make the child sit on her lap.

Observation. Licensing Officer observed the following: * while wearing a glove that was worn during a diaper change of a child, the Licensee went on to touch another child to remove them from a bedroom; touched a child's water bottle when removing it from a table to a tray; touched toys; touched another child on their shoulder * after wiping a child's nose with a tissue, the Licensee did not wash her hands * at another time, after wiping a child's nose with a tissue, the Licensee then used the same tissue to wipe another child's nose

Observation. Licensing Officer observed the following: * blind cords were positioned inaccessible to children - good! * bedding was stored on cots which were leaning against the wall and stacked against each other - bedding is to be stored separate so as to prevent spread of illness * after changing a child's diaper, the change mat was not cleaned with a soap / water mixture; Licensing Officer was informed that it is used by only one child and it is cleaned once per week; that if there are any visible fecal / urine on the mat, a Lysol wipe is used to clean the surface - the diaper change mat must be cleaned after each use * no soap / water spray bottle available for cleaning surfaces * a cloth was used to wipe a bleach /water spray off the diaper change mat; it then was placed on the bottom rung of a chair to be reused for the same purpose - Licensing Officer informed that a single-use hand towel is to be used once and then either discarded or laundered. The diaper change mat must be disinfected after each use. * no single-use hand towels were observed located at the kitchen sink * a toddler child was asked to stay in place on a diaper change mat which was located on a 20 inch (approximate) high shelving unit; the Licensee then went into a bedroom to bring a child out into the living room - this practice placed a child in a situation where they could have been injured by rolling / falling off the change mat / shelving unit * temperature by way of the Licensee's fridge thermometer read 6'C; as discussed, a safe fridge temperature is to be 4'C or less * located on the top shelf of the fridge was an open package of ground pork which was placed on top of a container that had fresh vegetables - Licensee informed that she has Food Safe training, that some of the food is for her personal use; as discussed with the Licensee, the storage of food in this manner is unsafe

Observation. Licensing Officer observed the kitchen gate not used as a barrier to prevent children's access. The following items were observed on the kitchen floor: * a container of outside surface de-icer * a spray bottle containing bleach / water solution * a bottle of 16% cooking wine After discussing children's accessibility to these items, the kitchen gate was closed. A short while later, the gate was again left open by the Licensee. Licensing Officer observed bingo dabbers stored on a shelving unit. When reading their label, the manufacturer provided instruction that they to be kept out of the reach of children. All interior sliding doors were observed as not having their locking mechanisms engaged therefore children had access to full storage areas which contained items such as laundry detergent and a washer / dryer.

Implementation of the fire drill system is practiced at least once each month, and implementation of the emergency plan is practiced at least once each year. [CCLR Sec. 22(2)(b)(c)]
Observation. August 15 2022 is the last entry documented for a fire drill having been practiced. No date was observed documented for the annual emergency evacuation drill.

A healthy and safe environment is provided at all times [CCLR Sec. 13(1)].
Observation. *Closets and cupboard doors throughout meant to secure items from children's reach were observed to be open and unsecured. *The kitchen surfaces such as the stove and counters were in need of cleaning, *Licensing observed a pot of cooked cauliflower on the floor of the kitchen. *Bulky items have been piled up in the bath tub as a means of storage.

The facility, the furniture, equipment, fixtures and supplies are of sturdy and safe construction. They are easy to clean, and are clean and in good repair and free from hazards. [CCLR Sec. 13(2) and Sec. 15(1)(a)]
Observation. *Licensing observed the gate blocking access to the kitchen is no longer attached and therefore is not an adequate barrier. *A dresser located in the small room is unsecured. This poses a possible tipping hazard. *The wallpaper is peeling away in the front activity room.

Observation. *A discussion was held with the Licensee in regards to the lack of a "home" environment. There are no soft furnishings or cozy home like items in the facility with the exception of the Licensee's personal bedroom.

Observation. *There is a care plan in place for a child with anaphylactic allergic reaction however the plan contains outdated information regarding administration of Benadryl and does not indicate when to administer the epi-pen (what are the signs?).
