Observation. The Licensee informed that the inspections of the outdoor space are conducted daily (i.e. at the park, patio, etc.). However, Licensing was unable to locate a record/checklist that accounts for this practice. The Licensee informed that she was unaware that such information needed to be documented. Licensing will provide a template to support the Licensee in demonstrating compliance with DOLSOP-Safe Play Space.

Observation. Licensing observed a cabinet against the wall with a nap mat right up to the cabinet. The cabinet was screwed to the wall with a Velcro strip. The Velcro strip had an amount of slack that tipped the cabinet forward and did not appear secure enough to withstand an earthquake. A discussion was held to remove the cabinet.

The drawer contained a battery that was removed at the time of inspection. 2) A few mouse droppings in the corner of the nap room, behind a suitcase. Licensing showed the area to the Licensee who responded that a mouse had entered into the apartment through the patio sliding door. This happened about 2 months ago. The licensee stated they had cleaned all the droppings in the apartment but had missed that corner because of the suitcase blocking the area. The licensee added that the mouse problem was no longer an issue.

Observation. Licensing observed electrical cords from patio lights hanging at a low level. The electrical cords had a large amount of slack, were accessible to children and posed a strangulation hazard. The Licensee agreed to refrain from using the patio until the cords are tied up out of children's reach. The Licensee had installed a new fence that measured 3 feet high and appears to be climbable. A discussion was held regarding measures to mitigate the possibility of a child climbing the fence. It was agreed upon that the Licensee must be present on the patio at all times when children are outside and must diligently supervise the children outside.

Observation. When Licensing arrived to conduct the inspection they were buzzed into the building by the Licensee and greeted at the front door of the apartment by the Licensee and one child. When Licensing went into the apartment they observed one child in the kitchen unsupervised. The safety locks in the kitchen were broken giving children access to hazardous items.

Observation. Licensing observed the following: * the counter beside the stove in the kitchen was peeling making the surface difficult to clean or sanities effectively. The Licensee had placed a wood beaded placemat over top of the counter, however this is not effective * some of the child proof locks on the kitchen cupboards are no longer locking and also the lock on the freezer in the nap room no longer locks * one of the drawers of the long dresser in the nap room was broken * the plastic on the diaper mat was torn in places exposing the foam underneath which makes the diaper mat no longer able to clean and sanitize effectively * the small patio had a cigarette lighter on the ground - the Licensee stated that it must be from the apartment above and threw it back upstairs * the small patio had a folded deck chairs, a stroller, a car roof storage container and other items that were taking up space for the children to use. Please remove these items so the children have adequate space to play on the patio Licensing also discussed that during the day to allow the children more access to the patio for play

Observation. Licensing observed that there is a large mattress on the floor that takes up a majority of the space in the nap room. The licensee stated this is where they sleep during nap time. Licensing pointed out that they should not be sleeping during the children's nap and the Licensee stated they just rest. This mattress is to large for the small space and touches the other children's mats on the floor. Licensing also observed that because there is not enough space on the floor one child's mat it stored in the crib on top of the crib mattress.

When Licensing went into the apartment they observed two children sitting in high chairs unsupervised. As stated above a child followed one of the Licensing Officers into the kitchen unsupervised.

Observation. During the inspection Licensing observed the children jumping on the couch, hitting and fighting with each other. It appeared that the Licensee was unaware of this as they were on their phone texting. During the inspection Licensing also observed two children with crayons in their mouths. One of the children ran across the room with the crayon in their mouth and the Licensing Officer asked the child to take the crayon out of their mouth. The Licensee was not aware of any of this until the Licensing Officer informed her that children had crayons in their mouth. The Licensee stated that it was because Licensing was there this happened and that normally she would be sitting at the table with the children. Licensing also observed a child take a small chair and place it in front of the built in shelving and climb on the chair to try and reach things on the shelving.

Observation. Licensing observed a bottle of vodka on the bottom shelf of the refrigerator. There was no lock on the refrigerator or any safety barrier of any kind to prevent the children from accessing the kitchen. During the inspection a child followed a Licensing Officer into the kitchen unsupervised or unknown to the Licensee.

Observation. When Licensing asked to see the children's files, the Licensee could only find 3 of the children's files. When Licensing asked to see the 4th child's file, the Licensee was not sure where it was. The Licensee then went through her phone and handed the phone to the Licensing Officer and showed her a child's file, however this was for a different child. The Licensee then scrolled through her phone and found the correct child's file.

Observation. Licensing observed that the program had put up a new fence on the patio without prior approval from Licensing. Licensing has the following concerns in regards to the fence: * the height of the fence is 2 feet 11.5 inches, the fence is deemed an insufficient height for the age and development level of children in care (eg. a four year old could easily climb the fence). * the design of the fence is climbable, with two separate footholds for the children to use * the gaps in the fence are 4 inches. The Director of Licensing Standards of Practice - Safe Play Space states: For prevention of entrapment, spaces (i.e., holes or openings) must be smaller than 9 cm (3.5 inches) or larger than 23cm (9 inches). * the latch on the gate is within children's reach On May 13th, 2022 Licensing spoke with the Licensee's adult daughter, who stated they wanted to replace the fence on the patio and wanted to know what height it was to be. LO explained that any structural changes were required to be approved by Licensing. Licensing stated that it has to be non-climbable and no gaps in between 3.5 and 9 inches as this could be an entrapment. Licensing would require plans showing the fence with its dimensions. As stated above, plans were not submitted to Licensing for review and approval prior to the fence being installed.

Observation. During the previous inspection on March 10, 2022 the Licensee stated that they take the children to Toys R Us on a regular basis. Taking the children to Toys R Us is not an age appropriate activity that promotes child's development. Children are required to be provided with activities that are age appropriate and designed for the development and care of children.

Observation. Licensing observed the children jumping on the couch, hitting and fighting with each other, a child was swinging the swing and let it go and it hit another child, a child was hitting another child with a piece of clothing and it hit the child in the eye. The Licensee was not aware of any of this and the Licensing Officer has to step in and assist the children. Licensing observed a child open the sliding door to the outdoor area and was trying to go outside. The Licensing Officer had to inform the Licensee that the child was trying to go outside. Licensing also observed a child jumping on the couch and dancing around as they were eating hard sucking candy. The child stated that the licensee had giving them the candy.

Licensing observed the following in the kitchen: * The counter beside the stove in the kitchen was peeling making the surface difficult to clean or sanitise effectively. * The counters where food was being prepared or stored were cluttered with a variety of items such as a vase with a plant stem, various electrical cords, dirty dishes, and a toy car. * Uncovered food left in a frying pan on the stove. * A cooking pot on the counter with warm rice, and chicken. The Licensee stated that they had cooked the food 1/2 an hour ago and were waiting for it to cool. * A plate of food with another plate on top of it, the Licensee stated at first it was for the baby, then stated it was a child's food from the day before that they hadn't eaten. Licensing stated that this was not appropriate and then the Licensee stated that it was on the counter as they were going to throw it in the garbage and then moved it to another counter, then the Licensing Officer asked the Licensee to then throw it in the garbage. * The fridge appeared to be over crowded with food piled on top of each other, uncooked ground beef was piled on top of a basket with different items in it and on top of a plate of food that was not covered.

Children are supervised at all times by a person who is an educator, an assistant or a responsible adult. [CCLR Sec. 39(1)]
Observation. When Licensing arrived and pressed the buzzer to be let in it rang and then went to voice mail. Licensing then observed the Licensee at the front door of the building and they let Licensing into the building, as the Licensee and Licensing went towards the Licensee's apartment they observed two children unsupervised in the hallway of the building. When Licensing went into the apartment they also observed two children in the apartment unsupervised. During the inspection Licensing observed one child alone in the kitchen, the child was sitting on the floor opening up one of the drawers and taking plastic containers out of the drawers and putting them in their mouth. When the Licensing Officer informed the Licensee, the licensee took the containers out of the child's hand and put them back into the drawer and picked the child up and brought the child into the main activity area. This happened on two separate occasions. Licensing also observed the Licensee leave the children unattended

Observation. CCFL observed that the environment is not healthy or safe, and the Licensee's physical redirection of children does not promote their dignity. This is a recurring issue of non-compliance and has been in non-compliance sine September 17, 2021

Observation. CCFL observed the following: * In the primary bedroom there were various items such as clothes, bedding, boxes, small blue chairs, plastic bags, food scattered on the floor and on furniture in the room. CCFL observed the new bed. * The primary ensuite bathroom had an excessive amount of clutter such as an open bag of flour on top of a rug in the bathtub, along with shelving, various packages of rice stored on shelving in the bathtub, cleaning supplies, boxes, paint cans, trash bags, and candles. found in the bathroom. The Licensee informed CCFL that this ensuite was not in use. * There is no mechanism in place to prevent children's access to this bedroom or bathroom. * The counter beside the stove in the kitchen was peeling making the surface difficult to clean or sanitise effectively. * Bi-fold closets in the nap room did not have locking mechanisms - therefore could be a potential pinching hazard. The Licensee informed CCFL that the spaces would be re-organized and de-cluttered over the weekend. However this was also stated during the routine inspection conducted on September 17, 2021

Observation. During the inspection the LO removed a small doll's hair brush from a toddler's mouth CCFL observed small choke-able pieces in the drawers and bins in the living room that were accessible to the children It appears that the children's nap room is also being used for storage. CCFL observed the following in the nap room: * electrical cords for a sleep machine, a floor model air conditioner as well as a heater accessible to the children * two red metal cabinets that could easily be tipped over were accessible to children * plastic bags holding various items were accessible to the children * a child's bike and scooter * blue chair, with a high chair on top * a Christmas tree box * boxes with an assortment of toys * diaper creams and diaper wipes were on a trolley that was accessible to the children during the inspection. The Licensee moved the diaper creams from the trolley, however placed them in a green basket that was still accessible to the children.

Observation. During the inspection CCFL requested the Licensee to provide identification which demonstrates that this premise is the Licensee's personal residence. CCFL requested this information from the Licensee due to the lack of bedroom, living and dining room furniture. After CCFL suggested to the Licensee about locating her purse, the Licensee continued to look in drawers in an attempt to locate information. The Licensee was unable to produce any documentation - for example driver's licence, personal health card, or any other identification - that reflects this information.

Observation. CCFL observed by the sliding doors in the living room a phone cord, a power cord and copper wire from lights hanging down from a high shelf. These wires were accessible to the children.

During the inspection the LO removed a small elastic hair band from a toddler's mouth CCFL observed small choke-able pieces in the drawers and bins in the living room that were accessible to the children CCFL observed a gate in place between the living room and the hallway as a means to try and prevent children from accessing the hallway and kitchen, however during the inspection CCFL observed a child crawling underneath the gate CCFL observed the following in the nap room: * electrical cords for a sleep machine, a floor model air conditioner as well as a heater accessible to the children * two red metal cabinets that could easily be tipped over were accessible to children * an aluminum baking dish contained various items such as pens, pencils and craft items that could potentially be a choking hazard * plastic bags holding various items were accessible to the children * diaper creams and diaper wipes were on a trolley that was accessible to the children

Observation. CCFL observed that the emergency supplies were not adequate. They included cheerios, crackers and cookies all with expired best before dates of 2018 and 2019. There were only two bottles of water and the bottles themselves had been used previously to hold other liquids. CCFL also observed a package of diapers and wipes.

Observation. During the inspection two children's registration information was sent to the Licensee electronically from her adult daughter. One child's file did not contain immunization status and one child's file did not contain a personal health number.

Observation. CCFL asked the Licensee if they check the parks for hazards prior to children's play. The Licensee informed CCFL that this does happen. When asked if the documentation presented to CCFL indicates that the parks are checked, the Licensee said no.

Implementation of the fire drill system is practiced at least once each month, and implementation of the emergency plan is practiced at least once each year. [CCLR Sec. 22(2)(b)(c)]
Observation. CCFL observed a record showing that a fire drill had been done August 11, 2021 and a record of fire drills conducted in January and February 2021. The Licensee was unable to provide a record for the months March 2021 to July 2021. No record was observed to account for the yearly emergency drill.
