Observation. Seven children's files were reviewed and three children did not have evidence of Medical Insurance Plan number or immunization status available.
Corrective action. Ensure children's registration is complete including CC # and Immunization status.

Observation. Seven children's files were reviewed and one child did not have evidence of enrollment date available.
Corrective action. Ensure all registration forms are complete including enrollment dates.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. Seven children's files were reviewed and two children did not have evidence of emergency contacts available.
Corrective action. Ensure all registration forms are complete including emergency contacts.
