  Observation. Pair of adult scissors were accessible to children in both classrooms.

  Observation. Copies of care plans do not include symptoms and the course of action recommended by health care professionals for a child requiring additional support.

  Corrective action. Staff said that they will revise the copies of care plans.

  Observation. In the outdoor playground there is a large rolling gate on the east side of the yard that is in disrepair and unsecured.

  Corrective action. Ensure the facility is in good repair at all times.

  Observation. There was no evidence of the qualifications required under Division 2 [Employee Qualifications] for one staff member.
