Observation. The following was noted at today's inspection: -Emergency exit door that opens to personal residence was ajar at the time of inspection, discussion with Staff about solutions that would keep this inaccessible to children. (repeat) -Multiple Skateboards and various scooters for older children accessible to the children in care. Manager stated that would store these items in the shed. -There is a mirror type wallpaper that is quite sharp at edges and broken in some areas.
Corrective action. Please provide a written response to indicate how you will address the contravention noted above on or before June 20, 2024
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Some of the walls are in disrepair, or chipped paint, there were a few screws exposed within the drywall where the drywall can be picked at. Discussion had with staff about filling in the holes so exposed drywall is not accessible to children in care.

11020 - CCLR s.13(2) - POLICIES AND PROCEDURES
A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Currently the facility practices the emergency drill by walking to the fire drill location with their emergency supplies. Discussion had with Staff about the intent of the emergency drill. Location is intended to be off site and it is recommended that it be a location where the children can seek shelter. It is recommended that the facility practice walking to this emergency location with the children in care and the emergency supplies.
Corrective action. Please provide a written response and updated emergency policy on or before June 20, 2024.
13020 - CCLR s.22(1)(b) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. 8 children's files reviewed, one child was missing the Dr's phone number within their file.

Observation. Licensing observed the following; - Two nails sticking at the bottom of the wooden fence along one side of the outdoor play area. - A wooden panel loose creating a space where a child's finger can get caught and pinched in it. - A dirty, wet rag on the grass outdoor area. - The emergency door was open. The door leads into the Manager's private residence front door entrance area and the rest of the residence. This is a repeat.
Corrective action. Ensure that a healthy and safe environment is provided at all times while children in care.

In respect of a record referred to in this Division, a licensee must (a) keep in a single place at the community care facility the records referred to in sections 56 to 58.
Observation. Licensing was unable to review the facility's active play and screen time facility, as the Manager was unable to find it. Licensing discussed ensuring that all records remain in a single place at the community care facility.
Corrective action. Ensure that all staff, children and facility records are kept in a single place at the community care facility.

A licensee must ensure that children have at all times immediate access to an employee who (a) holds a valid first aid and CPR certificate, provided on completion of a course that meets the requirements of Schedule C.
Observation. Licensing observed that one staff's first aid has expired in June 2023. The staff was unaware that their first aid had expired and works alone with children.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Licensing observed the following that are accessible to children; - The children play on the upstairs patio that has a propane tank, big barbecue and gas fire pit. Licensing provided education that if the children would like to use this area it would need to be made safe. - Toilet bowl scrubber sitting beside the toilet on the floor without a cover. - Diaper rash ointment on the diaper change table.
Corrective action. Ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
11200 - CCLR s.17 - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following; - The diaper change foam mat, has a rip and two dirty pieces of scotch tape on it. Licensing provided education that this does not allow for effective sanitizing of the mat. - a foam mat in one of the play room is ripped.
Corrective action. Ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following; - A dirty mop and another dirty mop bottom in the covered cement outdoor area. The Manager informed Licensing that the dirty mop bottom was supposed to be thrown away and the dirty mop was put outside to dry. Licensing provided education that once children are outside it needs to be removed so children do not have access to them. - Licensing observed the emergency door was wide open. The door leads into the Manager's private residence front door entrance area and the rest of the residence.
Corrective action. Ensure that a healthy and safe environment is provided at all times while children are in care.
11010 - CCLR s.13(1) - HYGIENE AND COMMUNICABLE DISEASE
A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.
Observation. Licensing observed the following; - The soap in the bathroom was extremely diluted with water. Licensing has previously provided education to not dilute soap with water. - A training toilet seat sitting on the floor beside the toilet and touching the toilet bowl scrubber. -A dirty and smelly cloth that is being used to clean the tables. The Manager removed the cloth. Licensing provided education. - Children's mats are being storing standing up against each other. The fitted sheets are on the mats and touching each other. The Manager corrected this.
Corrective action. Ensure that a program is established to instruct children in and to practice the rules of health and hygiene.
15030 - CCLR s.46(1) - OR SUPERVISION
Without limiting subsection (1), a licensee must ensure that (a) children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E.
Observation. Licensing observed the following; -Licensing asked the Manager how many children were in care and the Manager informed Licensing that there is always only eight children in care which is the maximum capacity of the community care facility. However, upon a review of the attendance record for July 18, 2022, Licensing observed the there were nine children in care. - There were four days in the months of June and July that there were four children in care under the age of 3 yrs. old. Licensing provided education that it is very important to ensure that there are never more then eight children in care at any given time.
Corrective action. Ensure that the children attending the community care facility are divided into groups, with each group having no more children than that permitted in Schedule E.
14020 - CCLR s.34(2)(a) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. Upon a review of the attendance records for the month of July, Licensing observed that there was one day that a child was not signed in .
Corrective action. Ensure that attendance records are accurate on a daily basis.
19090 - CCLR s.57(2)(c) - STAFFING
A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation)
Observation. Upon a review of two staff records, Licensing observed that one staff's certificate has not been verified through the Early Childhood Educator (ECE) registry. Licensing provided education that all renewed and new certificates need to be verified through the ECE registry.
Corrective action. Ensure that staff records are complete.
12140 - CCLR s.19(4)(a) - STAFFING
The licensee must not permit a person over the age of 12 to be ordinarily present on the premises of a community care facility while children are present, unless the person is of good character and the licensee has obtained a criminal record check for that person.
Observation. Licensing observed that there is an individual over the age of 12 that opened the door of the community care facility. The Manager informed Licensing the individual sometimes helps out at the facility however, there is no criminal record check (CRC) for the individual. The Manager was unaware that a CRC was required for the individual. Licensing provided education that any person over the of 12 that is ordinarily present on the premises requires a CRC.
Corrective action. Ensure that CRCs have been obtained for any persons over the age of 12 that are ordinarily present on the premises of the community care facility while children are present.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following; - There is a swing set that has been installed in the outdoor play area. The swing set is very close to the back wooden fence. Potential for a child to hit their body against the wooden fence if they were swinging on the swings. - There isn't sufficient resilient surfacing under one of the swings. Licensing provided education on the importance having resilient surfacing under the swing set to prevent a child from injuring themselves. - The bedding on the children's sleeping mats are touching. Potential for cross contamination and spreading of germs. Licensing provided education on ensuring children's bedding is not touching especially during a pandemic.
Corrective action. August 31, 2021.

Observation. Licensing observed the following that are accessible to children in care; - An open soiled diaper in one corner of the covered cement outdoor play area. - A diaper rash cream on the diaper change table located in the main play area. - A corner part of an outside drainage in the outdoor play area. There are sharp edges on the drainage part.
Corrective action. Ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
11200 - CCLR s.17 - OR SUPERVISION
A licensee must ensure that children are supervised at all times by a person who is an educator, an assistant or a responsible adult.
Observation. Licensing observed the following; - The gate to enter into the facility premise was locked. After speaking with an individual, Licensing was informed that they would open the gate for Licensing. After 5 minutes the staff from the facility came out to open the gate for Licensing, leaving the seven children in care unsupervised. Licenisng observed that there were children's snacks on the table. Licensing provided education on the importance of ensuring that children are supervised at all times especially if they are eating. - While Licensing was speaking to the Manager, Licensing observed two children screaming and fighting for a couple of minutes. The staff who was supervising the children was in another room on the phone. Licensing suggested, to the Manager, having a discussion with the staff regarding active supervision and phone usage while children are in care. Licensing informed the Manager that if the gate is going to remain locked then a system needs to be in place where the children will not be left alone so that the staff can open the gate.
Corrective action. Ensure that children are supervised at all times.
14060 - CCLR s.39(1) - RECORDS AND REPORTING
A licensee must have in writing from a parent, and maintain at the community care facility, consent (a) to call a medical practitioner or ambulance in case of accident or illness if the parent cannot immediately be reached.
Observation. Upon a random review of six children's records, Licensing observed one child missing consent to call a medical practitioner or ambulance in the event of an accident or illness if the parent cannot immediately be reached..
Corrective action. Ensure that children's records are complete prior to the child attending the community care facility.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. Upon a review of the attendance records for the month of August 2021, Licensing observed the following; - Three days that several children were not signed out. - Upon discussion with the staff and Manager there was a child that attended the facility today (Aug.23/21) and had left at 2:30 PM. The child was not signed in or out for the day. Education provided on the importance of ensuring that attendance records are accurate. This is a continuous repeated contravention.
Corrective action. Ensure that attendance records are accurate on a daily basis.

Observation (CORRECTED DURING INSPECTION): Upon arrival Licensing observed 4 children in care under the age of 36 months old. The Manager informed Licensing that this only happened today. Licensing provided education, for the service type that is being provided, allows for no more than 3 children under the age of 36 months.
Corrective action. Please ensure that children attending the facility are divided into groups, with each group having no more than that permitted in Schedule E.

Observation. Licensing observed the following in the outdoor play area; - A tricycle with rust on its handle and the rubber ripped off the handle exposing the metal underneath. The expose metal handle is bent and sharp. Potential of a child cutting themself. - The lid of a plastic toy storage box is cracked on the top. This is a repeat contravention.
Corrective action. Please ensure that all indoor and outdoor play materials and equipment accessible to children are safely constructed, free from hazards and in good repair.

Observation. Licensing observed only four bottles of water available and that all the food items and juice boxes have expired in the on wheels emergency bag. Licensing provided education on having a system in place to maintain emergency supplies. This is a repeat contravention.

In respect of a record referred to in this Division, a licensee must (c) produce records, on demand, to the medical health officer.
Observation. There was a child in care whose record Licensing requested to review. The Manager looked for the child's record for more than 15 minutes and was unable to produce the child's record. The Manager informed Licensing that the child attended last year and just returned now. Licensing provided education on the importance of having and maintaining children's records prior to the child attending the care facility.
Corrective action. Please ensure that records are available for Licensing to review.
