A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. Licensing observed the following in the outdoor play area; - entire area covered in leaves - broken wicker couches

Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

Observation. Licensing observed the following that are accessible to children in care; - large wooden pole on the ground in the outdoor play area. - rat box in the outdoor play area - bleach and water solution on the counter - chemical solution bottle in the first aid kit on the counter. The Manager removed the bottle and placed it on a higher up shelf.

Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.

A licensee must ensure that each piece of furniture or equipment for sleeping (a) is used by only one child at a time, (b) is clean and comfortable.

Observation. There is no mat available for one child to sleep on whenever required.

Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.

A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.

Observation. Licensing observed the following; - two children's blankets stored together. Potential for cross contamination. - the mat used for diaper change is used for one child as napping mat.

Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.

A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager.

Observation. The Manager's name was not posted anywhere within the community care facility.

Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. The emergency bag has no water inside it and some of the food has expired.

Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.

A licensee must keep current records of each of the following: (d) written policies and procedures that are intended to guide employees in the care and supervision of children.

Observation. There was no written policies and procedures regarding, behavioural guidance and reportable incidents.

Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

Observation. Children are not identified as absent when they don't attend the community care facility.

Corrective action. Will ensure that the attendance records is accurate on a daily basis.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. Upon a review of two children's records, Licensing observed that emergency contact information was not obtained for one child.

Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (c) a record of the person's work history.

Observation. One staff's work history record was not obtained.

Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.

A licensee must keep current records of each of the following: (b) for each employee, the records required under section 19 (1) [character and skill requirements].

Observation. One staff's criminal record check is not current and expired in 2023.

Corrective action. Needs to apply for a CRC .

A licensee must keep current records for each child showing (a) the information set out in subsection (2), (b) if applicable, the information and agreements set out in subsections (2.1) and (2.2), and (c) the consents referred to in subsection (3).

Observation. Upon a review of the attendance records, Licensing observed a child in care on several days. However, upon discussion with the staff, Licensing learned that the child does not have a file in place. Licensing provided education to the staff and Manager on when files are required.

Corrective action. A written response to be submitted to Licensing on how this contravention has been addressed.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. Licensing observed the following; - Areas of wear and tear showing on the walls - Wooden car table where the paper has ripped on the table.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

Observation. Licensing observed the following that are accessible to children in care; - A 1L bottle of hand sanitizer on a desk at the front entrance. - Toilet chemical cleaning solution bottle in an unlocked bathroom cupboard.

A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.

Observation. Licensing observed that the table that the children sat down to eat snack was not sanitized by the staff prior to the children having their snack. The staff said that they forgot to.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must display in a prominent place in the community care facility the certificate, or a copy of the certificate, of any educator or assistant employed to work in the community care facility.

Observation. Licensing observed that there is a staff who works in the afternoons however, their early childhood educator (ECE) certificate has not been posted anywhere within the community care facility.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must (a) ensure that a community care facility has (i) emergency exits that meet the requirements of the BC Building Code, and (ii) a fire drill system that is approved as definedby section 31 of the Fire Services Act.

Observation. Licensing was unable to verify if the fire extinguisher had been serviced in a year or when it was purchased. The Manager was unable to verify when the extinguisher was purchased.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

In respect of a record referred to in this Division, a licensee must (a) keep in a single place at the community care facility the records referred to in sections 56 to 58.

Observation. Licensing was unable to find one staff's ECE certificate. The Manager had to email it to Licensing once Licensing had completed the inspection and left the facility. Licensing discussed that all facility records (staff, children and policies) need to be kept at the facility in a single place for Licensing to have access to and review.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. Upon a review of three children's records, Licensing observed that immunization status was not obtained for three children.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

Observation. Upon a review of the attendance records for the month of September 2024, Licensing observed two days that children's time of departure was not recorded.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. Upon a review of three children's records, Licensing observed the following; - The telephone number of a medical practitioner was not obtained for one child - The name and telephone number of a medical practitioner was not obtained for one child

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.

Observation. Upon a review of three children's records, Licensing observed that a photograph or digital image of two children have not been obtained.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (a) the diagnoses relevant to the child's requirement for extra support, as made by health care professionals.

Observation. Upon a review of a child's record and discussion with the staff, Licensing observed that there is a child that has a diagnoses and requires extra support however, there is no care plan available for a child requiring extra support. Licensing discussed with the staff and Manager care plans, what to include in them and how often to review them.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must keep, for each child, a record showing the following information: (b) date of enrolment in the community care facility.

Observation. Upon a review of three children's records, Licensing observed that the date of enrolment was not obtained for one child.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

Observation. Upon a review of two staff records, Licensing observed the following; - Character references not obtained for one staff - Only one character reference obtained for one staff Licensing discussed with the Manager on the telephone what would meet the intent of this regulation.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (c) a record of the person's work history.

Observation. Upon a review of two staff records, Licensing observed that one staff's work history record was not obtained.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.

Observation. Upon a review of two staff records, Licensing observed that immunization status was not obtained for one staff.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation)

Observation. Upon a review of two staff records, Licensing observed that both staff's certificates have not been verified through the early childhood educator (ECE) registry.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must ensure that children have at all times immediate access to an employee who (a) holds a valid first aid and CPR certificate, provided on completion of a course that meets the requirements of Schedule C.

Observation. Upon a review of two staff records, Licensing observed that one staff's first aid and CPR certificate was not obtained, therefore, Licensing could not verify if the staff holds a valid first aid certificate. Licensing was informed that the staff works alone with the children in the afternoons.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must ensure that each employee (b) practices implementing the fire drill system at least once each month.

Observation. Licensing observed that fire drills were not practiced for the months of July and August 2024

Corrective action. A written response to be provided to Licensing on how this contravention will be addressed.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

Observation. Upon a review of two staff records, Licensing observed that a criminal record check was not obtained for one staff.

Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. While on site, the following was noted: -Noticeable unpleasant smell possibly coming from the washroom, deep clean needed in order to find out where the strong odor is coming from. -Walls in need of a wipe down as there is visible dirt and finger prints along wall and by light switches.Visible dirt along ledges and moulding that runs along the facility. -Corner of some ledges and by entry, paint has started to peel.

Corrective action. Please ensure facility is clean and in good repair.

Observation. No sign is posted to show who is the Manager for the facility.

Corrective action. Please ensure Manager's name is posted

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. -No information provided to indicate when the extinguisher was purchased or serviced. Manager stated that the receipt must have gotten ripped off the extinguisher. -Food within emergency bag has a best before date of 2022. Limited items within the emergency bag. -Limited amount of Band-Aids available within the First Aid kit.

Corrective action. Please ensure facility is able to prepare, mitigate, respond and recover from an emergency.

A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.

Observation. Manager was not familiar with a minor incident log book. Currently there is no log book maintained for incidents that involve children.

Corrective action. Please ensure a minor incident log book is maintained.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. -There is still a hole in the ground within the outdoor play space. The hole contains a piece of concrete which can act as a tripping hazard. A hockey net has been placed on top of the hole, however, it is no a permanent solution (repeat). -Two blind cords that are accessible to the children in care. Discussion had with staff about placing the cords up high so children cannot reach them.

Corrective action. Please ensure a healthy and safe environment.

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. Fire extinguisher has not yet been serviced (repeat).

Corrective action. Please ensure facility is able to prepare, mitigate, respond and recover to an emergency.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. The following were noted: -Areas where wear and tear has started to show, i.e. ledges where paint has peeled, trim by floor where paint has peeled. -Some areas that are in need of cleaning, ledges, tops of trim, floor at today's inspection had many pieces of jelly clings and pieces of debris on the ground etc.

Corrective action. Please ensure facility is clean and in good repair.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. The following were noted at today's inspection: -Change table which had previously been discussed in a follow-up inspection is still on site and stored in the bathroom against the wall, acting as a topple hazard. -Heater was very hot. Staff indicated that they only turn on the heater that is under a bench, however, it is still accessible to the children in care and could cause injury. -Tacks on wall above bench are accessible to children in care. -Staff's personal belongings were stored on the ledge, accessible to children (corrected). -Outside in play space, large hole in ground along with a piece of concrete near the hole which can act as a tripping hazard. -Play structure is located right next to the fence, as this is a climbing structure, there should be approximately 6 ft clearance from the fence.

Corrective action. Please ensure a healthy and safe environment.

A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].

Observation. Staff could not locate where or whether there was record respecting compliance to carrying out the fire and emergency drill for the facility.

Corrective action. Please ensure there is records of the fire drill and emergency drill which is accessible in order for Licensing to confirm compliance with section 22(2)(b)&(c)

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. Fire extinguisher was due to be serviced in March of 2022.

Corrective action. Please ensure fire extinguisher is serviced at least one time per year.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. 7 children's files reviewed, 3 children were missing Dr information and 4 children were missing emergency contact information.

Corrective action. Please ensure children's files are complete.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. 7 children's files reviewed, 7 children were missing immunization information.

Corrective action. Please ensure children's files are complete.

A licensee must ensure that each employee (c) practices implementing the emergency plan at least once each year.

Observation. Staff was not familiar with Emergency drill which is required to be practiced one time per year.

Corrective action. Please ensure that all staff practice implementing the emergency plan and is familiar with the emergency plan.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. The following were observed and are repeat contraventions: -No papertowel was available for drying hands after handwashing. There is a small container placed near the sink which usually has paper towels, however, it was empty when LO went to wash their hands. -Thumb tacks are used for name decals for the children and are accessible to the children in care.

Corrective action. Please ensure a healthy and safe environment.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.

Observation. Immunization status was not obtained for one employee.

Corrective action. Please ensure staff files are complete. As discussed with staff, should they not have records of their immunization status, a declaration of immunization status is also accepted.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person. Observation (CORRECTED DURING INSPECTION): Only an expired criminal record check was available for review within the staff's file. Manager contacted LO after inspections and provided an up to date criminal record check for the staff.

Corrective action. Please ensure the most updated criminal record check is accessible for review.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. The following was noted at today's inspection: -Broken bouncy horse accessible stored in outdoor play space and accessible to the children. -A few holes in the wall which require maintenance.

Corrective action. Please ensure facility is clean and in good repair.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. The following was observed at the time of inspection: -Heater guard behind/beside the bench was hot to the touch. Some discussion had with Licensee in regard to how to address this concern. -Licensee provides paper towel to the children when they come out of the washroom. Discussion had about having papertowel more readily accessible in the washroom or directly beside the washroom so children can dry their hands right away. -Electrical kettle accessible to the children in care. -Closet has not door and has many items stacked inside (i.e. sleep mats, puzzles, etc). Could be a topple hazard. -Tacks used for posting art and information are accessible to the children in care.

Corrective action. Please ensure a healthy and safe environment.

Without limiting subsection (1), a licensee must ensure that (a) children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E.

Observation. One child who is enrolled in the program is under 30 months of age. Discussion had with the Licensee in regard to meeting Schedule E of the Child Care Licensing Regulation (CCLR).

Corrective action. Please ensure that children enrolled in the program meet the criteria set out in Schedule E of the CCLR. Should the Licensee want to continue care to this child, an application for exemption must be submitted and be approved by Licensing.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

Observation. Licensing has not obtained a criminal record for their staff. Discussion had with the Licensee in regard to staff not working alone until a cleared site specific criminal record check is obtained.

Corrective action. Please ensure a site specific criminal record check is obtained for all staff.
