    Observation. Licensing observed the following;- -several buggies outside that have debris and stains on them. - metal toilet paper holder, n the group child care (30 months to SA) bathroom, broken. - a bed sheet, in the group child care (under 36 months), dirty with stains and debris on it. Discussed cleaning of bedding.

    Observation. Licensing observed the following;- - several broken items stored outside in the group child care (30 months to SA) playground. Such items included a ripped crib mattress, a broken metal crib spring and other items. - In the group child care (under 36 months), several older children sitting in smaller high chairs that were not age appropriate and strapped into the chairs. Licensing discussed age appropriate furniture for children and when it is acceptable to strap children in chairs.

    Corrective action. A written response to be provided to Licensing on how this contravention has been addressed.

    A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. There is a shrub within both outdoor play areas that grows red berries. Manager did not know if the berries were safe for the children should they be consumed. The berries could be potentially hazardous to the children should they be ingested.

    Corrective action. Discussion had with the Manager about finding out whether the shrub is a hazard to the children in care (if it is not and the facility would like to keep the plant, provide documentation to show that it is not a hazard). Should the plant pose a hazard, the facility must ensure that berry is not accessible to the children in care.

    Without limiting subsection (1), a licensee must ensure that (b) the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E. Observation (CORRECTED DURING INSPECTION): Manager's ECE Certificate expired last year. An exemption was in place until January 1, 2024 in order to allow for the Manager to work within ratio while they renewed their ECE Licence. Manager has not yet obtained their Licence and has been working alone within the GCC 3-5 program from 7:30 to sometimes 9AM since the exemption expiration.

    Corrective action. Manager had the completed exemption paperwork on their desk and it was provided to Licensing at today's inspection. The exemption was verbally approved.

    A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

    Observation. In the morning, when ratio within the GCC 3-5 program exceeds 8 children and there is only one staff, an IT staff will come over to act in ratio within the GCC 3-5 program. According to the Manager, today, an IT staff came over to help within the GCC 3-5 program and brought over 2 children from the IT program who were between the ages of 30-36 months. This movement of children was not reflected within the attendance records.

    Corrective action. Discussion had with Manager about ensuring that attendance reflects children's movements within the programs. Manager stated that they would ensure that this is done going forward.

    Observation. Few outstanding items noted: -Facility has installed some fencing which keeps the children away from the slats which were considered an entrapment hazard within the fence. LO still has concerns in regard to access to the ledge in which the fence is installed as children can still climb onto this ledge and the fence height is not very high. Please contact Licensing Officer to discuss. -Still some areas where there is some garbage piled up and still accessible within the IT outdoor play space and still some old cubbies accessible to the GCC 3-5 outdoor play space. -Toilet paper not placed on toilet paper roll. -Red bar on outdoor play structure is not secured.

    Corrective action. Please ensure a healthy and safe environment.

    A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. The following were noted: - Various broken toys and broken equipment accessible in both the Infant Toddler and GCC 3-5 outdoor play area. -Shrubs that run along the fence within outdoor play space have slowly died off over the years. The shrubs had helped keep the children away from the fencing which is climbable in some areas and has large gaps between the slats (4 inches) which can act as an entrapment hazard. -Some areas where netting has been added to fence to help with entrapment has started to come undo and making the fence accessible. -Emergency backpack hangs on a screw which does not appear to be fully secure and can be a topple hazard . -New cubbies in GCC 3-5 room can act as topple hazard as they are not secured to the wall. -Toilet paper not placed on the roll within GCC 3-5 program.

    Corrective action. Please ensure a healthy and safe environment.

    Observation. The following was noted: -Chip in drywall within the Infant Toddler program -Crib in Infant Toddler nap room is taped which does not allow it to be easy to clean and it unclear as to why it has taped on one end. -Water damage along the sinks within the GCC 3-5 program which does not allow it be easy to clean. -Red rod which appears to keep climbing structure in GCC 3-5 playground has become unattached to the structure.

    Corrective action. Please ensure facility is clean and in good repair.

    Observation. The following was noted at today's inspection: -One staff has an expired ECE and is therefore only considered a responsible adult. This staff had been working as the second staff on shift within the GCC 3-5 program. -There is no IT staff working from 4:30-4:45 until closing at 6 PM within the IT program -Staff who regularly closes within the IT program is an ECEA. -Exemption had been put in place which included a staffing plan allowing for another staff work in the IT. The staffing plan changed without contacting the Licensing Officer. This staff has not worked within the Infant Toddler program since April.

    Corrective action. Please ensure that the staff to child ratio is as per Schedule E.

    Observation. One staff had an expired criminal record check from April, a note on file indicates that someone had applied for a new criminal record check in July, however, it is not yet on file. Another staff does not have a criminal record check on file and there is a note that they applied for the CRC in July, however, nothing has been added to the file. One staff does not have a site specific criminal record check. A note on file indicates that facility applied for a site specific CRC, however, it does not indicate when this was done.

    Corrective action. Please ensure that all staff valid, site specific criminal record checks.

    Observation. There are limited toys and play equipment available for both programs and minimal variety based on the number of children who are in enrolled in the program. Some of the shelves within the Infant Toddler program are bare. Manager advised that what toys are visible on the shelves and in one storage room is what is available for the program.

    Corrective action. Please ensure that facility has materials that encourage creative endeavors, such as art, music, movement, imaginative play, story telling and construction and a variety of materials are available.

In reviewing the staff schedule it was determined: -Within the Infant Toddler program, the second staff who comes on shift is an ECEA as opposed the required ECE. -The staff who closes the Infant Toddler program is an ECE as opposed to the requirement of having an IT on site at all times. -Manager who would currently be considered a Responsible Adult as their ECE has expired has been opening the Groups Child Care program (3-5) and works alone in the program until 9AM when another ECE comes on shift.

    Corrective action. Please ensure the facility staffs the facility according to Schedule E

    A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. The following items were noted at today's inspection: -Latch at daycare entrance is loose and difficult to attach. -Shoe rack that is used for diapering storage is on top of toilet (not attached to wall) can act as hazard should it fall. -The sinks operate by sensor and 3 out of the 5 sinks did not work because the battery for the sensor were dead. One sink was continuously running due to an issue with the sensor. -The facility uses single use towels for drying their hands. The Manager indicated that the children are instructed to throw the towels on the ground and staff would put them in a small green basket beside the sink. This is not considered to be a sanitary practice and the dirty laundry should not be stored in a manner that is accessible to the children. -Diapering mat in the 3-5 program was stored between the wall and the garbage can (unsanitary practice). -Sharp scissors were stored in a drawer that is accessible to the children within the 3-5 program. -Multiple items intended for storage/garbage were accessible to the children (vacuum, empty boxes, sensory boxes not used during COVID). -Plunger accessible within the IT bathroom and toilet cleaning brush accessible within the 3-5 washroom.

    Corrective action. Please ensure a healthy and safe environment.

    Observation. The following items were noted at today's inspection: -2 broken bikes and multiple broken push toys were accessible to the children in care. -Some areas within the IT room where there is paint that is peeling or damage to the wall (by change table and by door to 3-5 program). -Water damage below the sinks within the GCC 3-5 program which makes it difficult to clean and sanitize.

    Corrective action. Please ensure facility is clean and in good repair.

    Without limiting subsection (1), a licensee must ensure that (b) the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E.

    Observation. After the first half hour of missing groups, the staff who opens (ECEA) takes children into the 3-5 program where they work for a half hour until the ECE arrives. At the end of the day, the IT staff leaves a half hour before the facility closes. ECE staff was closing the IT program.

    Corrective action. Please note that after the first half hour of mixing programs, it is required that both program are staffed according to Schedule E (i.e. IT staff working in IT program and ECE staff working in 3-5 program).     A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (b) the courses of action recommended by health care professionals to address the needs of the child requiring extra support.

    Observation. There is a care plan on site that is quite vague in regard to their course of action. As well as the type of medication that is to be used.

    Corrective action. Please ensure the care plan's course of actions are easy to follow with clear direction.

    A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee.

    Observation. Licensing observed a log entry that indicated that the facility had closed a few months back as there was no power. Children were in care and were sent home. Licensing was not made aware of this service interruption. Manager was not aware that this was required to be reported on a reportable incident form.

    Corrective action. Please ensure that anything described in Schedule H is reported to Licensing within 24 hours of its occurrence.

    The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.

    Observation. -One care plan has not been reviewed in over a year. -One care plan has not been dated and therefore cannot be confirmed that it has been reviewed/or created in the past year.

    Corrective action. Please ensure care plans are reviewed yearly.

  • STAFFING

    A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

    Observation. Manager's criminal record check was not on site, there is evidence to indicate that they applied for the CRC a few months ago, however, it has not yet been received. Two staff do not have site specific criminal record checks. There is evidence to indicate that they applied to change the site of the criminal record check back in September, however, the updated CRCs have not been received. Expired CRC for Manager is a repeat contravention.

    Corrective action. Please ensure staff all have a valid criminal record check that is site specific. Please note the Manager cannot work alone until updated CRC is obtained.

  • STAFFING

    A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (d) copies of any diplomas, certificates or other evidence of the person's training and skills.

    Observation. A staff who has been acting as a ECE for the facility does not have their ECE Certificate within their file or posted on the wall.

    Corrective action. Please ensure that there is evidence of person's training is present within the staff file.
