Observation. A staff changing a child's soiled diaper on the diaper change table has been moved out of the bathroom and placed directly next to the fridge and not beside a wash basin, in one of the GCC (under 36 months) program.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following; - the tree roots have lifted in several areas of the group child care (under 36 months) playground. There are woodchips however the woodchips have depleted. Potential tripping hazard -the wood surrounding the woodchip area of the playground is slippery when wet. Potential falling onto concrete hazard. - the fridge temperature in one GCC (under 36 months) classroom was measured at 5.2 degrees Celsius and 10 degrees Celsius in the other classroom.
Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed; - outdoor toys and equipment are dirty and have green grime on them - many branches through the playground - the outdoor sandbox was not completely covered with the tarp - two table's legs rusted in one of the GCC (under 36 months) program - lots of debris and dirt on the carpets in one of the GCC (under 36 months) program - a carpet curling in each corner - a carpet inside frayed at a corner and outside ripped at one end - a wooden shelf chipped in several areas - a cloth rocker dirty and stained in several areas - the fridge in one of the GCC (under 36 months) classrooms had brown stains and dried food on it.
Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.

A licensee must supply equipment, furniture and supplies that are (a) of sturdy and safe construction, easy to clean and free from hazards.

11080 - CCLR s.15(1)(a) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Licensing observed the following that are accessible to children; - diaper rash creams in an unlocked drawer in one of the GCC (under 36 months) program. The staff locked the drawer. CORRECTED. - two dirty mop covers hanging on the bathroom sink - cleaning solutions in an unlocked kitchen cupboard in one of the GCC (30 mnths to SA) program - a screwdriver in an unlocked kitchen drawer

11200 - CCLR s.17 - HYGIENE AND COMMUNICABLE DISEASE
A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.
Observation. The bleach and water solution in a couple of the programs is changed once a week. The bleach does not effectively sanitize the surfaces when only changed once a week

15030 - CCLR s.46(1) - LICENSING
A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager.
Observation. The Manager's name was not posted anywhere within the community care facility.

10050 - CCLR s.11(1) - MEDICATION
If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (b) readily accessible to employees.
Observation. A child has a two medications identified in their care plan however, the medications are not available to the staff and thus not readily accessible to employees.

16020 - CCLR s.53(1)(b) - POLICIES AND PROCEDURES
A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Licensing observed the following; - no emergency plan in place to guide staff on how to respond in the event of an emergency - no age appropriate food for the children in the GCC (under 36 months) programs and insufficient food in the emergency bag to recover from an emergency.

13020 - CCLR s.22(1)(b) - RECORDS AND REPORTING

Observation. Licensing observed many of the sleeping mats ripped at corners and other places. Provided suggestions on other durable sleeping furniture such as mesh cots.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Licensing observed the following accessible to children in care; - several knives in an unlocked kitchen drawer in on of the group child care (30 months to SA) program. - Lysol container in on of the programs. The Manager corrected this by moving it out of children's reach. - bleach and water solution spray bottles in the group child care (SA) programs.
Corrective action. A written response to be submitted to Licensing on how this contravention has been addressed.
11200 - CCLR s.17 - HYGIENE AND COMMUNICABLE DISEASE
A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.
Observation. In one of the group child care (under 36 months), Licensing observed several of the sleeping mats stacked against each other and the bedding touching one another. Licensing discussed cross contamination and spreading of germs when children's bedding touch one another.
Corrective action. The Manager will ensure that staff do not stack children's mats against one another
15030 - CCLR s.46(1) - RECORDS AND REPORTING
In respect of a record referred to in this Division, a licensee must (c) produce records, on demand, to the medical health officer.
Observation. The Manager was not able to locate the fire drills and emergency drills for two programs for the year 2024. The Manager said that the drills had been conducted and documented but had been filed away and doesn't know where.
Corrective action. A written response to be submitted to Licensing on how this contravention has been addressed.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. The following was observed at today's inspection: Outside- -Wood perimeters and tree stumps within outdoor play space have algae film on top and can be slippery for children. -A few broken toys noted within the play area (2 imaginary play structures with broken pieces). Inside- -Cubbies within GCC 3-5 and School Age program are not secured to the wall and can act as a tipping hazard. -White crib within Infant Toddler nap room has some peeling paint on the top horizontal slat.
Corrective action. Please provide a written response to indicate how you will address the contravention noted above on or before February 6, 2024
11010 - CCLR s.13(1) - RECORDS AND REPORTING
The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.
Observation. The following was noted: -Care plan did not contain notation to confirm that it was reviewed in 2023. Manager stated that it was reviewed and there was no change. Discussion had about providing some kind of notation in order for LO to confirm. -A second care plan did not provide adequate detail in regard to the symptoms and course of action.
Corrective action. Please provide a written response to indicate how you will address the contraventions noted above on or before February 6, 2024

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. -Cubbies in School age program and Infant toddler program are made up with an MDF type material that is porous. This does not allow for the cubbies to be easily cleaned. Discussion had with Manager about treating cubbies with a non porous material which allows for the cubbies to be adequately cleaned. -Art shelf within GCC 3-5 room which is made of a raw wood some of the corners of the shelf have been taped, which does not allow for easy cleaning. -Leather chair in School Age room is ripped and not easy to clean.
Corrective action. Please ensure facility is clean and in good repair.

Observation. 12 children's files reviewed, 2 children were missing Dr information.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. 8 staff files reviewed, 3 staff files contained RCMP criminal record checks and one staff had an expired criminal record check.
Corrective action. Please ensure staff criminal record checks are obtained from MCFD. RCMP checks are not accepted for staff criminal record checks.

Observation. 8 staff files reviewed, one staff only had one reference within their file. Please ensure that multiple references are checked for staff.

12090 - CCLR s.19(1)(f) - STAFFING
A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation)
Observation. 8 staff files reviewed, one staff who recently updated their Certificate does not have proof of verification with the ECE Registry.
Corrective action. Please ensure staff who hold a Certificate have show proof of verification through the ECE Registry.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing noted the following: -Walls that had some holes or dents that were repaired with spackle but not repainted. -Some classrooms (GCC 3-5 & Preschool) where the paint has started to chip off the wall. -Some walls/door that are in need of a good cleaning.
Corrective action. Please ensure facility is clean and in good repair.
11020 - CCLR s.13(2) - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].
Observation. Licensing could not confirm compliance of the Emergency Drill as it was not documented within the drill logs for Infant Toddler, GCC 3-5 program and Preschool program. Manager had confirmed that the drill was carried out but does not appear to have been documented.
Corrective action. Please ensure record respecting compliance to section 22(2)(b) & (c) is available in order to confirm compliance.
13050 - CCLR s.56(1)(c) - RECORDS AND REPORTING
The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.
Observation. Licensing observed two care plans that have not been reviewed in over a year.
Corrective action. Please ensure care plans are reviewed with the parent every year to ensure the plans are clear and accurate. Please provide a notation such as a date the care plan was reviewed with the parent in order to confirm compliance.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. The following were noted at today's inspection: -There are plastic bags stored under the bathroom sink within the infant toddler bathroom and the lock does not work, making the bags accessible to the children should they be in he washroom. -Within the Infant toddler outdoor play space and in the back corner of the GCC 3-5 program play space, there is an accumulation of leaves around the perimeter of the play space, making it difficult to conduct an adequate safety check.
Corrective action. Please ensure a healthy and safe environment
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. The following were noted at today's inspection: School Age: -Room had some accumulation of dust in some areas. Various pieces of garbage and clutter throughout. A discussion had with the Manager in regard to ensuring that the rooms are cleaned thoroughly with garbage and debris being removed. Especially during the COVID-19 pandemic, regular cleaning schedules need to include addressing all areas of the classroom. -In the bathroom used by the school age program, the stools are quite soiled and need to be cleaned and sanitized. Group Child Care 3-5: -There is a wood table within this space that has started to chip paint at the corners making it difficult to adequately clean. -Various pieces of furniture within the first GCC 3-5 room makes the room cluttered and doesn't allow the children to spread out. Licensing Officer (LO) noted multiple pieces of unnecessary furniture throughout (multiple Sensory bins, step ladder, plastic bins). Having an excess of clutter also makes the space difficult to clean and sanitize. LO asked that multiple unnecessary items big and small be purged from this room. -Red leather chairs have started to rip at the seams, making them difficult to adequately clean. Outdoor play space: -Within the corner of the outdoor play space, there are various pieces of garbage - i.e. broken broom, broken plastic planters, broken toys. -Two pieces of equipment have broken steps and is not safe to use - Blue Little Tykes Elephant, Little Tykes slide.
Corrective action. Please ensure facility is clean and in good repair.
11020 - CCLR s.13(2) - RECORDS AND REPORTING
In respect of a record referred to in this Division, a licensee must (a) keep in a single place at the community care facility the records referred to in sections 56 to 58.
Observation. Manager and one staff could not locate care plans for the GCC 3-5 program. Manager stated that there were children who required care plans in this program but the care plans were not located where they are normally posted. When LO asked the staff, they stated that they did not know where the care plans were located as they were not a regular staff in this room.
Corrective action. Please ensure that care plans are easily accessible in a location where all staff (including coverage) know where they are located.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. 5 staff files reviewed, one staff's criminal record check was expired. Manager and staff both stated that they had applied for the CRC in December but have not yet received it. Please note that this staff should not be working alone with children under a cleared criminal record check has been obtained.
