  Observation. Emergency drill was not recorded last year.

  Corrective action. The manager will make sure to record drills moving forward.

• RECORDS AND REPORTING

  A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (b) the courses of action recommended by health care professionals to address the needs of the child requiring extra support.

  Observation. Children requiring additional support do not have care plans in place. Licensing thoroughly discussed this with the licensee.

  Corrective action. The manager will correspond with the parents to create care plans for these children.

• RECORDS AND REPORTING

  A licensee must keep, for each child, a record showing the following information: (f) any medication administered to the child, including the amount and the time at which the medication was administered.

  Observation. No record showing the amount and time that a medication was administered to a child.

  Corrective action. The manager will create a record including the amount and time that the medication is administered.

• STAFFING

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

  Observation. One staff is missing a current criminal record check.

  Corrective action. The manager already started the process and is awaiting results.

  12040 - CCLR s.19(1)(a)
• STAFFING

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (c) a record of the person's work history.

  Observation. Upon review of several staff records, two staff members are missing a record of their work history.

  Corrective action. The manager said he will obtain and keep this record.

  12060 - CCLR s.19(1)(c)

• A licensee, other than a licensee providing a care program described as Occasional Child Care, must provide each child with daily outdoor play periods unless weather conditions would make it unreasonable to do so. (4) Despite subsection (1), a licensee who provides a care program described as Occasional Child Care need not provide a program of outdoor activities. (5) Despite subsection (3), that subsection applies to a licensee who provides a care program described as Child-minding only if the program provides care for 3.5 hours or more each day.

  Observation. Staff have not provided all children opportunities for outdoor play since September 2025.

  Corrective action. The licensee will provide a plan to address the contravention by November 26, 2025.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. Two couch bench seats were observed at the time of the inspection to be worn and damaged. The LO reviewed this item with the manager during the inspection.

  Corrective action. Ensure furniture is in good repair.

• RECORDS AND REPORTING

  A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (b) the courses of action recommended by health care professionals to address the needs of the child requiring extra support.

  Observation. Three care plan's do not include the course of action recommended by health care professionals for a child requiring additional support. The LO reviewed this requirement in depth at the time of the inspection.

  Corrective action. Ensure care plans are in place for children who require extra support which includes the courses of action recommended.

• STAFFING

  A licensee must ensure that each employee (b) practices implementing the fire drill system at least once each month.

  Observation. Fire drill records were reviewed and one month was missed for the current calendar year. The LO reviewed this requirement with the manager at the time of the inspection.

  Corrective action. Ensure employees implement a monthly fire drill practice.

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

  Observation. A minimum of two character references were not obtained for one or more employees.

  Corrective action. Ensure staff files are complete including character reference checks.

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (d) copies of any diplomas, certificates or other evidence of the person's training and skills. Observation (CORRECTED DURING INSPECTION): Copies of any diplomas, certificates or other evidence of the person's training and skills were not obtained.

  Corrective action. Ensure staff files are complete including certificates of training.

Observation (CORRECTED DURING INSPECTION): Immunization status was not obtained for one or more employees.

  Corrective action. Ensure staff files are complete which includes immunization status.

  Observation. Four children's files were reviewed and one child did not have evidence of and emergency contact documented.

  Corrective action. Ensure all children's files are complete including emergency or alternate contacts.

  Observation. Licensing determined an appropriate ratio of staff to children was not always maintained.

• EQUIPMENT AND FURNISHINGS

  A licensee must supply equipment, furniture and supplies that are (b) located so as not to block or hamper an exit in the case of fire or other emergency.

  Observation. Several items stored near the emergency exit blocking the door should it we needed in the event of an emergency. This is a repeat contravention.

  Corrective action. Ensure an emergency exit is not blocked.

• POLICIES AND PROCEDURES

  A licensee must have all of the following: (b) an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

  Observation. The fire extinguishers in the facility have expired. LO reviewed these items with the manager and the Licensee during the inspection. This is a repeat contravention.

  Corrective action. Ensure emergency plan equipment is maintained in order to mitigate and respond from an emergency.

  13020 - CCLR s.22(1)(b)
• RECORDS AND REPORTING

  A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

  Observation. Five children's files were reviewed and none of the files had evidence of immunization status. LO reviewed this item with the manager and Licensee during the inspection. This is a repeat contravention.

  Corrective action. Ensure children's files are complete prior to the children attending including immunization status.

• RECORDS AND REPORTING

  A licensee must keep current records for each child showing (a) the information set out in subsection (2), (b) if applicable, the information and agreements set out in subsections (2.1) and (2.2), and (c) the consents referred to in subsection (3).

  Observation. Three children in attendance did not have registration information available on site. LO reviewed this information and the legislative requirements and purpose with the manager and Licensee during the inspection.

  Corrective action. Ensure all children's files in attendance are available at all times.

  19060 - CCLR s.57(1)(a)(b)(c)
• STAFFING

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (d) copies of any diplomas, certificates or other evidence of the person's training and skills.

  Observation. Two staff files were reviewed and one staff did not have evidence of Responsible Adult certification available. LO reviewed this with the manager and Licensee during the inspection. This is a repeat contravention.

  Corrective action. Ensure all staff files are complete including evidence of a persons training and skills.

• STAFFING

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

  Observation. Two staff files were reviewed and both staff had valid Criminal Record checks available however they were not specific to the facility. LO review the process for sharing CRC's with the manager and Licensee during the inspection. This is a repeat contravention.

  Corrective action. Ensure staff files are complete including facility specific CRC's prior to the staff working in the programs.

  12040 - CCLR s.19(1)(a)
• STAFFING

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

  Observation. Two staff files were reviewed and both staff were missing confirmation of reference checks being completed. LO reviewed this with the manager and Licensee during the inspection. This is a repeat contravention.

  Corrective action. Ensure all staff files are complete including confirmation of references being checked.

  12050 - CCLR s.19(1)(b)
• STAFFING

  A licensee must ensure that each employee (c) practices implementing the emergency plan at least once each year.

  Observation. Emergency plan records indicate that no drill was practiced or reviewed during the 2020 fiscal year. LO reviewed the requirement with the manager and Licensee during the inspection.

  Corrective action. Ensure employees practice implementing emergency plans at least once a year.

  12200 - CCLR s.22(2)(c)
• STAFFING

  A licensee must ensure that each employee (b) practices implementing the fire drill system at least once each month.

  Observation. Fire drill records were reviewed and no drills have been conducted in 2021. LO reviewed requirement for fire drills with the manager and Licensee during the inspection.

  Corrective action. Ensure monthly fire drills are conducted and recorded.

• EQUIPMENT AND FURNISHINGS

  A licensee must supply equipment, furniture and supplies that are (a) of sturdy and safe construction, easy to clean and free from hazards.

  Observation. The following items were identified as safety hazards during the inspection; - electrical cord plugged in near a sink - shelf that had been previously secured was not secure enough to prevent an injury in the event of an emergency - alarm on the back door leading to a common hallway was not working - BBQ stored on the floor blocking an emergency exit - shelf in one of the bathrooms not secured to the wall LO reviewed all of these items with the Licensee during the inspection. This is a repeat contravention.

  Corrective action. Ensure equipment, furniture and supplies are sturdy, safe and free from hazards.

  11080 - CCLR s.15(1)(a)
• HYGIENE AND COMMUNICABLE DISEASE

  A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene. Observation (CORRECTED DURING INSPECTION): A child experienced a nose bleed during the inspection. A staff wiped the area with a tissue. LO discussed with the staff the requirement for disinfecting the area with an appropriate sanitizing solution when bodily fluids are present. The staff then cleaned the area with a bleach solution. LO additionally spoke to the Licensee regarding staff training in health and hygiene.

  Corrective action. Ensure health and hygiene practices are maintained at all times.

  15030 - CCLR s.46(1)
• OR SUPERVISION

  Without limiting subsection (1), a licensee must ensure that (b) the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E.

  Observation. Seventeen children were in attendance with one staff member and one volunteer putting the program over the staff to child ratio for a portion of the inspection. LO reviewed the staff to child ratio and Schedule E with the staff and the Licensee during the inspection.

  Corrective action. Ensure the employees to children ratio follows Schedule E of the regulations.

  14030 - CCLR s.34(2)(b)
• POLICIES AND PROCEDURES

  A licensee must have all of the following: (b) an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

  Observation. Two facility fire extinguishers were overdue for servicing with expired safety tags in place.

  Corrective action. Ensure equipment is regularly serviced and capable of responding to and recovering from an emergency.

  13020 - CCLR s.22(1)(b)
• RECORDS AND REPORTING

  A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status. Observation (CORRECTED DURING INSPECTION): Six children's files were reviewed and two children did not have evidence of medical insurance plan numbers present. This is a repeat contravention.

  Corrective action. Ensure children's files are complete including Care Card Numbers.

  19070 - CCLR s.57(2)(a)
• RECORDS AND REPORTING

  A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency. Observation (CORRECTED DURING INSPECTION): Six children's files were reviewed and two children did not have evidence of photo's in place. This is a repeat contravention.

  Corrective action. Ensure children's files are complete including photographs.

  19160 - CCLR s.57(2)(i)
• RECORDS AND REPORTING

  A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

  Observation. Si children's files were reviewed and one child did not have evidence of a medical practitioner or phone number available.

  Corrective action. Ensure children's files are complete including Dr's name and phone number.

  19100 - CCLR s.57(2)(d)
• RECORDS AND REPORTING

  A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure. Observation (CORRECTED DURING INSPECTION): The facility attendance records were not accurate with the number of children in care during the inspection. Additionally children were not recorded as arriving and departing in the morning and afternoon after school. LO discussed with the Licensee the requirement of recording accurate arrival and departure times at all times.

  Corrective action. Ensure accurate attendance documents are maintained including arrival, departure and absent records.

  19090 - CCLR s.57(2)(c)
• STAFFING

  Despite this section, licensee may employ a person as a volunteer who does not provide care to children or supervise children if the licensee or the manager has first met with the person and obtained all of the following (a) a criminal record check for the person.

  Observation. A volunteer present in the program did not have evidence of a CRC in the staff file.

  Corrective action. Ensure volunteers have complete files including CRC status.

  Corrective action. Ensure staff files are complete including valid CRC checks that are in place prior to the staff working with the children.

  A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the licensee using a system established and maintained by the director of the early childhood educator registry, Observation (CORRECTED DURING INSPECTION): Five staff files were reviewed and one staff with and ECE certificate did not have evidence of the certificate being validated. LO discussed this practice with the Licensee during the inspection.

  Corrective action. Ensure all staff files are complete including validating ECE certificates through the ECE Registry.

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (d) copies of any diplomas, certificates or other evidence of the person's training and skills.

  Observation. Five staff files were reviewed and one staff was missing a 1st Aide Certificate and one staff was missing a Responsible Adult certificate. LO discussed with the Licensee the process for submitting an exemption application for a staff member to work as a Responsible Adult. This is a repeat contravention.

  Corrective action. Ensure all staff files are complete including 1st Aide Certificates and Training and Skills Certificates.
