  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. Fridge temperature at 7.4 degrees Celsius.

  Corrective action. Manager will obtain thermometer for fridge to ensure it stays at appropriate temperature.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. In the outdoor area, the wooden frame around the playground was separating at some places.

  Corrective action. Manager will have the frame repaired.

• RECORDS AND REPORTING

  A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

  Observation. Licensing reviewed eight child files, one child was missing MSP number, one child was missing doctor's information and two we missing emergency contacts.

  Corrective action. Manager will obtain missing information.

  A licensee must keep, for each child, a record showing the following information: (b) date of enrolment in the community care facility.

  Observation. Licensing reviewed eight child files, three were missing start dates.

  Corrective action. Manager will update the paperwork.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. The following was observed in the Preschool/School Age room: -several fluorescent tube lights are not working creating a dark environment -one light fixture does not have a cover, exposing a fluorescent tube light

  Corrective action. Manager explained she has contacted the church already to request the lights to be replaced.

• EQUIPMENT AND FURNISHINGS

  A licensee must supply equipment, furniture and supplies that are (a) of sturdy and safe construction, easy to clean and free from hazards.

  Observation. The following was observed in the Preschool/School Age room: -a tall brown shelf holding books and other items does not appear stable -a TV sitting on a high shelf is not stable

  Corrective action. The Manager explained she will secure the shelf to the wall and secure the TV to the high shelf to ensure there is no tipping hazards.

• STAFFING

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

  Observation. A total of 10 staff files were reviewed and 4 staff were either missing a criminal record check(CRC), had an expired CRC, or had a CRC from another organization.

  Corrective action. The Manager explained she will ensure all staff have a valid CRC on file.

• HYGIENE AND COMMUNICABLE DISEASE

  A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.

  Observation. The bedding was touching one another on the nap mats

  Without limiting subsection (1), a licensee must ensure that (a) children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E.

  Observation. A child who has not yet started kindergarten was in attendance in the school age program; the attendance indicates this has happened more than once

  Observation. Immunization status was not obtained for one or more employees.

  Corrective action. Ensure to have this information on file for each staff

  Observation. There were cleaning supplies in the school age/preschool washroom that were accessible to children. This is a repeat contravention.

  Corrective action. Ensure any hazardous substance is inaccessible to children.

  A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager.

  Observation. The Manager's name was not posted.

  Corrective action. Ensure to post the name of the Manager in a prominent place in the facility.

• POLICIES AND PROCEDURES

  A licensee must keep current records of each of the following: (d) written policies and procedures that are intended to guide employees in the care and supervision of children.

  Observation. An active play and screen time policy was not available for review. This is a repeat contravention.

  Corrective action. Ensure to have these files on site available for review and shared with the parents.

• STAFFING

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

  Observation. One staff file was missing a second reference check.

  Corrective action. Ensure to have atleast 2 references on file for each staff member who is not the Manager.

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

  Observation. One staff had an expired criminal record check. This staff cannot be left alone with the children until a new clearance letter has been obtained.

  Corrective action. Ensure each staff has a valid criminal record check on file.

  A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): There were cleaning supplies under the sink in the back washroom that were accessible to children. The Manager did remove these during the inspection.

  Corrective action. Ensure all cleaning supplies or other substances are inaccessible to children.

  A licensee must supply equipment, furniture and supplies that are (a) of sturdy and safe construction, easy to clean and free from hazards.

  Observation. - There is a couch and chair in the toddler room that is ripped on the seat so cannot be sanitized effectively. - There was a toy in the outdoor play area that is plastic and the seat to sit on was cracked. The Manager removed this chair at the time of inspection.

  Corrective action. Ensure furniture and equipment is in good repair

  A licensee must keep current records of each of the following: (d) written policies and procedures that are intended to guide employees in the care and supervision of children.

  Observation. A screen time policy and active play policy was not available for review.

  Corrective action. Ensure these 2 policies are available for review on site and shared with parents.

  A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

  Observation. There are multiple days where some children are not indicated as absent and the attendance line is blank.

  Corrective action. Ensure each child has a departure and arrival time and marked absent when they are not at the facility.

  Observation. A sampling of staff files was reviewed and one file was missing references.

  Observation. There is a step on the play structure that is missing.

  Corrective action. Ensure the equipment is safe and free from hazards.

  Without limiting subsection (1), a licensee must ensure that (b) the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E. Observation (CORRECTED DURING INSPECTION): a) Infant/Toddler Ratio - Staff to Child Ratio is 1:4 (IT/ECE/ASST). At the time of Licensing's arrival, there were 9 children present with 2 staff. The child was moved to the group daycare program.

  Corrective action. Ensure to be in compliace with staff to child ratio at all times.
