  Observation. Indoors: - signs on wear on walls, doors and window sills Outdoors - outdoor equipment visibly dirty - garbage and debris on the ground throughout yard - fence along the back needs repair

  Corrective action. Ensure a healthy and safe environment is maintained.

  Observation. LO observed and discussed the following with the staff: - Naproom: floating shelf not installed properly (sagging) - Bi-fold closet on the corridor: no child safety lock and loose door knob - Change mat: visibly dirty and stored in the bathtub with a plunger

  Observation. Emergency Supplies onsite are insufficient to implement the emergency plan.

  A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure. Observation (CORRECTED DURING INSPECTION): No attendance record was available for today.

  Corrective action. Ensure that children's time of arrival, departure and absence is recorded accurately.

  Observation. 4 children's files were reviewed; 2 of them were missing their MSP #

  Observation. The following was observed: - paint is peeling in some areas of the facility - blinds are broken in the nap room - drywall mud is exposed - some electrical outlets were missing covers

  Observation. The following was observed: - nap time bedding is piled on top of one another - 1 wood fence board is undone from the top and leaning out - garbage in the backyard

  Observation. - The closet doors need to be secured so that children do not pinch their fingers in them - there are holes in 2 bedroom walls from the door knobs

  Corrective action. Ensure a safe environment at all time and that the facility is in good repair.

  Observation. -some toys were broken and should be removed - there was a milk bottle in a play pen that had been there since yesterday; discussion about children not drinking while laying down; supervision at all times while children are consuming food - there was a lawn mower in the backyard

  Corrective action. Ensure to develop with the parent and have in place a written care plan for any child who requires it. This care plan needs to be reviewed annually.
