    Observation. LO observed and discussed the following with the Manager: - Outdoor play space: backyard gate (facility's main entrance) does not close or locks completely.

    Corrective action. Ensure that door is fixed, so that it can be properly closed/locked while children are in care and using the outdoor play area.
