  Observation. The Facility practice at the end of the day is for the children to remain seated in chairs at the front door or in the high chair until they are picked up.

  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. The toilet paper was not mounted but was placed high up where the children cannot reach it.

  Corrective action. Ensure the toilet paper is hanging in an area where the children can reach it to promote independence and teach proper hygiene.

  Observation. The diaper change mat is torn and needs to be replaced

  Observation. A screen time policy and active play policy was not available for review.

  Corrective action. Ensure to have these policies in writing and provide them to parents.

  A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

  Observation. There are days where departure times are missing.

  Corrective action. Ensure to record daily attendance, including when a child is absent.

  A licensee must ensure that all indoor and outdoor play materials and equipment accessible to children are (b) safely constructed, free from hazards and in good repair.

  Observation. At the entry of the facility, there is some plywood and a plywood step, as they are both the same color it is difficult see the step, making it a tripping hazard. Discussion had with the Manager about adding a colored strip or something to make it more easy to see the step.

  Corrective action. Please ensure facility is free from hazards.
