  Observation. Licensing Officer observed medication for a child with a life threatening allergy did not have a completed Request for Administration of Medication form on file.

  The facility, the furniture, equipment, fixtures and supplies are of sturdy and safe construction.  They are easy to clean, and are clean and in good repair and free from hazards. [CCLR Sec. 13(2) and Sec. 15(1)(a)]

  Observation. In the children's washroom, Licensing Officer observed the following: -The paper towel dispenser is showing visible rust along the top surface and around the dispensing area where children access the towels. Additionally, one corner of the dispenser appears to be held together with tape. Discussed with manager that the paper towel dispenser does not appear to be easy to clean or in good repair. -The diaper change mat appears to be in poor repair. The underlay material is exposed, creating a surface that cannot be adequately cleaned and disinfected. This condition presents a risk of cross contamination during diapering routines. The manager replaced the diaper change mat with a new one during the inspection.

  Observation. In the outdoor play space, Licensing Officer observed a child sized sandbox however it did not appear to contain an adequate amount of sand for children to fully engage in play.

  Observation. While the issue of non-compliance has been addressed, during today's visit Licensing Officer observed an immunization status has not been obtained for 2 staff.

  Observation. Licensing observed a care plan was not developed in consultation with the parents of the child requiring extra support.

Licensing reviewed children's records: - Emergency card stated 1 child has an allergy however upon discussion, that child no longer has an allergy. - 1 child does not have medical practitioner contact information. - 1 child may have an allergy however there is no further information - 1 child does not have emergency contact information

  Prior to employing staff the manager has met with each person and has obtained the appropriate documentation. [CCLR Sec. 19(1)]

  Observation. Licensing reviewed all staff records: - 1 staff had a criminal record check that is not addressed to the facility. - 1 staff did not have character references. - 1 staff did not have documentation to support their certificate was verified using a system established and maintained by the director of the early childhood educator registry.

  Observation. In the main activity room and the nap room containing cots, LO observed that the walls appear to be soiled, stained and look worn. The wall with the decorative floral canvas has an area where the paint has peeled, exposing the drywall. In addition, there are areas of the walls where it is evident that the children have drawn on them with crayons or markers. It appears that the walls are no longer easy to clean and require some maintenance. In the children's nap room, LO observed a grey child sized arm chair appears to be soiled and stained with visible paint markings. The arm chair is no longer easy to clean and requires some attention.

  Observation. LO reviewed a sampling of the children's records and the following was observed: -date of enrolment is not kept for 1 child -telephone number for a medical practitioner is not kept for 2 children -a medical insurance plan number is not kept for 2 children
