    Observation. The facility has agreed to administer medication to a child; however, no instructions were provided in the child's registration form and a care plan and medication permission form have not been received or created.

    Corrective action. A staff confirmed that they will ensure to obtain a medication permission form along with a care plan providing detailed instructions.

    A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. Licensing observed an unsecured, tall, heavy shelf storing toys on open shelving. Some toys/supplies are stacked on top of the shelf. This could pose a hazard in the event of an earthquake.

    Corrective action. The Manager will secure the shelf and secure or remove any heavy items that are above children's height.

    A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. Licensing observed three suspended unsecured ropes attached to the fence in the outdoor play space, along with two unsecured, suspended electircal cords in the facility. All were potentially within the reach of children.

    Corrective action. A staff placed the electrical cords outside of children's reach during the inspection and confirmed that she will remove the outdoor ropes by the end of the day today.

Observation (CORRECTED DURING INSPECTION): A high chair was blocking the emergency exit to the front door. The high chair was moved during the inspection.

    Corrective action. Ensure that the emergency exit is not blocked.

    Observation. The door leading to the garage where tools and cars are stored was unlocked, however child safe door handle was on the door. There is concern that older children will be able to open the child safe door handle and access the garage area. A razor was located in the shower stall in the children's bathroom, accessible to children.

    Corrective action. Ensure that the children do not have any access to objects or substances that may be hazardous.
