Observation. An ECE Assistant currently working at the facility does not have their ECE certificate posted prominently. - Posting of ECE certificates provides transparency to parents that staff are qualified. Submit by June 10th, 2026, evidence of all employed educator or assistant certificates being posted prominently.

Observation. Related to an incident that occurred on May 26, 2026; The Group Child Care (School Age) program provides after school child pick up from three different locations. Before pick up time the staff group and review the attendance of children for the day. On the day of the incident children were not picked up from the one school location and were not acknowledged until they arrived at the facility by foot on their own accord unharmed. - Children unaccounted for and/or unsupervised are put at risk of safety, emotional distress, and exposure to environmental hazards. Submit by June 10, 2026, how you will ensure agreed upon child pick up arrangements will be maintained and children accounted for at all times. Neglect is defined in Schedule H of the Child Care Licensing Regulation as failure of a care provider to meet the needs of a child, including food, shelter, care or supervision.
- C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical
Observation. The posted emergency plan includes information that is not applicable to this physical location. It additionally identifies a relocation site of a community school which may not be accessible during non-instructional school days. The evacuation bag does not contain diapering supplies or food. - Emergency supplies are required to meet the basic needs of the children in care during an emergency. Submit by June 10th, 2026, evidence of the emergency plan being updated and supplies included to meet the needs of children in an emergency. The facility transportation safety plan was not followed by employees during a recent incident that occurred on May 26, 2026. This policy notes that attendance is to be taken for school pick-ups. - Attendance records provide documentation showing which children are to be pick up after school and will ensure staff are able to account for and supervise children appropriately. Submit by June 10th, 2026, evidence on how you will ensure the facility policies and procedures are implemented by employees regarding the care and supervision of children.

Observation. The fire drill for one of the care programs was not implemented for April 2026. It is recommended that fire drills also be implemented as a whole facility and using alternate exiting pathways. - Implementing monthly fire drills supports the effectiveness of the fire drill system. Submit by June 10, 2026, evidence of the monthly fire drill being implemented and how the care programs will ensure they are conducted monthly.

Observation. A child record identified a potential food allergy however there was no special instruction in writing from the parent on how to prevent, identify, and manage the potential medical issue. - Special instruction will support staff in supporting child individual needs safely and consistently. Submit by June 10, 2026, evidence of how child records are complete of special instruction. Related to an incident that occurred on May 26, 2026; During after school pick at one location the staff does not take an attendance sheet to sign the children into care upon meeting them. These children are currently signed into care upon returning to the facility which is not a true reflection of their time of arrival. Staff additionally do not have emergency cards that contain a child photo and other information to be used in the event of an emergency. - Accurate attendance records are vital for ensuring every child is accounted for during emergency evacuation drills and/or during daily transitions. - A child photo and other information can be used to verify identity in emergencies. Submit by June 10, 2026, evidence of how up to date attendance will be maintained and a photo and other information will be available at all times in the event of an emergency. A child care plan has not been reviewed since 2024. Care plans are additionally incomplete of having a record of any behavioral guidance provided and its effect. - Having up to date care plans support the plan effectiveness and provides consistency among staff. Submit by June 10, 2026, evidence of how all child care plans have been reviewed and behavioral guidance documented.

Observation. Nap rooms contained hanging cords from monitors/sound machines. - Loose cords pose a risk of being pulled on with a device falling onto them or a potential for strangulation. Submit by June 10, 2026, evidence of how all loose cords have been secured.

Observation. A picnic table on the 30 Month to School Age playground has a protruding screw. The table will need to be inspected frequently as it is beginning to rot and could present safety issues in the future. Issues like this should be found, noted and addressed on the outdoor inspection checks that are being done daily. Ensure staff who are completing these safety checks are properly trained in how to identify safety issues.

Observation. Several children were unsupervised in an area of the off-site outdoor play area where a disagreement occurred and resulted in a serious incident between them and one child was harmed.
- C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical
Observation. As reported to licensing in an incident report dated October 19, 2021, the facility staff did not follow facility policies and procedures on care and supervision resulting in a child becoming injured and an incident of aggressive or unusual behaviour. Children were unsupervised for a period of time where staff were not in place to intervene and guide children in problem solving and appropriate conflict resolution.

Observation. The current policies and procedures for playground safety are found to be ineffective as an unsafe object was found on the off-site outdoor play area. Staff stated there is an area of fencing that is in need of repair. If this is not adequate to ensure children are safe it must be repaired prior to use of this area. The unsafe object (a 7.5 foot heavy metal pole with exposed and protruding screws) found by children was used in an unsafe manner by children and harmed a child.
