Observation. C4.1E - Reg. 57 - A child record was observed today missing special instruction in writing from the parent regarding medical information that requires medication. Several child records include special instruction/medical forms however this information is not available when the staff are off site with the children. Ensure child records are complete and information is available at all times relating to special instruction and any potential medical needs.

Observation. C7.1L - Reg. 17 - The indoor space was observed with an electrical outlet not covered presenting a potential risk if a child inserted an object. This item was resolved during the inspection with electrical plugs installed.

- C7.2 - Is there adequate and appropriate indoor and outdoor space, materials, and equipment?Critical
Observation. C7.2E - Reg. 16 - An incident report was submitted to Licensing identifying a child obtained an injury in the lower outdoor play yard. The outdoor play space is utilized by both care programs at alternate times and all equipment is accessible to everyone. At the time of the incident a child was using a piece of equipment that was not age appropriate. Licensing was informed that a new practice is in place to not allow bikes/tricycles in the lower level play yard. Identify the system implemented to ensure children only use equipment that is appropriate for their age and development as well as how all staff and children will be aware of play space practices/policies. The Standard of Practice - Safe Play Spaces document has been attached as a reference.
- C10.2 - Are children supervised, and are employee to children ratios maintained?Critical
Observation. C10.2D - Reg. 39 - At the time of the incident there was 4 staff present with 17 children which exceeds the required ratio of 1:12. It has been acknowledged by staff that the children on the lower play area were not being supervised when the incident occurred. One staff had taken some children to the washroom and three staff were in the upper area of the play area. Licensing was advised that there are new systems in place to ensure the entire upper and lower play areas are continually supervised. Submit to Licensing the system/policy implement to ensure children are supervised at all times.

Observation. C4.1B - Reg. 55 - A reportable incident occurred on October 18, 2023 and it was not submitted to Licensing within the required 24 hour period. Identify the system implemented to ensure all staff are aware of reportable incidents as per Schedule H of the Regulation and the requirement to report to Licensing.
- C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical
Observation. C3.1C - Reg. 19 - Staff files were reviewed and found complete for the staff present the day of the incident. Upon arrival today there was a recently hired staff present without a complete file. Identify the system that will be implemented to ensure staff files are complete and on site prior to staff commencing employment.
