Observation. C1.1A - Reg. 10(1) - There have been changes to the facility operations and Licensing was not notified. - The hours of operation have changed and there is additionally an evening option for care that is occasionally offered. Submit a notification to Licensing Direct and include the care program changes, employee plan, budget, and updated evacuation plan to include evening relocation sites. - The facility has implemented an optional snack program which requires an updated food and drink policy to be submitted.
- C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical
Observation. C2.2C - Reg. 22 - The facility has an emergency plan however, it does not set out procedures on how to recover from an emergency. An emergency planning resource has been provided with this report.
- C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical
Observation. C7.1A - Reg. 13(1) - The tables that children use for consuming food are currently not being sanitized prior to use. The staff are wiping the table surface using soap and water and were informed by the furniture representative that chemicals will damage the surface. This surface requires a food safe sanitizer be used to prevent the spread of diseases. A resource has been provided with this report as well as a link to the furniture manufacturer website that identifies appropriate sanitizers to use.

Observation. A sampling of child records were reviewed today and of the ones checked one was missing a medical insurance plan number.
- C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical
Observation. The Birch room has a grey foam mattress with a torn cover exposing the foam and a memory foam mattress without a cover. This presents a choking hazard if a child picks the accessible foam and swallows it. The Willow room which is infant/toddlers had some small wooden parts children were playing with which are small enough to be a choke hazard. These were removed during the inspection. Ensure children do not have access to small items that are a possible choke hazards. The outdoor large play area is moving forward with renovations in the very near future. A file for this reno/notification is open. As discussed, the facility will provide licensing with time frames for the work, health and safety plan and the plan for alternative outdoor play time during the construction.

Observation. Outstanding contraventions from the previous inspection report are as follows: - All outdoor play area issues on the main play area have not yet been addressed. The Hydrangea shrubs were cut down, the tires were addressed and the empty sand box was removed. All other concerns remain. This indicates that the room is not being sanitized on a regular basis. Please provide a cleaning schedule and advise how it is implemented.

- C7.2 - Is there adequate and appropriate indoor and outdoor space, materials, and equipment?Critical
Observation. The indoor rooms have the following concerns: Maple Room: - Numerous rotten boards are observed on the deck walking surface which staff state children use with supervision to access a hill for sliding in the snow during winter. If this is to be used by children it must be made safe. Previously it was stated that children never have access to this area. - The base of the blue Ikea chair is loose. This is noted on a previous inspection report and continues to be an issue. - The moulding on a table is coming off. Bacteria could gather in this area and cannot be sanitized adequately. - The chairs appear soiled. The manager states they are sanitized daily. - A cupboard accessible to children has a cord stored in it. - One of the area rugs is curled up on both ends and presents a trip hazard. Oak room: - Two glass mirrors are accessible to children and are not safely hung to prevent them from being pulled down and possibly shattered. Birch room: - Please ensure all plants (indoors and out) are child safe. Atrium: - Ensure climber has a safe surface underneath as it is on top of a cement floor. Safe use of this climber is the responsibility of staff and manager.

Observation. Provide your outdoor activity plan that shows how you are meeting the minimum requirements of Schedule G and Section 44.
- C8.1 - Are children offered a variety of activities and instructed on the safe use of the play space?Non-critical
Observation. Some of the mud balls thrown at the building are up as high as 15-20 feet indicating that staff may be participating/encouraging this activity. In discussing the mud throwing on the wall of the neighbors building the manager did not see any issue with this.
- C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical
Observation. The infant/toddler outdoor play area does not meet minimum health and safety standards and must not be used until it is made safe. The following are safety issues noted: - The outdoor play area used for infant/toddler children (mostly infant children as stated by staff) has Hydrangea shrubs that are easily accessible to children. Hydrangeas are poisonous. The facility was not aware they are toxic. - There are several tires being used in which some are gathering standing water. - The tires that were observed exposed to the sun were extremely hot and could burn the children. - The placement of tires around two large boulders presents fall and safety hazards for the children. - The surface in this play area is cement. The staff state the area is used for the infant children. Crawling and falling on this surface could cause injury and does not provide an area that promotes safe physical movement. One mat was outside (standing up). - Outdoor equipment/activities appear to be inadequate and do not meet the requirements of Schedule G. The following is observed on the main outdoor play area: - Equipment and a variety of activities appear to be lacking. It is observed today that children have access to water/mud play, three riding toys, a sandbox with no sand, tires, tables to sit at, and a noise activity on the fence (pots and pans etc.). No other equipment appears to be available. - The tires are gathering standing water. - The play area has numerous potholes throughout the play space, the riding track is not maintained (it is covered with dirt and mud as are the riding toys). - With the numerous holes created in the play area, running activities/games would be difficult and hazardous as children would need to navigate the pot holes. - Though mud play is not to be discouraged, having a designated area for this activity would allow for other activities to occur. - The adjacent building has one of its walls that is on the boundary of the play area. This wall is covered in mud which has been thrown on the wall and hand painted with mud. This is not the daycare's property and this activity does not teach children to respect the personal property of others as is required in the Child Care Licensing Regulation (Schedule G 5 (c). - Several Hydrangea shrubs are located on this play area but appear to be out of reach of children. The following is noted in the inside area of the building: - The fire exit leading to the infant toddler outdoor play area is hampered by storage practices of a wagon partially in the way of the fire exit and other items. This issue has been noted on a previous inspection where the same fire exit was partially blocked.

Observation. The current log of play space inspections is said to be completed however it appears somewhat ineffective as some equipment was found to be in disrepair today. Update this play space inspection to ensure it is effective in identifying and removing hazards. The manager states her records are current and complete and that no child starts without records being complete. The facility has a couple of children who have recently been identified as a "bolters". The manager is taking a pro-active approach and states she has plans to secure the doors to the care programs so children cannot get out easily and plans to have a care plan in place for one of the children.

Observation. The blue IKEA chairs in the 30 month to school age room are missing screws and are quite wobbly . This was an issue in the last inspection. A crib in the infant/toddler nap room has been chewed along the entire rail. Chunks of wood have been bitten out leaving a rail that has many areas with splinters. It is also a choke hazard if the wood chunks are chewed out and swallowed. The bathroom has step stools stacked up on the back of the toilet which are not secure and are tippy presenting a hazard to the children using the toilet. In the last inspection report inappropriate storage in the bathroom was also noted. A wooden spool used for play on the outdoor play space has splinters which could present a hazard to the children. As two of these concerns continue to be an issue since the last inspection an effective system must be put in place that assures equipment is free from hazards and in good repair. This system must be used daily to ensure safety. Ensure staff are trained/educated in identifying hazards and taking steps to ensure safety.

Observation. The outdoor play yard has had a large storage shed added to the space which takes away from the available outdoor space. Provide updated site plans with dimensions of the play area and the shed so space requirements can be confirmed.
- C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical
Observation. The diaper change mat has cloth on it which could easily become soiled and unsanitary as it cannot be sanitized after each use. The safe walking path to the outdoor play area is not longer in place. The children must walk through a very busy parking lot and the parking stall that was specifically designated to provide adequate space and caution cones (when first licensed) is no longer available presenting an unsafe environment. Provide licensing with a written safety plan in this regard prior to reopening. The nap room in the Birch Room has safety concerns. It is observed that the window covering which children can easily reach is a black plastic garbage bag; the second crib in the nap room is used for storage and contains bags that are accessible to the child napping/resting in the other crib. These could also present a possible suffocation hazard. A full hot cup of coffee was observed on the top of a shelf within reach of the children (Birch Room - Infant/toddler). The light blue bucket chair in the Maple Room is wobbly and in need of repair. The coat hooks in the Oak Room which store the backpacks for first aid and records is coming out of the wall and appears ready to fall off the wall which could cause injury. A staff purse was in easy reach of the children. Ensure staff have a safe place to store belongings that may contain hazards. A wicker tent on the outdoor play yard is in disrepair and has sharp wicker pieces that are broken and splintered and could be hazardous to the children.

Observation. Children are not hand washing after diaper changes. This does not meet the requirements to practice the rules of health and hygiene.

Observation. At this time it appears the outdoor play space is inaccessible. A written plan is required to address outdoor play plans and safety during this construction period.

Observation. One of the two fire exits was found partially blocked today with a trolly and small countertop stove blocking one of the doors. Ensure all fire exits are free of equipment and any other items at all times. It is recommended a system is put in place that all staff will follow to assure future compliance. The manager states the items have been removed.
