Observation. The complaint alleges that the licensee is not complying with the employee-to-child ratio requirements in the Group Child Care (30 Months to School Age) and Group Child Care (Under 36 Months) programs. One of the Group Child Care (Under 36 Months) rooms is staffed by one Infant Toddler Educator (ITE) and two Early Child Educator Assistants (ECEA) while caring for nine children. The staff roster provided on July 10, 2026 indicated that an Early Childhood Educator (ECE) was scheduled to work in Group Child Care (Under 36 Months) Room 1 on July 17. However, the licensee later decided not to proceed with the ECE’s employment. The licensee contact explained that the scheduled ECE shift was being replaced by an ECE-Assistant on that date. An absent employee refers to an employee who is temporarily absent because of illness, emergency, vacation or other temporary leave. The individual originally identified as the ECE cannot be considered as “absent employee” as they were never employed by the facility. Their absence from the schedule does not meet the definition of an absent employee. A review of records identifies additional occasions when the Group Child Care (Under 36 Months) are being staffed with one ITE and two ECE-Assistants. Staff interviews confirms that this staffing arrangement occurs in the rooms. Interviews with staff indicate that some employees are not familiar with the employee-to-child ratio requirements applicable for the program room. Children's attendance records for June show that three children under 36 months of age were attending the Group Child Care (30 Months to School Age) program. Based on the information received, the allegation is substantiated. Submit by August 3, 2026, a written response how you have corrected the issue and how you will ensure ongoing compliance to the regulation.

Observation. As part of the complaint investigation, attendance records are reviewed which identified that some children were not signed out, and in some instances, children’s attendance were not recorded, including whether they were absent. Missing departure times poses a risk to children's safety during emergencies. This may also leave a facility vulnerable in the event of missing children or custody disputes, as children are unaccounted for in cases of unauthorized pick-ups. Submit by August 3, 2026, a written response how you have corrected the issue and how you will ensure ongoing compliance to the regulation.

Observation. A reportable incident occurred on May 6, 2026 involving a missing/wandering child. Children were not adequately supervised during the incident in which a child wandered away from the group and was unsupervised. The child was located by staff outside the facility front door unharmed. This incident occurred during a transition to on-site outdoor play that additionally involved a staff substitution. The facility doors are coded to prevent unknown incoming access however the exiting access is not limited for emergency purposes. The lack of adequate supervision placed the child at risk of harm and reduced staff's ability to ensure the child's safety. The facility Manager updated policies and staff procedures to enhance the current systems already in place which include: • Headcounts increased to a minimum of 3 checkpoints during transitions. • Headcounts are to be verbally confirmed with another staff and documented. • Staff handovers are to include a total child count. • A care plan has been created for a specific child with strategies related to elopement risk. • Staff substitutions during transitions are to be avoided wherever possible. • A child safety hook has been placed on the exiting door along with a door chime. • Staff have reviewed and signed the updated policies and procedures. This contravention is resolved and corrective actions appear appropriate.
