Observation. Not all Early Childhood Educators certificates are posted, as required. Display all certificates in a prominent location by May 25, 2026.

Observation. Children were not adequately supervised during the incident in which a child wandered away and was missing for approximately 30 to 45 minutes. The daycare is located within a residential care facility, and the child was later found in a room down the hall with residential activity employees. During the period between the child arriving at the location and daycare employees noticing the child was missing, the two Multi-Age Child Care groups had been combining. Combining these groups is not permitted during indoor care. The lack of adequate supervision placed the child at risk of harm and reduced staff's ability to ensure the child's safety. Combining the two Multi-Age Child Care groups during indoor care may have impacted the staff's ability to effectively supervise and account for all the children. Submit by May 25, 2026 a detailed written response how you have corrected the identified contraventions relating to lack of supervision and combining of care programs.

Observation. A reportable incident occurred on May 5, 2026 involving a missing/wandering child. The parent was not immediately notified nor was licensing notified within 24 hours as required. Delayed reporting may impact the ability to assess and respond to potential risks to children in care. Ensure reporting occurs within the required time periods.

Observation. A facility addressed criminal record check is not on file for one employee, limiting verification that the individual has been appropriately screened to work in the facility. Prior to working in the facility, ensure required records, including facility addressed criminal record checks, are obtained.

Observation. Employee records were reviewed. There was no evidence that one Assistant’s certificate had been validated. Educator and Assistant certificates must be validated with the BC Early Childhood Educator Registry. Submit by November 28, 2025, confirmation that the certificate has been validated. An Assistant has been working for an absent Educator for more than 30 consecutive days. An Educator is required to be replaced by an Educator if absent for more than 30 consecutive days. Submit by November 28, 2025, confirmation that an Educator is providing care to children.

Observation. Digital images used to identify a child in an emergency were reviewed. One child’s digital image was missing. Ensure there is a current digital image for each child. Submit by November 28, 2025, confirmation that the missing image has been collected.

Observation. Employee records were reviewed. Some required information was missing, including one employee's immunization status, and one employee's reference checks. Ensure all employee records are obtained prior to an employee starting. Submit by November 28, 2025, confirmation that the missing records have been added to each employee's file.

Observation. There is damaged metal flashing accessible to children, and exposed screws in some rotting wooden steps in the outdoor play space. These pose a risk to the safety of children accessing the play space. Submit by December 05, 2025, confirmation that the facility's outdoor play space is in good repair.

Observation. C10.2A & C10.2 C The two Multi Age Child Care programs are not being staffed by qualified Early Childhood Educators at all times on a regular basis. The staff schedule for October indicated one of the ECE leaving regularly at 2:30 pm leaving that program with an ECEA. Today the two Multi Age Programs were operating with only one ECE for one program and an ECEA for the other. There were additional ECEA's present. The Child Care Licensing Regulation, Schedule E requires that an ECE is required at all times for each Multi Age Child Care program with 8 children. C10.2 B The two Multi Age Child Care programs were combined outside which is allowed with in the legislation. However, not all the children from both programs went outside leaving some children from each group inside; these children were combined inside together.

Observation. C10.2D A staff was observed with a few children in the washroom for routine care which left the majority of the group in the classroom unattended. A child under the age of 3 was in the classroom engaged in playing with magnets on the fridge while standing on the chair unsupervised. As per Child Care Licensing Regulation Section 39 (1) ensure children are supervised at all times.

Observation. C7.1A The Licensing Officers observed a microwave was on top of the fridge which could potentially fall on a child. As per Child Care Licensing Regulation Section 13 (1) ensure the microwave does not pose a risk of falling on the children. C7.1I During a conversation with the Licensee, she stated that one of the children in the program regularly sleeps in a stroller. The Licensing Officers observed this to occur. As per Child Care Licensing Regulation Section 15 (2)(c) ensure sleeping arrangements are developmentally-appropriate for all children in care. The Licensee stated that she will work with the parents to come up with a sleeping plan.

Please provide to Licensing by September 5, 2022 a detailed written plan which addresses (1) How you have corrected the contravention and (2) The systems you have implemented for ongoing monitoring thus ensuring sustained compliance at all times.

Observation. It was alleged that there were children in care without complete registration records on site and that the daily electronic attendance system was not functional. During the inspections the Licensing Officer confirmed children were in attendance with no registration records. Additionally the electronic attendance system had experienced technical problems. Substantiated - Section 57 Records for each child • C4.1E & C4.1H; Child Care Licensing Regulation 57 (1) (2) Ensure complete children records are available at all times while children are in attendance and a functional system is in place to record daily attendance of children. _______________________________________________________________________ It was alleged that an incident occurred involving aggressive or unusual behaviour and emergency restraint that was not reported to Licensing. Licensing confirms no report was provided for the reportable incident. Substantiated - Section 55 Notification and illness or injury • C4.1B; Child Care Licensing Regulation 55(2)(a) Provide to Licensing an Incident Report for the occurrence of aggressive or unusual behaviour and emergency restraint. ______________________________________________________________________ During the course of the two on site inspections (Aug 8 & 9, 2022) the Licensing Officer found there was an incomplete registration form for a child who was no longer in care. The manager stated a complete form was done however, it could not be located. Records must be kept for 2 years Contravention found during inspection - Section 60 How long record must be kept • C4.1S; Child Care Licensing Regulation 60(5) Ensure there is a system in place to ensure records are retained, secure and accessible.

Observation. It was alleged that a child's short term medication doze was missed being administered at the scheduled time. Additionally, employees were unaware of the requirement to administer a child's emergency medication (epi pen) and there were no instructions regarding its use. The Licensing Officer reviewed the medication record for the short term use and notes one doze was missed and when brought to the employees attention was given late. The parent was informed of the late administration time. No record or instructions regarding the epi pen was documented and the manager admitted the process had been missed. Substantiated - Section 53 Medication • C9.1A; Child Care Licensing Regulation 53(1)(a) Ensure there is a system in place for administration of medication including instruction and ensuring employees are aware of the medical needs of the child especially in the case of emergency medication like an epi pen.
