Observation. Reviewed the submitted reportable incident stating a staff member in the under 36 month program hit a child across the face with her hand. The employee was immediately suspended from the facility and parents were notified. An investigation was conducted and found harmful actions not permitted did occur. The employee has been since terminated from the facility. Submit by Oct 17, 2025, a compliance plan how you have address the requirement of ensuring a child must not be subjected to shoving, hitting or shaking by an employee.

Observation. The Licensing Officer found two incomplete children's files missing personal health number and emergency contacts. The Child Care Licensing Regulation requires the Licensee to keep current records for each child so that in the event of an emergency the Licensee has current personal information regarding the child. Submit by October 8, 2025, how you have addressed the requirement to keep current personal information records for each child.

Observation. Licensing Officer observed an ECE. Assistant providing care to the Multi-age program. This was past the 30 days from the date the previous employee ceased employment.

Observation. Licensing Officer observed fire drills were not recorded for last month.

Observation. Licensing Officer was unable to review staffing files as the filing cabinet was locked and inaccessible. Staff in Multi-age room had an expired first aid certificate posted.

Observation. Licensing Officer observed the large climbing structure in the 30 months to school age program had missing screws/bolts. This may affect the overall structure of the climber which may be a hazard. Licensing Officer observed tarps over the outdoor play areas which are only 4 feet off the ground. This may increase the risk of heat stroke as the tarp can trap the heat instead of providing cool shade.

Observation. Licensing Officer found the minor log entries for injuries on the himama app did not include all the names of the children involved. Licensing Officer was unable to review child registration forms as the filing cabinet was locked and inaccessible. Reviewed child emergency cards and observed no information which can be used to readily identify the child in an emergency, (i.e. hair colour, eye colour, height and weight),

Photos of the children were available on the himama app, there was discussion on how staff would access the files while being off-site. Reviewed incident reports, minor log, daily attendance and care plans.
