It is a requirement to document any minor incidents that occur at the facility. Submit a compliance plan to address this issue on or before October 8, 2025.

    Observation. During the inspection, a playpen was observed in a bedroom upstairs, which the licensee stated was used by one child during nap time. A child sleeping on separate level of the home does not allow for adequate supervision by the licensee at all times. Provide a plan to ensure that all children are supervised continuously by the licensee during nap time.

    Observation. The log of minor incidents has not been updated since November 2020. Keeping a current log of any minor accidents illness and unexpected events is required. Enhance or make changes to the current system to ensure that any incidents are being recorded regularly. Upon review of several child records, it was observed that each file either did not have a photo of the child, or the photo was out of date. Having a clear and current photo of each child is important in case of emergency. Ensure that a photo or digital image of each child is obtained and a system is created to ensure photos are current.
