Updated photos and medical practitioner information are required in case of emergency. Submit a compliance plan to address this issue on or before May 4, 2026.

  Observation. The licensee did not have a system in place for logging the daily outdoor play space inspections. This was corrected during inspection.

  Observation. The licensee does not keep a record of yearly emergency plan drill.

  Observation. Children's files do not include a record of each child's immunization status.

  Observation. During today's inspection, Licensing reviewed with the Licensee the facility procedures for inspecting the outdoor play space. Although the Licensee is doing daily inspections, not all actions are being recorded. The DOLSOP for Safe Play Space was reviewed with the Licensee. This was corrected during the inspection as the Licensee began a record for the inspection and any identified concerns with safety measures taken and the system to be used was discussed.

Facility child record forms do not include written authorization for releasing a child, any indication of whether there is a person whom may not access the child, or adequate information that may be used to readily identify the child in an emergency. Licensing provided to the Licensee, via email, a resource tool for ensuring child records include all required information.

  Observation. Licensing reviewed required policies for the facility. The emergency evacuation plan for the facility requires more detail. The policy for the safe release of children is missing required information. Licensing provided resource material for the Licensee, via email, in the review and development of the policies and procedures.
