Observation. One staff file did not contain a criminal record check. To ensure safety of persons in care, all staff must have a valid criminal record check in order to work with children.. Submit a plan regarding how this issue has been addressed on or before August 21, 2025.

Observation. It was alleged that a staff member spoke harshly to a child while guiding a child's behaviour on two different occasions. Follow up to this allegation included interviews with the facility manager and staff, review of facility policies, children's care plans and the minor incident log. As a result of follow up, it has been determined that this allegation is unable to be substantiated. It was alleged that a staff member left a child in a corner by themselves for an extended period of time. Follow up to this allegation included interviews with the facility manager and staff, review of facility policies, children's care plans and the minor incident log. As a result of follow up, it has been determined that this allegation is unsubstantiated. It was alleged that a staff member restrained a child by picking them up and moving them to the corner as a form of behaviour management. Follow up to this allegation included interviews with the facility manager and staff, review of facility policies, children's care plans and the minor incident log. As a result of information gathered during staff interviews, it has been determined that this allegation is substantiated, as this method of behavior guidance is not outlined in the child's care plan. Submit a written compliance plan on or before July 18, 2025 regarding how this issue will be addressed. It was alleged that a different staff member restrained a child by picking them up and returning them back to the corner. Follow up to this allegation included interviews with the facility manager and staff, review of facility policies, children's care plans and the minor incident log. As a result of information gathered during staff interviews, the allegation regarding this specific incident was unsubstantiated. Separate to the allegations made in the complaint, as a result of staff interviews it was determined that a staff member has used restraint methods as a form of behaviour guidance that are not outlined in a child's care plan. Therefore a contravention to 52(1)(b) has been identified. Submit a written compliance plan on or before July 18, 2025 regarding how this issue will be addressed. It was alleged that a staff member carried a crying child outside, and put the child in a separate fenced play area alone and locked the gate. Follow up to this allegation included interviews with the facility manager and staff, review of facility policies, children's care plans and the minor incident log. It was determined through staff interviews that the child was not left alone or confined. It was determined through staff interviews the staff did carry a child unwillingly, as a form of behaviour guidance that is not outlined in the child's care plan. This allegation is substantiated regarding physical restraint by an employee that is not outlined in a care plan. Submit a written compliance plan on or before July 18, 2025 regarding how this issue will be addressed.

Observation. Separate to the original complaint, during follow up it was determined that staff at the facility are not keeping a record of compliance with care plans, including any behavioural guidance provided to a child and its effect. Submit a written compliance plan on or before July 18, 2025 regarding how this issue will be addressed.

Observation. Some monthly fire drills are not recorded. System for recording drills requires enhancement to ensure accurate and consistent record keeping for each classroom.

Observation. Some child records do not have an emergency contact listed.

Observation. Licensing has not received results for radon testing.

Observation. Children's records do not identify the sex of each child. One child's record was missing a care card number.

Observation. A shelf located in the 30m to School Age Program (Sage) has a shelf that is not secured and therefore posing a risk to safety.
