  Observation. A review of children's records found that one record was missing a physician's phone number and two were missing a space for any person not permitted access to a child. Submit by the follow up date noted the system developed to ensure all of the required information is in a child's record. The Licensee confirmed that play space inspections are occurring, however, they are not being logged. Submit by the follow up date noted the system developed to ensure that play space inspections are being logged.

• C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

  Observation. There was no evidence that the emergency plan had been practiced in the last year. Practicing the yearly emergency plan will help to ensure the safety of children in the event of a true emergency. Submit by the follow up date noted your plan for ensuring a yearly practice.

  Observation. The emergency plan was not practiced in the last year.

• C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

  Observation. Each child has their own bedding, however, all the bedding and pillows are stored touching each other allowing for the possibility of the bedding to be unclean and unsanitary.
