Observation. All children's records are missing other information, such as physical characteristics, which can be used to readily identify a child in an emergency. This is a reoccurring contravention originally found out of compliance on February 7, 2025. Submit by February 13, 2026, evidence that the information has been added to the records.

Observation. A review of the daily attendance records showed that on some days the arrival and departure times were not recorded. This information is required in order to reflect the numbers of children present at any given time in case of emergency, or parent inquiry. A system for ensuring children's times of arrival and departure are recorded is required. Submit to licensing, by the follow up date noted, the system developed to ensure the daily attendance will accurately reflect children's arrival and departure times. Facilities must keep a current photo or digital image of each child and other information that can be used to readily identify the child in an emergency.
