    Observation. The following was observed inside the facility: - Plastic bag used to cover the diaper change pad (A staff removed the plastic bag) - Little Tikes slide without sufficient clearance around the equipment - The door knob at the entrance of the facility is broken - Dishes being used by children are not cleaned and sanitized using the 3 sink method The following was observed in the outdoor play space: - Dog feces in the designated playground - Grass and plants are overgrown and not maintained - Broken railings and panel accessible to the children - Garden hose accessible to the children - Approximately 10ft shelf with large items not secured

    Corrective action. Submit a corrective action plan.

    A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. Licensing observed the following: - Cabinet under the sink in the bathroom contained cleaning products. The cabinet was not secured and accessible to the children. CORRECTED DURING THE INSPECTION. - Diaper cream was left on the counter in the kitchen area, this was accessible to the children. CORRECTED DURING THE INSPECTION. - Clean- up bleach spray and diaper cream was stored in the mirror cabinet in the bathroom which is accessible to the children. - There is an over growth of grass growing in the outdoor play area. The Licensee contact reported this will be fixed later this evening. - Door to the outdoor play area came off the hinge. The Licensee contact reported this will be fixed later this evening.

    Corrective action. Ensure a healthy and safe environment is provided at all times.

    Observation. During review of the outdoor play equipment, it was found that the resilient surfacing around the climbing structure has eroded and is in need of refilling. The landscape fabric is starting to show underneath and poses a tripping hazard to the children.

    Corrective action. Ensure that there is sufficient resilient surfacing around the climbing structure. Ensure the landscape fabric is repaired or replaced.
