Observation. Two children's records are incomplete: 1. One is missing a medical practitioner's telephone number. 2. One is missing the medical insurance plan number, start date, and medical practitioner's name and telephone number. Complete and current information must be maintained for each child to ensure their health and safety. Submit by April 6, 2026, evidence that the children's records are complete with current information.

Observation. There is no evidence that employees practiced the emergency plan in the last year. Implementing the emergency plan at least once each year will ensure employees are prepared in the event of an emergency. Submit by April 6, 2026, evidence all employees have conducted the required yearly emergency plan.

Observation. One employee's criminal record check is expired. A person must not be employed until the manager has obtained the required documents confirming the person is suitable to work with children. Submit by April 6, 2026, evidence that a valid criminal record check has been obtained.

Observation. Upon review of staff files, the Licensing Officer observed one staff one staff did not have their certificate verified by the Licensee using a system established and maintained by the director of the early childhood educator registry. The Manager is required to submit, by the follow up date, a plan to ensure staff files are complete.

Observation. Upon review of staff files, the Licensing Officer observed some files to be incomplete missing a copy of work history, references and vaccination status. Prior to a person being employed in a facility it is the responsibility of the Licensee to obtain all the required records set out for staff in the Child Care Licensing Regulation. Submit the system developed to ensure staff files are complete.

Observation. A review of the facilities records indicated the employees had not practiced implementing the fire drill system in February. In the event of a real fire employees will know what is expected of them when practices have occurred as required by the Child Care Licensing Regulation. This is a reoccurring contravention originally found out of compliance on February 22, 2023, and February 7, 2024. A system needs to be developed to ensure monthly fire drills are occurring.

Observation. During the inspection, Licensing reviewed a sample of the facility records for children. Some were missing the name and phone number for a medical practitioner. The Licensee must ensure any current registration forms are complete with all required information. Submit to Licensing by the follow up date the system to ensure children's records are complete. Three children's records were missing a photo or digital image of the child. It is a requirement to have a photo or digital image of the child to readily identify the child in an emergency. Submit by the follow up date the system to ensure all children's records will have a photo or digital image.

Observation. One staff file was missing evidence that the person has complied with the Province's immunization and tuberculosis control programs.

Observation. The system to ensure children's records meet all legislative requirements appears to be ineffective. Licensing found several to be incomplete. Missing items included: - immunization status and medical insurance plan numbers - a space for persons not allowed access to the child - medical practitioner's name and phone number - enrollment date not filled in This is a reoccurring contravention originally found out of compliance on July 21, 2021, August 25, 2022, and February 22, 2023 Daily attendance records were reviewed and revealed several departure times missing. Several children's records were missing other information that can be used to readily identify a child in an emergency. Attendance forms are not kept are the facility and could not be reviewed by Licensing.

Observation. A system for ensuring the fridge temperature is maintained to ensure food is kept at a healthy temperature needs to be developed. A system for ensuring thorough daily, monthly and annual inspections of the outdoor play space needs to be created and implemented. This is a reoccurring contravention originally found out of compliance on February 22, 2023. The depth of the protective surfacing under some play equipment was worn away and left the ground hard.

Observation. A cupboard containing products for cleaning and sanitizing was open and accessible to children. Several mats are worn and cannot be cleaned and sanitized appropriately. There were no records of daily, monthly and annual inspections of the play space available for Licensing to review.

Observation. Child registration records were reviewed and many were found to be incomplete. On some registration forms there is no space to record who did not have access to the child. Medication was administered to a child, but no record completed. Not all staff had access to the care plans for children in their care. The care plan for at least one child was not up to date and did not have consultation with the child's parents.

- C10.2 - Are children supervised, and are employee to children ratios maintained?Critical
Observation. It was alleged that an employee would leave the facility out of ratio for short periods for frequent smoke breaks. Licensing was unable to substantiate the facility was out of ratio on all these "breaks". However it was confirmed by the manager, and all staff interviewed that the facility combines the Group Child Care (30 Month to School Age program and the Group Child Care (School Age) program Friday afternoons, and did combine the two groups daily the last two weeks of June 2021 for longer periods that the first and last half hour of the program. It was alleged an employee spends large amounts of time during the day engaging in cell phone use while supervising children. This was confirmed by all staff and the employee.

Observation. It was discovered during the investigation the Screen time policy does not specify the amount of time per day that children will be engaging in screen time. It was confirmed through staff interviews children have exceeded 30 minutes per day of screen time on multiple days during the last two weeks of June 2021.

Observation. During the course of the investigation through information gathered during interviews the following concerns regarding an employees character and skills were brought forward: - Lack of flexibility when caring for children. - Lack of effective interpersonal communication skills. - Lack of professional leadership skills for the role of manager. - Inconsistent temperament regulation. - During the interview the employee stated they are aware of the regulations pertaining to Section 34 - Group sizes and employee to children ratios, and the Director of Licensing Standard of Practice - Screen Time, however it was confirmed the employee chose to disregard the regulations on multiple occasions. - Through interviews with staff and the managers own admission the manager lacks managerial training and skills.

Observation. The submitted log of minor incidents does not contain any documentation of minor incidents. When interviewed the manager and staff stated knowledge of the minor incident log book but that it is not used and parents are verbally notified of minor incidents. The submitted care plan is incomplete. When interviewed the manager stated that she wrote the care plan herself with minimal consultation with the parent. The care plan is missing the diagnoses as made by health care professionals and the courses of action recommended by health care professionals to address the needs of the child requiring the extra support

Observation. It was alleged that "gummy bears' candy are given to children as toilet training rewards. The manager and staff interviewed confirmed that this has occurred at the facility. Allegation to Section 52 - Harmful actions not permitted - Unable to substantiate It was alleged that a child's wrists were held tightly by an employee for a period of time as a means of discipline for throwing rocks. Licensing was unable to determine if the wrists of the child were held. It was alleged a employee would leave a child for extended periods of time in a soiled diaper for "being a jerk". Licensing was unable to determine if this occurred based on the evidence received. It was alleged that an employee often uses a harsh, belittling, disrespectful tone toward children and other staff. While there was evidence that a staff used a loud voice with children Licensing was unable to determine if the severity of this behavior met the definition in Section 52 of the CCLR. Section 51 - Behaviour guidance - Substantiated It was alleged an employee would place a child on the toilet even though the child was clearly distressed at being placed on the toilet. The employee admitted to being a "follow through" type of care giver and did not exercise flexibility regarding this issue at times. It was alleged that an employee would provoke a child's behavior by singing a song that the child clearly did not enjoy and that would cause the child to become very agitated. During the course of the investigation it was discovered the child would like this song at times, but it could also upset the child at times. The employee did not exercise mature judgement regarding the use of this particular song. It was alleged that a child's sleep mat would be removed from a child at nap time if the child was "misbehaving". This was confirmed by facility staff as a practice that would occur inconsistently depending on that employees mood.
