Observation. Employee files are incomplete. One file is missing character references and evidence the person has complied with the Province's immunization program. This is a reoccurring contravention originally found out of compliance on November 14, 2024. A person must not be employed until the manager has obtained the required records. A response is required by December 4, 2025 to confirm employee files are complete as required.

Observation. Information was observed on file from one child's parent in regards to physical restraint and behaviour guidance. The child has a care plan in place that does not include all of the relevant information for the child's care. It is the responsibility of the Licensee to ensure that care plans are maintained for each child requiring extra support for the health and safety of the child. A response is required by December 4, 2025 to outline how care plans will be maintained. Care plans on file for children do not contain evidence that the care plans have been reviewed in the past year. Evidence includes signatures from both the manager and parent along with the date it was reviewed. Care plans are required to be reviewed at minimum once per year to ensure the health and safety of children in care. A response is required by December 4, 2025 to confirm all care plans have been reviewed as required. The care plans on file do not contain a record of compliance with the plan requirements. The Licensee must record compliance with the care plan of a child to ensure what was agreed upon with the parent is being implemented at the facility, for the health and safety of the child identified in the plan. A response is required by December 4, 2025 that outlines how compliance with care plan requirements are being recorded.

Observation. It was observed on two separate occasions during inspection an employee using physical restraint as a form of behaviour guidance for one child. The child does have a care plan in place that does not outline the use of restraint for the child. A child must not be subjected to confinement or physical restraint by an employee except as authorized in a child's care plan for the health and safety of the child. A response is required by December 2, 2025 to outline how the risk has been mitigated.
