Observation. Children's records are incomplete: all files are missing the sex of each child, one file is missing the name and telephone number of a medical practitioner. It is required that the Licensee has all required information for each child on file in the event of an emergency. A response is required by October 14, 2025 to confirm all children's records are complete. A care plan was observed to be in place for a child with extra medical needs. The plan had not been reviewed in the past year. Care plans are required to be reviewed at least once per year to ensure the information in the plan is relevant for the child's health and safety. A response is required by October 14, 2025 to confirm the plan has been reviewed with the parent as required. Employee files were unable to be reviewed as they are not kept at the facility. The files are kept in another area of the building and were inaccessible during inspection. All facility files need to be kept accessible in single location at the facility for Licensing review. A response is required by October 14, 2025 that outlines how all employee files for the facility will be kept in a single location at the facility.

Observation. Children were observed riding bicycles in the outdoor play area without injury prevention equipment on. The children were riding on a variety of terrains that included a pavement hill, dirt and rocks. Injury prevention equipment is required when riding bicycles to prevent the risk of head injury for children. A response is required by October 8, 2025 to address how the risk of injury in children has been mitigated.

Observation. The daily attendance records do not include the correct time the child arrived at the facility. The form has been pre-filled for afterschool care with a set time of 2:10pm for every child's arrival. Daily attendance records are a real time account of what children have been signed into care that includes the time of arrival and departure. Outline the system developed to ensure attendance records reflect the times each child is in care each day. One child who requires an epi-pen does not have a corresponding care plan to support the allergy or any additional information to identify the allergy. Care plans are required for any child that has a medical condition that may require extra support. Outline the system developed to ensure care plans are developed and shared with staff. Confirm the care plan has been developed for the child who requires an epi-pen in consultation with the parent and any other third party.

Observation. Staff files are incomplete. Two files are missing evidence of character references. One file is missing a record of the person's work history. One file is missing evidence of the person's training. Two files are missing valid first aid certificates. A licensee must not employ a person in a community care facility unless the required records have been obtained. Outline the system developed to ensure staff files are complete and on site.

Observation. A fire drill was not conducted for the month of June 2024. A record indicating each employee has practiced implementing the fire drill system once a month is required. Outline the system developed to ensure fire drills are being conducted monthly.

Observation. It was observed a group of children playing in a room near the front of the facility that was not visible from the main play area. During the inspection it was noted that staff were not actively supervising the children in this room and did not check in on the children for 40 minutes.

Observation. A sharp knife was in the dishwasher and within reach of the children. Old ceiling lights are being stored in the front play yard that are metal with sharp edges and accessible to children.

Observation. It was disclosed that children who require an epi-pen keep the pens in their backpacks. The backpacks are stored in a manner that are accessible to all children.

Observation. It was observed children were not being monitored for hand washing after using the washroom or before eating snack.

Observation. Information that can be used to readily identify the child in an emergency not available. Play space inspections are occurring however not being documented daily. Some child files are missing an emergency contact other than the parent, one file missing medical number.

Observation. Snack is provided for children daily. Information is not shared with parents regarding what food and drink has been given to children.
