A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. Sharp corners of shelving units that are child height were not covered.

Corrective action. Please cover these corners.

A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager.

Observation. The CCF Licence was not displayed.

Corrective action. The manager said it has been misplaced & she will look for it.

The licensee must (a) develop the care plan in consultation with a parent of the child requiring extra support and any person requested by the parent.

Observation. Accare plan was last reviewed on 02/Oct/2024.

Corrective action. Care plans must be reviewed w/ parents annually.

A licensee must keep, for each child, a record showing the following information: (b) date of enrolment in the community care facility.

Observation. 10 children's files were reviewed & 3 start dates were incomplete ("ASAP," month only, or blank).

Corrective action. All requirements of s.57(2) must be obtained by children's 1st day of care.

A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation) Observation (CORRECTED DURING INSPECTION): 2 employees' certificates have not been validated.

Immediately after a person who was the subject of a character reference is no longer employed by or ordinarily present at the community care facility, a licensee must return all character references to the person, or destroy the character references. Observation (CORRECTED DURING INSPECTION): A former employees' reference remained in her file.

A licensee must ensure that each piece of furniture or equipment for sleeping (b) is clean and comfortable.

Observation. Upon inspection, licensing observed children's sleeping mats were stored in a way the mats were touching each other with cover sheets on. This poses a potential cross-contamination risk.

Corrective action. The Director said she will get dividers to ensure mats do not touch each other when stored away.

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. Upon inspection, no emergency foods observed in the emergency supply kit and water was expired. This was a repeat contravention from inspection reports of JKIM-D6ZM85 (insufficient supplies identified) and CLAN-CVUMRT (insufficient supplies identified).

Corrective action. The Director said she will supply sufficient foods and water to the emergency supply kit.

Corrective action. Ensure that the name of the manager is posted in a prominent place within the facility.

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. LO observed and discussed the following with the Manager: - Not sufficient supplies to accommodate number of children and staff on site. - Fire extinguisher's is due for service.

Corrective action. Ensure that sufficient emergency supplies are available (72 hrs recommended), and service the fire extinguisher yearly.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. 7 children's files were reviewed; 1 child's MSP card number was missing.

Corrective action. Ensure that all children's files are complete.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. 7 children's files were reviewed; 2 children were missing their Dr's information, and 4 children were missing their emergency contact information.

Corrective action. Ensure that all children's files are complete.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): During the inspection Licensing observed the following: -A staff backpack was observed on the ground. This was accessible to children. This is a repeat contravention from inspection #CLAN-BH2Q3X, #CLAN-CLEN4Y -A bottle of bleach was observed on the floor in an unlocked cupboard.

Corrective action. Please ensure children do not have access to any objects or substances that may be hazardous to their health and safety.

A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.

Observation. Licensing observed the paper towels for drying children's hands are far away from the sink used for handwashing. Please ensure paper towels are placed in the washroom or directly outside the washroom to ensure children practice proper hygiene practices and have supplies readily available. This is a repeat contravention from inspection #SMOY-BRJTAH

Corrective action. Please ensure to practice proper hygiene practices.

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. Licensing observed the emergency supplies. There was insufficient food available in case of emergency. In discussion the Manager stated that the food had expired and she has not replaced it. This is a repeat contravention from inspection #CLAN-CBYN2W

Corrective action. Please ensure you have sufficient food in the emergency supplies to be prepared in a potential emergency.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

Observation. Licensing reviewed the staff records. One staff record had a criminal record check (CRC) from another Country. The serious health and safety risk to children in care was discussed with the Manager.

Corrective action. Please ensure each staff working at the facility completes and receives a clear criminal record check from the CRC review board prior to starting employment at the facility. A plan is required to be submitted by the Manager to mitigate any potential risk.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs. Observation (CORRECTED DURING INSPECTION): Immunization status was not obtained for one employee.

Corrective action. Please ensure staff records are complete

Observation. Licensing observed the following inside the facility: -Baseboards in the washroom had paint chips and signs of wear. This is a repeat contravention from inspection #CLAN-BH2Q3X and #SMOY-BRJTAH, #CLAN-CBYN2W, CLAN-C9HVST

Corrective action. Please ensure the facility is in good repair

Observation. During the inspection the following was observed outside the facility: -Rust was noted in the middle of a spool in the outdoor play area. This can cause a potential health hazard. This is a repeat contravention from inspection #SMOY-BRJTAH, #CLAN-CBYN2W, # CLAN-C9HVST, CLAN-BH2Q3W -In discussion with the Manager she states the staff wash tongs used for serving children's snack in the washroom sink. Licensing recommended that the program wash the tongs in the sink in the church kitchen or use disposable items as there could be cross contamination from the bathroom sink to the food items. This is a repeat contravention from inspection # SMOY-BRJTAH, # CLAN-C9HVST. Please correct by November 22, 2022

Observation. Upon arriving at the facility licensing observed a door to a shed was unlocked. Located in the shed on the floor was a bag of ice melter. This could pose a potential hazard to the children in care. Inside the classroom Licensing observed a bag with personal items belonging to one of the staff. This could present a potential hazard. This is a repeat contravention from inspection # CLAN-BH2Q3X and #SMOY-BRJTAH. This was corrected during the inspection.

Corrective action. Please ensure the children do not have access to any object or substance that may be hazardous to their health and safety.

If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (a) administered to the child in the amount and at the times specified by the child's parent or in the child's record or care plan.

Observation. A child at the facility requires occasional medication. Licensing reviewed the documentation and did not observe a record of the medication given to the child. In addition the medication authorization form was not signed by a parent or a medical practitioner.

Corrective action. Please ensure a medication authorization form is signed by the parent prior to administering a child's medication and the date and time of administration of medication is documented by the staff.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.

Observation. Immunization status was not obtained for one employees. This is a repeat contravention from inspection #SMOY-BRJTAH.

Observation. One staff record was missing one character reference. This is a repeat contravention from inspection #SMOY-BRJTAH

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

Observation. Licensing reviewed two staff records of four. One staff record was missing a site specific criminal record check. This is a repeat contravention from inspection #SMOY-BRJTAH.

Corrective action. Please ensure staff records are complete with current information.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. During the inspection the following was observed inside the facility: -Baseboards in the washroom and walls and ledges in the classroom had paint chips and signs of wear. This was not corrected by the date specified on the routine inspection. This is a repeat contravention from inspection #CLAN-BH2Q3X and #SMOY-BRJTAH Licensing observed the following in the outdoor play area: -The play house needs to be painted as the paint is chipping and the porous wood surface could cause splintering. This was not corrected by the date specified on the routine inspection. The Manager has discussed these repairs with the church.

Corrective action. Please ensure the facility is in good repair.

Observation. Licensing observed the following: -A porous surface was noted on large spools outside in the play area. This could cause splintering. This was not corrected by the date specified on the routine inspection. -Rust was noted in the middle of a spool in the outdoor play area. This can cause a potential health hazard. This was not corrected by the date specified on the routine inspection. This is a repeat contravention from inspection #SMOY-BRJTAH. -An electrical outlet with a power bar was missing 1 plug cover. This was not corrected by the date specified on the routine inspection. This is a repeat contravention from inspection CLAN-BH2Q3X.

Corrective action. Please ensure a healthy and safe environment is provided at all times.

Observation. During the previous inspection Licensing discussed with the Manager the need for more emergency supplies. The Manager stated she has not purchased more food for the emergency kit.

Corrective action. Please ensure to have sufficient food in the emergency supplies in case of emergency. Licensing recommends enough supplies for a 72 hour period.

During the inspection the following was observed inside and outside the facility: -the washroom walls had visible dirt. -A porous surface was noted on large spools inside the classroom and outside in the play area. This could cause splintering, -Rust was noted in the middle of a spool in the outdoor play area. This can cause a potential health hazard. This is a repeat contravention from inspection #SMOY-BRJTAH. -An electrical outlet with a power bar was missing 3 plug covers. This was corrected during the inspection. This is a repeat contravention from inspection CLAN-BH2Q3X. -A bottle of bleach and water was within easy reach on the windowsill outside the washroom. This was observed at the previous inspection and is a repeat contravention from inspection Please correct this by December 9, 2021 -Plates, bowls and cutlery were observed. In discussion with the Manager she states the staff wash these items in the washroom sink. Licensing recommended that the program wash dishes in the sink in the church kitchen or use disposable items as there could be cross contamination from the bathroom sink to the food items. This is a repeat contravention from inspection # SMOY-BRJTAH. Please correct this by December 9, 2021.

Observation. Licensing observed the following inside the facility: -Baseboards in the washroom and walls and ledges in the classroom had paint chips and signs of wear. Animals may access the sandbox as this may contaminate the sand. -The play house needs to be painted as the paint is chipping and the porous wood surface could cause splintering.

A licensee must make available to parents information on the food and drink given to children.

Observation. The program provides snack to the children in the afternoon. A snack menu was not observed. This is a repeat contravention from inspection #CLAN-BH2Q3X.

Corrective action. Please ensure parents/guardians are informed of the food and drink that is given to their children on a daily basis.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. Five children's records of twenty were reviewed. One MSP number was missing for the child's record. This is a repeat contravention from inspection # SMOY-BRJTAH. In discussion the Manager stated that the child recently arrive from another province. Another province's medical number can still be recorded and used.

Corrective action. Please ensure children's records are complete

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. Five children's records of twenty were reviewed. Two children's records were missing a medical practitioners name or phone number. Licensing recommended that if a child does not have a doctor the parent or guardian can record the closest name and phone number of a medical clinic

Corrective action. Please ensure children's records are complete

A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation)

Observation. Two out of five staff records were reviewed. One staff record was missing a verification from the Early Childhood Educator (ECE) Registry.

Corrective action. Please ensure all certificates are verified by the ECE Registry.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. 1) Licensing observed a mini fridge with a microwave stored on top of the fridge and on top of the microwave a kettle was stored. The manner in which the three items were stored is of concern as there the potential for one or all three items to fall onto an individual causing an injury. 2) Licensing saw a dish rack with dishes stored on a shelf. In discussion with the Manager there are times when staff need to wash dishes when a child doesn't have cutlery, or dishes for their snack or lunches. Staff will take the dishes to the adjacent church and run them through the dishwasher and there are times when the staff wash the dishes in the bathroom sink.

Corrective action. 1) The Manager will need to store the microwave and kettle in a manner that the items cannot be easily tipped over. This may mean placing the items side by side on a counter, removal of the items or any alternate means the Manager may chose to implement to address this concern. 2) Licensing informed the Manager that staff must stop washing dishes in the bathroom sink due to cross contamination concerns. Dishes may continue to be washed through the dishwasher or alternately the Manager may use single serve (disposable) dishes and cutlery.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

Observation. 1) During the current COVID 19 pandemic many child care facilities have hand sanitizer available for parents' use. During today's inspection hand sanitizer was located outside of the facility on a table top, on the top of a shelf beside the washroom and stored in a unsecured cupboard in the washroom. Additionally there were spray bottles of a bleach and water solution stored on the top of the shelf beside the washroom. 2) Licensing saw staff's personal belongings stored in a unsecured cupboard. This contravention was noted on the previous inspection report #CLAN-BH2Q3X dated October 17, 2020.

Corrective action. 1) Hand sanitizer, and disinfectant must be kept out of the reach of children to prevent accidental poisoning. Ensure these items are stored in a manner that are inaccessible to children. 2) Staff may have items in their belongings that pose a hazard (i.e. choking, poisoning, etc) to children. Ensure these items are stored in a manner that are inaccessible to children.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. Licensing observed: 1) in the washroom the covers for the bolts that secures the toilet to the floor are missing. The bolts are exposed and are showing signs of rust. The facility has large wooden spools, both inside and outside for the children's use. These items have exposed bolts which have become rusty. The bolts on the outdoor climbing structure have had their protective coating worn away and are starting to rust. 2) in the washroom the paint on the windowsill, and baseboards are showing signs of wear and tear and are now porous. The Manager said she had addressed this issue after the previous inspection.

Corrective action. 1) All items that are showing signs of rust must be repaired due to the risk of tetanus to both children and adults. 2) all surfaces must be non-porous for proper cleaning and sanitization. This item was previous contravened on facility inspection report #CLAN-BH2Q3X dated October 17, 2019.

A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.

Observation. During the inspection Licensing was unable to locate paper towels in the washroom or any other means to dry their hands. The Manager said the paper towels are located in the classroom half way between the two washrooms to prevent congestion in the washroom.

Corrective action. The process for washing hands includes having running water, liquid soap, and paper towels (or individual cloth towels - laundered after each use) in one area. The Manager will need to provide a means for individuals to wash and dry their hands in one location.

A licensee must keep (a) a copy of the written statement, referred to in section 19 of the Act, that the licensee delivers to persons who prepay part of the cost of services

Observation. Licensing noted on the previous inspection report #CLAN-BH2Q3X dated, October 17, 2019 the facility's repayment policy was incomplete. Upon review of the facility's policy it was noted that the policy remains incomplete. The Manager will need to revise their policy to include what procedure is for returning cheques when parents prepay the cost of the services.

Corrective action. 04 August 2020

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. Upon review of 5 children's files there was 1 file that did not contain the medical insurance plan number.

Corrective action. The family may include their international insurance information or if the child has no medical insurance the family may write in none under this heading.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. Upon review of 5 children's files 2 files did not have the name and number of a medical practitioner and 1 of the 5 files did not have a emergency contact.

Corrective action. Families may use the name of a medical clinic they use if they do not have a medical practitioner. In the case of naming an emergency contact some families are unable to list anyone other than the parents. If none of these options are applicable the family may write in none under the applicable heading. It is recommended that the Manager check in with the family in a few months to update their child's file.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (c) a record of the person's work history.

Observation. Licensing reviewed seven staff files and noted one file did not contain any resume or work history.

Corrective action. Prior to an individual caring for children the Manager must have a copy of the individual's resume or work history on file.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

Observation. Upon review of 7 staff files Licensing noted one file did not contain any character references.

Corrective action. The Manager must ensure, prior to an individual caring for children, have 3 character references on site and evidence that the Manager has verified the references.

A licensee must keep current records of each of the following: (b) for each employee, the records required under section 19 (1) [character and skill requirements].

Observation. Upon review of seven staff files Licensing noted three files contained a criminal record clearance letter that was addressed to another employer or agency.

Corrective action. The Manager must have criminal record clearance letters on file that are addressed to the Licensee. The Manager may apply on line with the Criminal Records Review Agency to have these results shared with them.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.

Observation. Upon review of seven staff files Licensing noted one file did not have the individual's immunization status on file.

Corrective action. Prior to an individual caring for children a copy of their immunization status must be maintained on site.
