A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

Observation. Paint and drano was located in the bathroom in the hallway accessible to children.

Corrective action. Ensure that hazardous objects or substances are not accessible to children in care.

A licensee must display in a prominent place in the community care facility the certificate, or a copy of the certificate, of any educator or assistant employed to work in the community care facility.

Observation. Four certificates were not posted.

Corrective action. Ensure that any educator or assistant employed to work has their certificate displayed.

Without limiting subsection (1), a licensee must ensure that (b) the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E.

Observation. The Group Child Care (under 36 month) programs did not have an infant toddler educator working with 1-4 children. There was an Early childhood educator assistant working alone with 4 or less children. There was an Early childhood educator and Early childhood educator assistant working alone with up to 8 children.

Corrective action. Ensure that staff ratio are no less than schedule E.

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. The facility did not have a written emergency plan that set out procedures to respond to and recover from any emergency. There were policies created, however, the information was not recorded in the policy.

Corrective action. Ensure that the plan is created and sets out the procedures how to prepare for, mitigate, respond to and recover from any emergency.

The licensee must (a) develop the care plan in consultation with a parent of the child requiring extra support and any person requested by the parent.

Observation. Care plan not developed in consultation with the parent or any person requested by the parent. A support plan was prepared by another agency without input from the facility

Despite this section, licensee may employ a person as a volunteer who does not provide care to children or supervise children if the licensee or the manager has first met with the person and obtained all of the following (c) evidence that the person has complied with the Province's immunization and tuberculosis control programs.

Observation. Immunization status was not obtained for one or more volunteers.

Corrective action. Ensure immunization status is obtained

Despite this section, licensee may employ a person as a volunteer who does not provide care to children or supervise children if the licensee or the manager has first met with the person and obtained all of the following (b) character references in respect of the person.

Observation. A minimum of two character references were not obtained for one or more volunteers.

Despite this section, licensee may employ a person as a volunteer who does not provide care to children or supervise children if the licensee or the manager has first met with the person and obtained all of the following (a) a criminal record check for the person.

Observation. A criminal record check was not obtained for one or more volunteers.

Corrective action. Ensure that criminal record checks are obtained

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. Two fridges in two classrooms were not maintained at the safe temperature.

Corrective action. The manager explained they will either adjust the temperature, repair or replace the fridge. Contents will be moved to the main fridge.

The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.

Observation. The care plan was not reviewed annually with the parent or any person requested to by the parent.

Corrective action. Manager explained the care plan will be reviewed again and it was noted the medication was expired.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

Observation. The following was observed during the inspection: -Plastic bags in an open container on the floor of the washroom -Two loose cords. This is a repeat contravention from inspection #CLAN-C9Y2VX, #CLAN-BTYVSW, #CLAN-BJ5PNW

Corrective action. Please ensure children do not have access to any objects or substances that could be harmful to their health and safety.

A licensee must have one toilet and wash basin for every 10 children or fewer.

Observation. Licensing observed only one toilet in the washroom for infant and toddlers. There are 12 infants and toddlers in the room. When the facility was licensed a potty chair was accepted in place of a second toilet. The potty chair was not present.

Corrective action. Please ensure to have one toilet for every 10 children. Two toilets are required for more than 10 children.

A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.

Observation. Licensing observed a staff change gloves after changing a child's diaper. The staff did not wash their hands after removing their gloves.

Corrective action. Please ensure staff engage in proper hand hygeine when changing diapers.

A licensee must display in a prominent place in the community care facility the certificate, or a copy of the certificate, of any educator or assistant employed to work in the community care facility.

Observation. Licensing observed the certificates posted on the wall. One staff certificate was not posted. This is a repeat contravention from inspection #CLAN-CLUSKP, #CLAN-BJ5PNW, #CLAN-BTYVSW

Corrective action. Please ensure staff certificates are posted in a prominent place.

A licensee must ensure that a child's medication is not accessible to any child, except that a child may have access to his or her own medication if (a) the child's parent has instructed the licensee to permit this. Observation (CORRECTED DURING INSPECTION): Medication was accessible to children in care. This was located in a child's cubby. This was corrected during the inspection

Corrective action. Please ensure medication is not accessible to children in care.

A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.

Observation. Licensing observed that a minor incident, that was discussed, was not recorded in the minor incident log

Corrective action. Please ensure to record minor incidents in the minor incident log

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

Observation. Licensing observed the following in the facility: -Wasp chemical treatment was under the sink. A lock was present but was not applied. This was removed during the inspection. -Staff bags were observed on two shelves. These were within reach of the children's in care and could contain items that could potentially be hazardous to the children's health and safety. This is a repeat contravention from inspection #CLAN-BJ5PNW, #CLAN-BTYVSW, #CLAN-C9Y2VX , #CLAN-CCBNLT. -Multiple bottles of sunblock were located in a bin on the counter within reach of the children. This is a repeat contravention from inspection #CLAN-C9Y2VX.

Corrective action. Please ensure children do not have access to any objects or substances that may be hazardous to their health and safety.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. Licensing observed the following during the inspection: -Two dents in the wall. This is a repeat contravention from inspection #CLAN-C9Y2VX -Two paper towel dispensers were jammed and not working -Duct tape was on the edge of the carpet (duct tape is difficult to clean) -Paint chips were observed on the shelves. This is a repeat contravention from inspection #CLAN-BJ5PNW and #CLAN-C9Y2VX

Corrective action. Please ensure the facility and equipment in the facility are in good repair.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. Licensing observed the following during the inspection: -A climber in the playground was placed 3 feet from the fence. A climber must have a fall zone of 6 feet. This is a repeat contravention from inspection # CLAN-CCBNLT.

Corrective action. Please ensure a healthy and safe environment is provided to the children at all times.

Observation. One staff certificate was not posted in a prominent place. This is a repeat contravention from inspection # CLAN-BJ5PNW and #CLAN-BTYVSW

Corrective action. Please ensure staff certificates are posted in a prominent place.

If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (a) administered to the child in the amount and at the times specified by the child's parent or in the child's record or care plan.

Observation. Licensing observed the contents of the first aid kit. Items with a drug identification number (DIN) were present in the first aid kit. Permission for applying this medication is required prior to application.

Corrective action. Please ensure written permission is obtained from a parent or guardian prior to the administration of any medication

Without limiting subsection (1), a licensee must ensure that (a) children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E.

Observation. Licensing reviewed the children's records for one of the rooms for children 30 months to school age. Three children present were under 36 months. A maximum of two children under 36 months is permitted in one room as outlined in the legislation.

Corrective action. Please ensure the children are divided into groups according to the legisltion.

In respect of a record referred to in this Division, a licensee must (a) keep in a single place at the community care facility the records referred to in sections 56 to 58.

Observation. Licensing observed that a staff record was not present at the facility. In discussion with the Manager she stated that she was unaware that a staff record was required for a specific employee.

Corrective action. Please ensure staff records are present at the facility prior to the employee starting work at the facility.

The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.

Observation. Licensing reviewed a care plan on file. The care plan was not reviewed annually with the parent as a date and signature were not observed on the care plan.

Corrective action. Please ensure the care plan is signed and dated by a parent or guardian to document that this has been reviewed and signed annually.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. Six children's records were reviewed. One children's record was missing an emergency contact other than the parent. This is a repeat contravention from inspection # CLAN-BY2UHS and #CLAN-BTYVSW

Corrective action. Please ensure children's records are complete.

A licensee must keep current records of each of the following: (b) for each employee, the records required under section 19 (1) [character and skill requirements].

Observation. Two staff certificates posted on the wall were expired. Current certificates were not obtained. Licensing reviewed the staff records and the current certificates were not present as they are currently in process.

Corrective action. Please ensure staff certificates are current

Observation. The following was observed at the inspection: -A children's climber was close to the fence. Please remember to keep a six feet fall distance between the climber and the fence.

Observation. Licensing observed the following during the inspection: -In one classroom staff bags and purses were within easy reach of the children. Locked cabinets have been provided to staff but these were not in use. This was not corrected by the date specified on the routine inspection. This is a repeat contravention from inspections #CLAN-BTYVSW and #CLAN-BJ5PNW. The Manager moved the bags and locked them in the cabinet during the inspection. -A chemical cleaner was observed under the sink in the washroom. A lock was present but not applied. This was removed during the inspection. -Several cans of shaving cream were in a low cabinet. A lock was present but not applied. -A cubby in the classroom was not secured. Two screws were observed behind the cubbies but these were not secured to the cubbies. This presents a potential tipping hazard. This was not corrected by the date specified on the routine inspection. This is a repeat contravention -Electrical outlets on a power bar did not have plug covers. This was not corrected by the date specified on the routine inspection. This is a repeat contravention. Upon entering a classroom Licensing observed a kettle that was open at the top and was boiling. The kettle appeared to be stuck on boil and was not shutting off automatically. No one was present in the classroom. The Manager turned off the kettle. The serious health and safety risk was discussed with the Manager.

Corrective action. Please ensure children do not have access to any objects or substances that are hazardous to their health and safety

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. During the inspection the following was observed: -Wear marks on shelves where particle board was showing. This may make it difficult to clean and could harbour bacteria. This was not corrected by the date specified on the routine inspection. This is a repeat contravention

Corrective action. Please ensure the facility is in good repair.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. Licensing observed the following: -a hole in the wall at the children's level where drywall was exposed. -a bookshelf was damaged -marks on the wall missing paint where a garbage can was placed -wear marks on shelves and a table where paint and sealant was removed could harbour bacteria and make it difficult to clean.

Corrective action. Please ensure the facility and equipment are in good repair

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

Observation. Licensing observed the following during the inspection: - in several classrooms staff purses were within easy reach of the children. Locked cabinets have been provided to the staff but some of the staff bags were in unlocked cabinets while others were at the children's level in the classroom. This is a repeat contravention from inspection # CLAN-BTYVSW and #CLAN-BJ5PNW -An ipad was proped against a narrow window ledge. Children were sitting in a circle directly below the ipad. -Several cords and two stereo cords were not secured. This is a repeat contravention from inspection # CLAN-BTYVSW and #CLAN-BJ5PNW -A cubby in the classroom was not secured. This could present a potential tipping hazard -Paint was under the sink -5 chairs were stacked over 4. 5 feet tall. This was observed int he infant room. The could present a potential falling hazard -multiple bottles of sunblock were on a low shelf. This was accessible to children. -A string of Christmas lights had a loose cord. This was within easy reach of the children -several electrical outlets were missing plug covers including a power bar with multiple outlets. This is a repeat contravention from inspection #CLAN-BJ5PNW.

Corrective action. Please ensure children do not have access to any objects or substances that are harmful to their health and safety.

A licensee must do all of the following: (b) operate the community care facility in a manner that will promote (i) the health, safety and dignity of persons in care,

Observation. During the inspection licensing observed two children being changed by two teachers in the classroom. The children were changed out of their clothes and into a CEFA uniform. One child's bare chest was exposed to the children, teachers, the Manager and the Licensing Officer who were present in the room.

Corrective action. Please ensure to change the children in the washroom to ensure you promote and preserve the child's privacy and dignity.

A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].

Observation. Licensing observed that a record of a fire drill being completed was not recorded for the month of October. In discussion the Manager stated it was completed but not recorded. This is a repeat contravention from inspection #KSEY-B8JRK6

Corrective action. Please ensure fire drills are recorded to comply with the legislation

A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (a) the diagnoses relevant to the child's requirement for extra support, as made by health care professionals.

Observation. No care plan available for a child requiring extra support.

Corrective action. Please ensure a care plan is created for each child that requires medication in an emergency.

The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.

Observation. The care plan was not reviewed annually with the parent or any person requested to by the parent.

Corrective action. Please ensure care plans are reviewed and dated once per year.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

Observation. Five staff records were reviewed. One staff record was missing character references. This is a repeat contravention from inspection #CLAN-BJ5PNW.

Corrective action. Please ensure staff records are complete.

A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation)

Observation. Five staff records were reviewed. One staff record was missing a verification from the ECE Registry. This is a repeat contravention from inspection #KSEY-B8JRK6.

Corrective action. Please ensure staff records are complete

A licensee must supply equipment, furniture and supplies that are (a) of sturdy and safe construction, easy to clean and free from hazards.

Observation. During the inspection the following was observed inside the facility: -A toilet lid, covering the inside workings of the toilet, was broken. This was held together by sticky shelf liner paper. In discussion with the Manager she stated the lid broke this week and the facility is awaiting a replacement piece. -A small table had marks in it that may harbour bacteria. This was not corrected by the date listed on the routine inspection and therefore is a repeat contravention from inspection #CLAN-BTYVSW.

Corrective action. Please ensure the furniture is easy to clean and free from hazards.

Observation. During the inspection the following was observed inside the facility: - a cupboard in one of the classrooms was not locked and bleach was present and accessible. This was not corrected by the date listed on the routine inspection and therefore is a repeat contravention from inspection #CLAN-BTYVSW. - Rust was observed on two toilet fixtures in the washroom. This could present a possible health hazard. In discussion with the Manager she stated that a maintenance person replaced the wrong piece on the toilet. Recommendations were made to the Manager. This was not corrected by the date listed on the routine inspection and therefore is a repeat contravention from inspection #CLAN-BTYVSW.

A licensee must supply equipment, furniture and supplies that are (a) of sturdy and safe construction, easy to clean and free from hazards.

Observation. Licensing observed that a small table was not sturdy and had marks in it that may harbour bacteria. In addition a portion of the counter in the washroom was splitting.

Corrective action. Please ensure the equipment and furniture are sturdy and safe.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

Observation. Licensing observed the following inside the facility: -Many bleach bottles were within easy reach of the children. This could present a possible poisoning or blinding hazard. This is a repeat contravention from inspection #CLAN-BJ5PNW -A bottle of disinfectant spray was on the counter at the children's level. This could present a possible poisoning or blinding hazard. This is a repeat contravention from inspection #CLAN-BJ5PNW -Two cupboards with bleach under the sink were left unlocked. A safety lock was present but it was not fastened. -In some of the classrooms staff bags and purses were easily accessible to children. This could present many potential hazards. This is a repeat contravention from inspection #CLAN-BJ5PNW -Rust was observed on two toilet fixtures in the washrroms. This could present a possible health hazard. This is a repeat contravention from inspection #CLAN-BJ5PNW -Several electrical outlets were exposed. This is a repeat contravention from inspection #CLAN-BJ5PNW -Several cords and a stereo were not secured. This could present a potential strangulation hazard. This is a repeat contravention from inspection #CLAN-BJ5PNW -Several classrooms contained open garbage containers with rubber gloves and paper towels. Due to the COVID-19 pandemic this could present a potential hazard. - An epipen was observed in the 0-36 month cubby area in a clear plastic bag. This could cause a serious hazard to the children's health and safety. This was corrected during the inspection. The following was observed in the outside play area: -Rust on two bolts in the play area and rust on fixtures on the stairs leading to the outdoor play area. This could present a possible health hazard.

Corrective action. Please ensure children do not have access to any objects or substances that are hazardous to their health and safety

A licensee must ensure that the entire outdoor play area is (b) constructed in a manner, and using materials, that are suitable for the age and development of the children intended to use it.

Observation. Licensing observed that the landscape fabric in one of the playgrounds was exposed. This could present a potential tripping hazard. This is a repeat contravention from inspections # KSEY-B8JRK61 and # CLAN-BJ5PNW.

Corrective action. Please ensure the playground is constructed in a manner that is free from hazards

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. -Licensing observed the following inside the facility: A full piano keyboard was placed on top of a smaller shelf.This could be a potential falling hazard. The piano keyboard was removed during the inspection. This is a repeat contravention from inspection # CLAN-BJ5PNW -A toilet brush and a pluger were accessible to the children 0-36 months of age. The toilet brush is a repeat contravention from in spection # CLAN-BJ5PNW. -stacked boxes of photocopy paper were observed in the hallway. This could present a falling hazard.

Corrective action. Please ensure a healthy and safe environment is provided at all times to the children in care.

Observation. During the inspection it was observed that two staff certificates were not posted. This is a repeat contravention from inspection # CLAN-BJ5PNW.

Corrective action. Please ensure staff certificates are posted in a prominant place.

A licensee, other than a licensee providing a care program described as Occasional Child Care, must provide each child with daily outdoor play periods unless weather conditions would make it unreasonable to do so. (4) Despite subsection (1), a licensee who provides a care program described as Occasional Child Care need not provide a program of outdoor activities. (5) Despite subsection (3), that subsection applies to a licensee who provides a care program described as Child-minding only if the program provides care for 3.5 hours or more each day.

Observation. During the inspection licensing discussed the frequency of outdoor play with the staff in each classroom. In discussion with the staff they stated that the children do not participate in outdoor play time when it is raining.

Corrective action. Please ensure children have daily outdoor play.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. Licensing observed nine children's records. Four children's record were missing a doctor's name and number. This is a repeat contravention from inspection # KSEY-B8JRK6 and # CLAN-BJ5PNW. Two children's records were missing an emergency contact other than a parent. This is a repeat contravention from inspection # CLAN-BJ5PNW.

Corrective action. Please ensure children's records are complete.

A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (b) the courses of action recommended by health care professionals to address the needs of the child requiring extra support.

Observation. During the inspection licensing observed emergency medication for one of the children in a classroom. There was no information or instructions recorded in a care plan that gave direction on when the medication was needed or how it was to be administered. The serious risk to health to safety of the child in care was discussed.

Corrective action. Please ensure a care plan is created that provides instructions on how and when to administer emergency medication.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. Licensing observed 9 children's records. Two records were missing a child's gender and two records were missing a child's MSP number.

Corrective action. Please ensure children's records are complete.

A licensee must have in writing from a parent, and maintain at the community care facility, consent (a) to call a medical practitioner or ambulance in case of accident or illness if the parent cannot immediately be reached.

Observation. Licensing reviewed nine children's records. Four children's records were missing written consent to contact a medical practitioner or ambulance in case of emergency.

Corrective action. Please ensure children's records are complete.

A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.

Observation. Licensing observed nine children's files. Three children's records were missing a photo.

Corrective action. Please ensure the children's records are complete.

A licensee must ensure that each employee (b) practices implementing the fire drill system at least once each month.

Observation. During the inspection the record of fire drills was observed. No fire drill was practiced or recorded for the month of August. This is a repeat contravention from inspection #CLAN-BJ5PNW.

Corrective action. Please ensure fire drills are practiced at least once a month.
