A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. Licensing reviewed 5 children's files of which 2 were missing the name and/or phone number of a medical practitioner

Corrective action. Staff stated they will follow up with families and complete the forms

A licensee must have in writing from a parent, and maintain at the community care facility, consent (a) to call a medical practitioner or ambulance in case of accident or illness if the parent cannot immediately be reached.

Observation. Licensing reviewed 5 children's files of which 4 were missing medical consent

Corrective action. staff stated they will follow up with families and ensure forms are all complete
