Observation. The facility minor log lacks essential details, including the time of incidents and immediate notification to parents. Furthermore, some entries document incidents of aggressive or unusual behavior that were not addressed in an individual care plan, making them reportable to Licensing. During the most recent routine inspection on September 12, 2024, both the Site Manager and Licensee Contact were present, and education and guidance were provided regarding the requirements and expectations for maintaining the facility minor log, recording minor injuries, illnesses, unexpected events, and ensuring immediate parent notifications. Failing to promptly report incidents and document required information jeopardizes children's safety by causing delays, reducing oversight, increasing repeat incidents, and ultimately compromising the facility's ability to provide a safe environment. Submit by February 25, 2026 a completed online reportable incident form detailing the aggressive or unusual behaviors documented in the facility's minor log in October 2025, including any actions taken by the facility at the time and more recently to prevent future incidents. Submit a compliance plan by March 2, 2026 detailing how the Licensee will monitor the facility's compliance with record-keeping and reporting requirements and include the methods for documenting this monitoring at the facility.

Observation. Some employee records are incomplete and lack the following information: • Criminal record clearance letters obtained through the Criminal Record Review Program • Verification that character references have been checked by the Licensee • Evidence that staff have received training in implementing emergency plans, including the use of emergency equipment Hiring employees prior to obtaining the required criminal record clearance may pose potential safety concerns. Additionally, the absence of documented reference checks and training on emergency procedures suggests that staff might not be fully prepared to respond effectively in emergencies or to maintain a consistently safe environment, which could impact the overall safety and well-being of children in care. Immediate compliance is requested ensuring any persons without adequate criminal record clearance letters do not have direct care and supervision of children in care. Submit a compliance plan by March 2, 2026, detailing how the Licensee will ensure and monitor ongoing compliance with employee record requirements and specify how such monitoring will be documented at the facility.

Observation. Not directly related to the complaint allegation received, during today's inspection staff documentation was reviewed. A person employed as a Responsible Adult does not possess the qualifications required for at least 20 hours of courses in child development, guidance, health and safety or nutrition. Please submit confirmation any staff that does not currently qualify as a Responsible Adult will not be working with children.

Observation. Not directly related to the complaint allegation received, during the investigation, the facility policy for behavioural guidance was reviewed and requires revision. Discussions with staff indicated that some were not familiar with written procedures; however, did detail the implementation of appropriate preventative and intervention strategies. Please submit a revised Behavioural Guidance Plan. Once Licensing has accepted a revised plan, the Licensee must train the facility staff on the updated plan and provide parents with a new copy.

Observation. Not directly related to the complaint allegation received, during today's inspection staff documentation was reviewed. Some records were found to be missing required records. One first aid certificate did not meet the training requirements for CCLR Schedule C and the staff was observed to be alone with children during pick up. Please submit confirmation that staff records are complete, that any staff without a criminal record clearance from the Criminal Record Review Program are not working until it is received, and that staff without valid first aid are not working alone with children.

Observation. The complaint information received alleged the following events occurred while in the care of the Licensee: (1) An employee choked a child while in the care of the Licensee. Based on the review of information gathered during the Licensing investigation; Licensing is unable to substantiate the allegation. (2) An employee made a comment to a child which threatened the child's life while in the care of the Licensee. The staff involved with the allegation admitted to making the comment in a joking manner. Licensing determined the allegation substantiated. (3) An employee shook a child while making a comment which threatened the child's life while in care of the Licensee. Based on the review of information gathered during the Licensing investigation; Licensing determined the allegation unsubstantiated. (4) An employee made a comment to a child in care which was harsh, belittling or degrading. The staff involved in the allegation, admitted to making the comment in a joking manner; Licensing determined the allegation substantiated. Licensing has based their investigation findings on information gathered which included: written statements, voice recordings, interviews, facility documentation, and Licensing observations while on-site. Please submit an action plan detailing how the Licensee will ensure compliance and appropriate staff interactions with children in order to prevent any future occurrences.

Observation. Licensing observed non compliance with staffing records at a previous inspection. The Manager has since revised systems to ensure compliance is maintained and is waiting for criminal record reviews for the staff to be received and will update Licensing once completed. Currently Licensing has accepted RCMP checks for the staff until the Criminal Record Review Program provides the reviews to the Licensee. An extension to the follow up date has been provided.

Observation. There are children attending the facility that are not eligible to attend the Group Child Care (School Age) program. This was reviewed and discussed with the Manager at the time of inspection. The Manager is to provide to Licensing by the follow up date, confirmation this contravention has been corrected. Licensing spent time observing staff supervision practices while children were engaged in indoor and outdoor activities. Washroom supervision procedures were also observed and discussed with staff. Licensing did not have concerns at the time of inspection; therefore, this part of the complaint was not able to be substantiated.

Observation. Licensing reviewed staff records. Evidence that all staff have the required education and training to work as Responsible Adults were present in the files. Therefore, this part of the concerns received in the complaint information was not substantiated. However, some required information and documents for staff were not present and files were found to be incomplete. LO King reviewed with the Manager during the inspection. Manager to submit to Licensing details regarding the system or process developed to ensure staff files are complete and on-site available for review, as well as, confirmation files have been updated and include all required documents.

Observation. Previous to today's inspection, Licensing confirmed with the Manager a reportable incident had occurred at the facility involving a child in care and had not been reported to Licensing. Licensing provided education to the Manager regarding incident reporting requirements and processes. The required incident report has been received by Licensing. The equipment involved in the incident was removed by the Manager after the incident. Licensing reviewed the equipment at the time of inspection and found them to be deteriorated. Licensing confirmed the facility is not recording inspections, repairs, or maintenance for the outdoor play area. Licensing requests the system to be developed and implemented by the facility to include the gym and gym equipment.
- C8.2 - Is each child provided a comprehensive and coordinated program of developmentally appropriate activities that encourages physical, intellectual, language, emotional, and social development?Non-critical
Observation. Licensing reviewed with the Manager the programming and activities for children in care. Currently, children are provided with a variety of free play activities indoors and outdoors. The Manager is to develop program planning that includes comprehensive and coordinated activities for all of the ages and level of development of the children attending. The Manager is to submit to Licensing by the follow up date a copy of a weekly program plan developed and details on how the program planning will occur carrying forward.

Observation. During the inspection, records and plans for emergencies were reviewed. The record for monthly fire drills was incomplete and the record for the annual emergency evacuation practice did not show previous years. The emergency evacuation plan for the facility was found to be inadequate. Licensing reviewed with the Manager at the time of inspection and provided resources to assist in the revisions for the emergency evacuation plan. The Licensee is to submit to Licensing by the follow up date requested a written plan detailing the procedures for implementing and recorded required monthly fire drills and annual emergency evacuation practices that must include evidence all staff have participated. In addition, submit to Licensing a revised emergency evacuation plan.

Observation. During the inspection, Licensing became aware that previous processes the facility implemented to inform parents what food and drink the facility provided is no longer being used. The Licensee is to submit to Licensing by the follow up date requested, evidence the contravention has been corrected.
